Re: Kadcyla - Scotland

Hi Juls, my back pain is not that bad right now, but it was bothering me a lot last night. It was more side pain, and it was so bad that it was making me feel nauseous. I finally took two Percocet and two Robax Platinum, and that numbed the pain. This morning I just took two Robax Platinum, and the pain is not bad. It never goes away though. It’s always there, it’s just numbed by the pain medication. I have a physio appointment tonight, and a massage appointment on Monday, so I hope that helps. I leave for my trip after that, and I don’t want to be in pain during my trip.

My doctor told me that even though I had some slight progression while on Perjeta/Herceptin/Taxol, it’s still working. She says it’s slowing down the growth of my lung mets, and keeping the lymph nodes in my chest stable. She doesn’t want me to transfer to T-DM1 yet, because, in Canada, once you move onto T-DM1, you can’t go back to Perjeta/Herceptin/Taxol if the T-DM1 doesn’t work. It’s not because the Perjeta/Herceptin/Taxol won’t work the second time around. It’s because our health care won’t pay for it again. My doctor wants me to stay on Perjeta/Herceptin/Taxol for as long as possible, but, I can’t take the Taxol anymore and I need a break, at least until I can get my breathing back to normal, and get rid if this back/side pain.

I’m glad you had such good test results after only two doses of T-DM1. I’m sorry you weren’t feeling well the last few days though. What types of side-effects are you getting from the T-DM1? I’ve heard that some people get normal chemo side-effects like neuropathy and shortness of breath, etc, while others get no side-effects at all. If I do transfer to this drug, I hope I get minimal side-effects so that I can continue to work.

12/15 – First mammogram
01/16 – Second mammogram and ultrasound.
01/16 – Meet surgeon and go for third mammogram, second ultrasound and biopsy. Surgeon confirms cancer in left breast and lymph nodes and sets surgery date.
01/16 – Chest scan and bone scan done– all looks good.
02/16 – Surgery - left breast mastectomy and 16 lymph nodes removed (8 had cancer).
02/16 – CT scan done – small nodules on lung but Doctor advises it’s post surgical. They will continue to monitor just in case.
03/16 – Meet radiation oncologist and find out results of Pathology Report. I’m told that I have locally advanced breast cancer, based on the size of my tumour (7 cm!) and the fact that they found cancer cells in eight lymph nodes. I’m also told that I’m HER 2 positive, with high levels of estrogen and progesterone and that my cancer is stage 3, grade 2.
03/16 – Meet oncologist and am told that my cancer is actually grade 3, and that I should have done chemo before surgery. Too late now!
03/16 – Start first of six doses of chemo (Carboplatin and Docetaxal) and Herceptin (for 18 months). I was unable to get a full dose of Docetaxal after my first treatment due to liver enzymes being high.
04/16 – Have port put in.
06/16 – Finished chemo! One month off and then I start radiation.
06/16 – Start Tamoxifen.
07/16 – First radiation treatment – 24 more to go!
08/16 – Went for Genetic Testing to see if I have the BRCA gene. Tested negative for BRCA I and II
08/16 – Radiation oncologist biopsies “scar tissue” on my chest wall. I am told that I have a local recurrence and need to have rush surgery.
09/16 – Meet surgeon who advises that I need to meet with a plastic surgeon, as they will need to do a skin graft to close me up after surgery. Meet plastic surgeon and all looks good. A surgery date is set for October 4.
09/16 – Go for rush ultrasound, bone scan, breast MRI and CT scan.
09/16 – Meet oncologist who advises that the ultrasound and bone scan results look good, and that MRI shows three small masses at surgery site, but lymph nodes are clear. Still awaiting the results of the CT scan, but we are positive it will look good.
09/16 – Get a call from my oncologist, who advises that CT scan shows small spots on my lungs, and a large lymph node in the middle of my chest. This means the cancer has spread! She looks into getting me funded for TDM-1 and cancels my surgery.
10/16 – Meet oncologist, who advises that I have to take Perjeta before I can take TDM1. I start Perjeta/Herceptin every three weeks for an indefinite amount of time, and Taxol, which I will take two weeks in a row with one week off and then two weeks in a row for 8-16 treatments. Stop Tamoxifen.
10/16 – Meet surgeon, who reviews my CT scan and advises that the spots on my lungs may not be cancer, and that he doesn’t see a lymph node in my chest. He thinks it’s a spot on my lung. I’m feeling very confused! He advises that my oncologist doesn’t want me to have surgery to remove the three small masses on my scar line, as she wants to use them as a way to determine if the treatment is working. He advises that if they have not shrunk in 6 months, he will revisit surgery.
10/16 – CEA blood test to determine Tumour markers. Results were normal (2.7). My doctor advises that this could mean two things: (1) that the treatment is working, and the tumours are shrinking, or (2), that I'm one of those people who never get elevated CEA levels. Given that some people never get an elevated CEA level, this test doesn’t seem very accurate to me! Asked for PET scan, but am told I don’t qualify.
10/16 – Brain MRI – NED!
11/16 - CA-15-30 blood test – Tumour markers are normal at 19.
11/16 – Second CEA blood test – Tumours markers are still normal at 1.6 Second CA-15-30 blood test – Tumour markers are still normal at 19
11/16 – Develop lymphedema and have to wear a sleeve
12/16 – CT Scan shows that the tumors on my lungs and the lymph node in the middle of my chest are shrinking, and that some have resolved. Also, the small masses along my scar line are no longer visible. This means the medication is working!
12/16 – Small “pimple” shows up where old tumour on chest wall was located. Doctor is going to monitor it for now.
01/17 – A second “pimple” shows up on chest wall, as well as a small lump under the skin. My doctor thinks it’s scar tissue and will monitor it for now.
03/17 – Third CEA blood test and CA-15-30 blood test – Both normal at 2.5 and 25
03/17 – CT Chest scan to see if there’s improvement to chest and lungs – results are mixed. The good news is, lymph nodes are stable, there is improvement of previous lung changes due to radiation, and the cancer has not spread to my bones, spine or liver, etc. The bad news is that while some lung nodules remained stable, others increased by 1-2 mm and new ones have shown up. All of them were less than 1cm.
03/17 – Received my genetic test results from Foundation One. It found 11 genomic alterations, 3 therapies associated with potential clinical benefit, and 13 clinical trials from which I might benefit.
03/17 – Second brain MRI –NED!
03/17 – Ultrasound done to determine size of “pimples” on my chest wall. They will follow up with a second Ultrasound in three months, to see if they are growing.
04/17 – Was told that I have developed atelectasis due to shortness of breath, which is what’s causing my back pain.
05/17 – Full body CT scan scheduled to see if I’m still “stable”.

Re: Kadcyla - Scotland

Hi Tiffany
Oh that's not good about the pain!
I hope the Physio and massage helps. Hopefully holiday should help as well - everything is better in the Sun!

I think its the same here with the drugs - more to do with cost than anything else.

So far side effects have been tired, temperature up and down, headache, nausea & today a bit of numbness. In fact just an overall strange feeling which makes me think I might be on this Immunotherapy drug (Atezolizumab) as well. I looked back at my diary notes and it seems to be about 3-6 days into cycles. Not complaining though if results good!!

Re: Kadcyla - Scotland

Thanks Juls. I hope the physio and massage helps too!

Your side-effects don’t sound that bad. They are no worse than chemo side-effects, and I think I could handle them. I think I would handle T-DM1 better than I’m handling Taxol, and up until recently, my Taxol side-effects haven’t been that bad. I really do think it’s time to change drugs. We shall see.

I hope you are getting the immuno drug as well as the T-DM1. Anything that helps you beat cancer is great!

Hi Tiffany
Yes - not too bad but its the strange feeling that goes with them. Last about a day and then goes!
Don't get me wrong - I'm not complaining! It passes!

Re: Kadcyla - Scotland

Juls--Abraxane and Taxol have the same active ingredient (Paclitaxel). The difference is the stuff they're dissolved in. Interestingly, I got an email from my oncologist yesterday saying perhaps I should try another taxane.

My Foundation Medicine testing shows a "MYK" amplification that also appeared on my Guardant 360 "liquid biopsy." I don't know what MYK is, but families with breast and colon cancer and lymphoma often have it. Drugs for it are in Phase I trials right now.

Tiffany--thanks for mentioning shortness of breath from chemo. I remember I had a bit of that before I had any detectable lung mets. Now I have multiple causes for shortness of breath. At the moment chemo is not one of them. I'm enjoying it while it lasts.

My doctors have also been conservative about changing treatments. That has worked out well for me.

2/28/14 Begin Herceptin/Perjeta every 3 weeks.
6/8/14 PET "mixed," with no new lesions, and everything but lower lungs improving. My breathing is better.
8/18/14 PET "mixed" again. Upper lungs & one spine met stable, lower lungs less FDG avid, original tumor more avid, one lymph node in mediastinum more avid.
9/1/14 Begin taking Xeloda one week on, one week off. Will also stay on Herceptin and Perjeta every three weeks.
12/11/14 PET Scan--no new lesions, and everything looks better than it did.
3/20/15 PET Scan--no new lesions, but lower lung lesions larger and a bit more avid.
4/13/15 Increasing Xeloda dose to 10 days on, one week off.
7/1/15 Scan "mixed" again, but suggests continuing progression. Stop Xeloda. Substitute Abraxane every 3 weeks starting 7/13.
10/28/15 PET scan shows dramatic improvement everywhere. All lesions except lower lungs have resolved; lower lungs noticeably improved.
12/18/15 Last Abraxane. Continue on Herceptin and Perjeta alone beginning 1/8/16.
1/27/16 PET scan shows cancer is stable.
5/11/16 PET scan shows uptake in some areas that were resolved on the last two scans.
6/3/16 Begin Kadcyla and Tykerb combination
6/5 - 6/23 Horrible diarrhea from K&T together. Got pneumonia.
7/15/16 Begin Kadcyla only every 3 weeks.
9/6/16 Begin radiation therapy on right lung lesion that caused the pneumonia.
10/3/16 Last of 12 radiation treatments to right lung.
11/4/16 Huffing and puffing, low O2, high heart rate, on tiniest bit of exertion. Diagnosed as radiation pneumonitis. Treated with Prednisone.
11/11/16 PET scan shows significant improvement to radiated part of right lung BUT a bunch of new lung lesions, and the bone met is getting worse.
11/22/16 Begin Eribulin and Herceptin. H every 3 weeks. E two weeks on, one week off.
3/6/17 Scan shows progression in lungs. Bone met a little better.
3/23/17 Lung biopsy. Tumor sampled is ER-, PR+ (5%), HER2+++. Getting Herceptin and Perjeta as a maintenance treatment.
5/31/17 Port placement
6/1/17 Start Navelbine & Tykerb

Re: Kadcyla - Scotland

Amy, for me, shortness of breath, cough and chest congestion have been my worst side-effects, and they are getting worse with each treatment. That’s why I want a longer chemo break, in order to get my breathing back to normal, and try and get rid of this back pain.

Enjoy your time off chemo while you have it. I know how nice it is to not have to deal with chemo side-effects for a little while.

Re: Kadcyla - Scotland

My oncologist agrees that chemo is a double-edged sword. It can kill the cancer, but it suppresses the immune system, which may hinder Herceptin and Perjeta in doing their job helping my body handle the cancer. Plus it's just nice to have eyebrows and a little hair, if only temporarily.

9/24/12 CT scan--No new mets. Everything stable.
3/11/13 CT Scan--two small new possible mets and odd looking area in left lung getting larger.
4/2/13--Biopsy of suspicious area in lower left lung. Mets to lung confirmed.
4/30/13 Begin Kadcyla/TDM-1
8/16/13 PET scan "mixed," with some areas of increased uptake, but also some definite improvement, so I'll stay on TDM-1/Kadcyla.
11/11/13 Finally get hormone receptor results from lung biopsy of 4/2/13. My cancer is no longer ER positive.
11/13/13 PET scan mixed results again. We're calling it "stable." Problems breathing on exertion.
2/18/14 PET scan shows a new lesion and newly active lymph node in chest, other progression. Bye bye TDM-1.

Hi Juls, it looks like I’ll be joining you and trying T-DM1 when I get back from vacation in May. I discussed my future treatment plan with my doctor yesterday, and we decided, given everything that has happened to me recently, that I just can’t take Taxol anymore, and, my doctor thinks that the Perjeta and Herceptin won’t work for me on its own without the Taxol. The T-DM1 will be a lot easier for me to take, and I will only have to go the hospital every three weeks to have it administered, so I will be there less often. This is a good drug, and I think I will have success on it. We shall see.

¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬ ¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬
04/17 – Was told that I have developed atelectasis due to shortness of breath, and a pulled diaphragm, which is what’s causing my back pain. I need to see a chiropractor physio for chest therapy.
05/17 – Full body CT scan scheduled to see if I’m still “stable”.
05/17 – Bye bye Herceptin/Perjeta/Taxol, hello T-DM1

Re: Kadcyla - Scotland

Hi Tiffany
Sorry for late reply - not been online for a few days.
Sounds like a good plan- As you say T-DM1 is a good drug, and 3 week treatment cycle is much easier to deal with. We can compare notes!
Have a great holiday.

Re: Kadcyla - Scotland

Update about Foundation One Test.

My Oncologist checked up on it ( trial info states they do this test) & according to him it's only done if you fail on the trial!
He also thinks that it is not the best time to ask the patient to have the test. I had hoped that test already done & that it might have been some help now or in the future!
I guess if I want a genetic test done I'll have to pay for it myself!

Trial Info :-

In February when offered the trial, I was told I had to decide immediately as trial closed in less than 4 weeks. Have just found out trial is still open.
It is the Kate 2 trial - Kadcyla with immunotherapy drug Tecentriq (or placebo).

Re: Kadcyla - Scotland

That's great news Juls. On another note, I haven't been reacting that great to Kadcyla. Most people have no side-effects from this drug, but not me. Of course I'd have to be different!

Hi Tiffany

I've noticed more as I've had further IV's. Now on 6th cycle. A bit tired and some stiffness/burning in feet and hands ( going to look into acupuncture for that!)
