calling gurus--pre-cancerous needle biopsy

I will be talking with someone this weekend that had a needle biopsy report of pre-cancerous cells.

What is the prevaling school of thought on procedures and/or treatment? What questions can I recommend she ask when she meets with the onc on Tuesday?

I want to be able to give her a good sense of direction and you guys know best!

Normally the words would be "atypical cells." If there is already a lump somewhere, it can be cancer as was in my case.
What made them do the needle biopsy?

My needle biopsy (of a lump I found) turned up "atypical cells" because the needle did not hit the actual tumor (it was small) but some "atypical cells" were in the surrounding tissue. Next I had a core biopsy, which showed the very same result. My tumor was closer to my chest wall. Finally an excisional biopsy was done which included a mass in which my tumor was embedded. I had to go the whole route to get my final diagnosis.

I hope she is seeing a SURGEON who can go the next step and order further testing and/or imaging. A breast MRI may be a good screening method for this lady.

Here is what Mayo Clinic says on their web site:
"Atypical hyperplasia is thought to be part of the complex, multistep process by which breast cancer develops. The process begins when normal cell development and growth become disrupted, causing an overproduction of normal-looking cells (hyperplasia). Atypical hyperplasia occurs when the excess cells stack upon one another and begin to take on an abnormal appearance. The abnormal cells can continue to change in appearance and multiply, evolving into noninvasive (in situ) cancer, in which cancer cells remain confined to the area where they start growing. Left untreated, the cancer cells may eventually become invasive cancer, invading surrounding tissue, blood vessels or lymph channels."
In my case there was already a tumor that was affecting neighboring tissue.

MY STORY SO FAR ~~~~
Found suspicious lump 9/2000
Lumpectomy, then node dissection and port placement
Stage IIB, 8 pos nodes of 18, Grade 3, ER & PR -
Adriamycin 12 weekly, taxotere 4 rounds
36 rads - very little burning
3 mos after rads liver full of tumors, Stage IV Jan 2002, one spot on sternum
Weekly Taxol, Navelbine, Herceptin for 27 rounds to NED!
2003 & 2004 no active disease - 3 weekly Herceptin + Zometa
Jan 2005 two mets to brain - Gamma Knife on Jan 18
All clear until treated cerebellum spot showing activity on Jan 2006 brain MRI & brain PET
Brain surgery on Feb 9, 2006 - no cancer, 100% radiation necrosis - tumor was still dying
Continue as NED while on Herceptin & quarterly Zometa
Fall-2006 - off Zometa - watching one small brain spot (scar?)
2007 - spot/scar in brain stable - finished anticoagulation therapy for clot along my port-a-catheter - 3 angioplasties to unblock vena cava
2008 - Brain and body still NED! Port removed and scans in Dec.
Dec 2008 - stop Herceptin - Vaccine Trial at U of W begun in Oct. of 2011
STILL NED everywhere in Feb 2014 - on wing & prayer
7/14 - Started twice yearly Zometa for my bones
Jan. 2015 checkup still shows NED
2015 Neuropathy in feet - otherwise all OK - still NED.
Same news for 2016 and all of 2017.
Nov of 2017 - had small skin cancer removed from my face. Will have Zometa end of Jan. 2018.

Also, in addition to Steph, are they atypical cells with or without hyperplasia. Atypical cells without hyperplasia need to be watched but aren't nearly as concerning as with hyperplasia. But remember - the next step in most cases is DCIS and then a stage one cancer. However, she should insure that it isn't more than that now.

My own biopsy was inconclusive. The tumor was removed because my breast surgeon said that my tumor looked like cancer and we shouldn't have biopsied - just did a lumpectomy and get it removed. She was right.

I opted for a bi-lateral mastectomy last year and then, as we sometimes do, was second-guessing my decision after the surgery. I had two tumors in my right breast and was so afraid of the cancer that I just wanted to remove any possiblity of a recurrence. Then I got a phone call from my surgeon when she got the pathology reports - my left breast had "extensive atypical ductal hyperplasia". The surgeon said she felt sure that I would have been back to see her in as few as five years out ,having to go through the entire surgery, chemo, radiation thing again. She actually said "You dodged a bullet and made a good decision". I wouldn't think that normally there would be any treatment for ADH - just vigilant monitoring for any changes. Considering the complications I've had with the surgeries, chemo and radiation having the prophylactic mx of the left breast may have been a life-saver (one good guardian angel I've got).

4/2011 - pneumonia ??? Nope-radiation pneumonitis. No more Herc.
5/2011 - NED!!! port out.
8/2011 - clean PET & CT scans.Still NED
7/2012 - Still NED/very blessed.
2/2013 - 6 mos checkup-all clear. CA2729 down frm 13 to 11.

At the same time I was diagnosed with IDC in my left breast a biopsy of some calicifications in my right breast showed atypical hyperplasia. Then a breast MRI confirmed the cancer in the left breast, none in the right. They were going to do surgery to remove the atypical cells in the right breast at the same time as the lumpectomy or mastectomy of the left, but I ended up being diagnosed as Stage IV from the get-go, so no surgery. At the time the surgeon told me they just don't know which precancerous cells might become cancerous.

4/2/13--Biopsy of suspicious area in lower left lung. Mets to lung confirmed.
4/30/13 Begin Kadcyla/TDM-1
8/16/13 PET scan "mixed," with some areas of increased uptake, but also some definite improvement, so I'll stay on TDM-1/Kadcyla.
11/11/13 Finally get hormone receptor results from lung biopsy of 4/2/13. My cancer is no longer ER positive.
11/13/13 PET scan mixed results again. We're calling it "stable." Problems breathing on exertion.
2/18/14 PET scan shows a new lesion and newly active lymph node in chest, other progression. Bye bye TDM-1.
2/28/14 Begin Herceptin/Perjeta every 3 weeks.
6/8/14 PET "mixed," with no new lesions, and everything but lower lungs improving. My breathing is better.
8/18/14 PET "mixed" again. Upper lungs & one spine met stable, lower lungs less FDG avid, original tumor more avid, one lymph node in mediastinum more avid.
9/1/14 Begin taking Xeloda one week on, one week off. Will also stay on Herceptin and Perjeta every three weeks.
12/11/14 PET Scan--no new lesions, and everything looks better than it did.
3/20/15 PET Scan--no new lesions, but lower lung lesions larger and a bit more avid.
4/13/15 Increasing Xeloda dose to 10 days on, one week off.
7/1/15 Scan "mixed" again, but suggests continuing progression. Stop Xeloda. Substitute Abraxane every 3 weeks starting 7/13.
10/28/15 PET scan shows dramatic improvement everywhere. All lesions except lower lungs have resolved; lower lungs noticeably improved.
12/18/15 Last Abraxane. Continue on Herceptin and Perjeta alone beginning 1/8/16.
1/27/16 PET scan shows cancer is stable.
5/11/16 PET scan shows uptake in some areas that were resolved on the last two scans.
6/3/16 Begin Kadcyla and Tykerb combination
6/5 - 6/23 Horrible diarrhea from K&T together. Got pneumonia.
7/15/16 Begin Kadcyla only every 3 weeks.
9/6/16 Begin radiation therapy on right lung lesion that caused the pneumonia.
10/3/16 Last of 12 radiation treatments to right lung.
11/4/16 Huffing and puffing, low O2, high heart rate, on tiniest bit of exertion. Diagnosed as radiation pneumonitis. Treated with Prednisone.
11/11/16 PET scan shows significant improvement to radiated part of right lung BUT a bunch of new lung lesions, and the bone met is getting worse.
11/22/16 Begin Eribulin and Herceptin. H every 3 weeks. E two weeks on, one week off.
3/6/17 Scan shows progression in lungs. Bone met a little better.
3/23/17 Lung biopsy. Tumor sampled is ER-, PR+ (5%), HER2+++. Getting Herceptin and Perjeta as a maintenance treatment.
5/31/17 Port placement
6/1/17 Start Navelbine & Tykerb

Thanks for your responses, ladies!

I will know more when I have an opportunity to speak with her directly. I don't know how much info she has at this point either. I just know she is in a scarey place right now (as we all remember well) and I hope to be a help emotionally and educationally.

I would suggest that she get a BRCA testing done and talk to a genetic counselor if she's had cancer history in the family. Besides the BRCA genes, she needs to consider the HNPCC cluster (or Lynch syndrome) if cancer of the colon, endometrium, uterine, or ovarian, etc. has been recorded in her family.

The story of Nancy Brinker's Sister (Susan G. Koman) haunts me whenever I hear 'pre-cancerous' diagnosis. She had begun her first mammogram at a fairly young age because of this type of 'suspecious' results. Her life would have been saved if proper action had been taken...

My family doctor thought I was being hypochondriac when I insisted on having a prophylactic hysterectomy/oophorectomy. After the surgery, we were told (by the surgeon) that there was hyperplasia seen in the uterus already...
