now I'M scared

Ironic-feeling so calm yesterday and this morning... Just out of the shower and I see an angry rash. Wasn't there yesterday. It is exactly where it should not be - around the scar line, like the radiation "flush". Not particularly itchy. Just red from the hot shower and tiny pin pricks. Maybe this is why the Pet and CT did not agree last time. I had a pretty big bruise from acupuncture (same sternum area-between the breasts) and my onc said she spoke with the radiologist and she felt certain that the CT was right since all other signs were stable. Do I have skin lymphatic involvement?

I can't believe how yesterday I was fine and this morning the rash and I am a wreck! !**@#&#*)$*#@&$(t^!!!

1996 cancer WTF?! 1.3 cm lumpectomy Er/Pr neg. Her2+ (20nodes NEGATIVE) did CMF + rads. NED.
2002 recurrence. Bilateral mastectomy w/TFL autologous recon. Then ACx2. Skin lymphatic rash. Taxotere w/Herceptin x4. Herceptin/Xeloda. Finally stops spreading.
2003 - Back to surgery, remove skin mets, and will have surgery one week later when pathology can confirm margins.
‘03 latisimus dorsi flap to remove skin mets. CLEAN MARGINS. Continue single agent Herceptin thru 4/04. NED.
‘04 '05 & 06 tiny recurrences - scar line. surgery to cut out. NED each time.
1/2006 Rads again, to scar line. NED.

3/07 Heartbreaking news - mets! lungs.sternum. Try Tykerb/Xeloda. Tykerb/Carbo/Gemzar. Switch Oncs.
12/07 Herceptin.Tykerb. Markers go stable.
2/8/08 gamma knife 13mm stupid brain met.
3/08 Herceptin/tykerb/avastin/zometa.
3/09 brain NED. Lungs STABLE.
4/09 attack sternum (10 daysPHOTONS.5 days ELECTRONS)
9/09 MARKERS normal!
3/10 PET/CT=manubrium intensely metabolically active but stable. NEDhead.
Wash out 5/10 for tdm1 but 6/10 CT STABLE, PET improving. Markers normal. Brain NED. Resume just Herceptin plus ZOMETA
Dec 2010 Brain NED, lungs/sternum stable. markers normal.
MAR 2011 stop Herceptin/allergy! Go back on Tykerb and switch to Xgeva.
May-Aug 2011 Tykerb Herceptin Xgeva.
Sept 2011 Tykerb, Herceptin, Zometa, Avastin.
April 2012 sketchy drug trial in NYC. 6 weeks later I’m NED!
OCT 2012 PET/CT shows a bunch of freakin’ progression. Back to LA and Herceptin.avastin.zometa.
12/20/12 add in PERJETA!
March 2013 – 5 YEARS POST continue HAPZ
APRIL 2013 - 6 yrs stage 4. "FAILED" PETscan on 4/2/13
May 2013: rePetted - improvement in lungs, left adrenal stable, right 6th rib inactive, (must be PERJETA avastin) sternum and L1 fruckin'worsen. Drop zometa. ADD Xgeva. Doc says get rads consultant for L1 and possible biopsy of L1. I say, no thanks, doc. Lets see what xgeva brings to the table first. It's summer.
June-August 2013HAPX Herceptin Avastin Perjeta xgeva.
Sept - now - on chemo hold for calming tummy we hope. Markers stable for 2 months.
Nov 2013 - Herceptin-Perjeta-Avastin-Xgeva (collageneous colitis, which explains tummy probs, added Entocort)
December '13 BRAIN MRI ned in da head.
Jan 2014: CONTINUING on HAPX…
FEB 2014 PetCT clinical “impression”: 1. newbie nodule - SUV 1.5 right apical nodule, mildly hypermetabolic “suggestive” of worsening neoplastic lesion. 2. moderate worsening of the sternum – SUV 5.6 from 3.8
3. increasing sclerosis & decreasing activity of L1 met “suggests” mild healing. (SUV 9.4 v 12.1 in May ‘13)
4. scattered lung nodules, up to 5mm in size = stable, no increased activity
5. other small scattered sclerotic lesions, one in right iliac and one in thoracic vertebral body similar in appearance to L1 without PET activity and not clearly pathologic
APRIL 2014 - 6 YRS POST GAMMA ZAP, 7 YRS MBC & 18 YEARS FROM ORIGINAL DX!
October 2014: hold avastin, continue HPX
Feb 2015 Cancer you lost. NEDHEAD 7 years post gamma zap miracle, 8 years ST4, +19 yrs original diagnosis.
Continue HPX. Adding back Avastin
Nov 2015 pet/ct is mixed result. L1 SUV is worse. Continue Herceptin/avastin/xgeva. Might revisit Perjeta for L1. Meantime going for rads consult for L1
December 2015 - brain stable. Continue Herceptin, Perjeta, Avastin and xgeva.
Jan 2016: 5 days, 20 grays, Rads to L1 and continue on HAPX. I’m trying to "save" TDM1 for next line. Hope the rads work to quiet L1. Sciatic pain extraordinaire :((
Markers drop post rads.
2/24/16 HAP plus X - markers are down
SCIATIC PAIN DEAL BREAKER.
3/23/16 Laminectomy w/coflex implant L4/5. NO MORE SCIATIC PAIN!!! Healing.

APRIL 2019 - ONE DOZEN YEARS LIVING METASTATIC
MAY 2019 - progression back on herceptin add xeloda
JUNE 2019 - "6 mos average survival" LMD & CNS new single brain met - one zap during 5 days true beam SBRT to cord met
10/30/19 - stable brain and cord. progression lungs and bones. washing out. applying for ds8201a w nivolumab. hope they take me.
12/27/19 - begin ds8401a w nivolumab. after 2nd cycle nodes melt away. after 3rd cycle chest scan shows Improvement, brain MRI shows improvement, resolved areas & nothing new. switch to plain ENHERTU. after 4th cycle, PETscan shows mostly resolved or improved results. Markers near normal. I'm stunned but grateful.
10/26/20 - June 2021 Tucatinib/xeloda/herceptin - stable ish.

It sounds like this rash suddenly appeared?

Flori, you know the drill. Don't aggravate it. If it persists, get it checked out. See how it looks tomorrow. Try not to panic - sometimes a rash is just a rash.

I don't have anything to offer on the medical side. But Beau looks like he has nice big dog ears and would cuddle up and listen to your concerns and provide comfort until you can get to a doctor.

Flori...using a new soap or body wash? Heat and water and friction will intensify a rash....I know how stressed this makes you, but give it a day and see.....I am betting it is nothing but a rash, and we would be the only ones to be happy to have a plain old rash!!!!

Relax I am sure it is just a rash. I know it is hard for us to believe these things knowing what we go through, but let's tke a deep breath and wait a few before getting excited. You must remember this. A rash is just a rash. the fundamental things apply as time goes by.

Flori, I can understand the fear. You are always so brave and strong and this has really upset you.

Oh MY! This just goes to show how how keenly balanced our "teeter-totter" life is now.

Our emotions are running very close to the surface and it does not take much to shake us out of our hard-won sense of calm.

Did you put some cortisone cream on it? Whenever I get a red bump (which I do often), I just dab a little on and it goes away in a couple of days. My spring allergies are already devilling me.

If you need a punch biopsy get it asap.

MY STORY SO FAR ~~~~
Found suspicious lump 9/2000
Lumpectomy, then node dissection and port placement
Stage IIB, 8 pos nodes of 18, Grade 3, ER & PR -
Adriamycin 12 weekly, taxotere 4 rounds
36 rads - very little burning
3 mos after rads liver full of tumors, Stage IV Jan 2002, one spot on sternum
Weekly Taxol, Navelbine, Herceptin for 27 rounds to NED!
2003 & 2004 no active disease - 3 weekly Herceptin + Zometa
Jan 2005 two mets to brain - Gamma Knife on Jan 18
All clear until treated cerebellum spot showing activity on Jan 2006 brain MRI & brain PET
Brain surgery on Feb 9, 2006 - no cancer, 100% radiation necrosis - tumor was still dying
Continue as NED while on Herceptin & quarterly Zometa
Fall-2006 - off Zometa - watching one small brain spot (scar?)
2007 - spot/scar in brain stable - finished anticoagulation therapy for clot along my port-a-catheter - 3 angioplasties to unblock vena cava
2008 - Brain and body still NED! Port removed and scans in Dec.
Dec 2008 - stop Herceptin - Vaccine Trial at U of W begun in Oct. of 2011
STILL NED everywhere in Feb 2014 - on wing & prayer
7/14 - Started twice yearly Zometa for my bones
Jan. 2015 checkup still shows NED
2015 Neuropathy in feet - otherwise all OK - still NED.
Same news for 2016 and all of 2017.
Nov of 2017 - had small skin cancer removed from my face. Will have Zometa end of Jan. 2018.

Hi Flori,

It's times like these I reach for my Valium because (for me) in this type of situation only drugs can help to calm me down.

I had a rash pop up on my chest recently right after a shower. Mine was red - prickly - itchy and it freaked me out. I took a little corner of a Valium and went back to work. The next day it was gone.

Other times when I get really nervous and I know that I can't shake the fear - I go to the doctor because they are only a few blocks away.

I am sorry that you feel like a wreck today and I hope that the rash resolves soon.

I understand your fear, but it may be just a rash. Watch it, try to stay positive and if it doesn't go away have it addressed pronto.

Heck, now that I have been through chemos I have my daughter pick out my "bathing etc" items because she uses natural and the most non-allgergenic items there are. I had many scares and it is much healthier for me etc.

Hey Flori
What a teeter totter world we live in. I am hoping you get that checked out right away and not wait past Monday, if possible. I have a rash on my right breast (which is the IBC) but it is slightly raised, not pin prickly like yours. It sounds more like something you ingested yesterday, but I know how diligent you are and this kind of things can send us scurrying.
I'll be praying for you this week-end. If you can, go to Whole Foods and get some Suki Oil (the one with Myrhh in it), put some on the rash and if it is an allergy it will disappear in a day. It is the finest oil and works everytime.
Let me know how you are...I'll drive some down to you, if that's what it takes, but I'd have to leave today because I'll be in beautiful, downtown Bakerfield all week-end at a soccer tournament.

Wink now I am scared

Are you using anything new on your skin? Are you eating or drinking anything new? Are you taking any new meds or supplements? Are you wearing new clothes or using new towels? Have you been in a new environment?
The powder used as a binder in my generic blood pressure medicine caused a rash on my face and neck. The doc had to get permission from my insurance to prescribe the brand name of the blood pressure medicine. The rash disappeared after that.
Get it checked out by your oncologist, radiation oncologist or a good dermatologist if you need answers.

have no idea what what you have looks but there is an entity called radiation recall

where when you give a chemotherapy agent AFTER radiation the area radiated gets skin problems --one reason they like to give the chemo before the radiation

Certain agents are more likely to be culprits than others.

Hope you oncologist can see you soon and ease your mind.

Hyaluronic acid (HA) is a high-molecular weight polysaccharide, a glycosaminoglycan that enhances fibroblast movement and metabolism during wound healing, increasing collagen fibers forming granulation tissue. We describe a 42-year-old female, who developed radiation recall dermatitis following an allogeneic stem cell transplant from an unrelated donor (MUD-PBSCT). Her skin was treated with a topical sodium hyaluronate gel twice daily (RadiaplexRX, MPM Medical). There was marked improvement after 5 days of therapy without discontinuation of any of her therapeutic agents. At a time when the health care providers were faced with considerable treatment dilemmas, this patient demonstrated improvement in the symptoms of radiation recall dermatitis with the topical application of sodium hyaluronate and no change in her medication profile. This agent could successfully allow practitioners to treat radiation recall dermatitis without discontinuation of the recall trigger drug and thus jeopardizing the outcome of the treatment regimen.

good article on radiation recall dermatitis with list of chemo agents associated

Since the article was written there have been reports of radiation recall with avastin for pancreatic cancer, and an anti EGFR monoclonal antibody for head and neck cancer from a quick review of pubmed searching for radiation recall dermatitis

it is open access. a list of chemodrugs which could be culprits
from the article:
Docetaxel
Doxorubicin
Gemcitabine
Paclitaxel
Trimetrexate
Methotrexate
Hydroxyurea
Tamoxifen

the % of cases of radiation recall due to these each of these agents varied -- the first few on the list were around 13%, the last few on the list about 5% and those inbetween were between 5 and 13%. That is NOT the percentage of the people getting those drugs after radiation therapy that develop radiation recall dermatitis--that is a rare phenomenon, but I wonder if all cases are recognized as such and correctly diagnosed.

If your skin problem doesn't fit this at all, sorry for adding to the confusion, but hopefully I have added to the armantarium of knowledge of others, so they may not become unduly concerned if they develop a skin problem which might fit this description.

Perhaps it will also explain to many why chemo tends to be given before radiation therapy.

steady now

I want to say just try to keep the boat steady. Take comfort in anything and everything you can. You've made it through 13 years of these fears, we know you can sail through this...despite the rocky sea.

Received 6doses of Taxotere, Carboplatin with Herceptin continuing for a year...DONE.
1/28/09 Left Modified Radical Mastectomy, Right Simple Mastectomy.
Surgery pathology: No invasive carcinoma present and 17 lymph nodes removed all negative! Only small amount of carcinoma in situ in left breast.
March/April '09, Radiation to left chest wall.
Currently involved in Neratinib clinical trial.
