Update After My 4th Treatment
Hello all -
I completed my 4th treatment of Her/DM1 and was ok'd by my onc to go ahead with number 5.
My CEA tumor marker has come down from somewhere at above 1200 to 295 taken this last time. The lymph node in my neck that we could feel is almost gone. My CT are still pending since I switched locations of where I get them done and my onc wants to speak with the radiologist regarding one tumor in particular that he reported showing bigger but my onc. thinks that is incorrect. Since this trial requires CT's and I can not take the Iodine contrast, they can't get the best view. I want to see if we can throw and Pet scan in there soon and see what that shows.
In anycase, my onc feels I am responding and we will continue with our plan. We are very happy about that news.
I am still getting fever right after I take my treatment which makes me feel lousy. I just had my 5th treatment last Wed. and by today the fevers are gone, which is great since I usually have them for about 6 days.
I am hoping to perk up out of this funk I have been in all year. I just seem to be tired all of the time but yet all of my blood work is perfect. I am thinking that so much chemo has taken is toll on me now. But I will pounce back, just need to get out of my house and do something fun for a change.
This seems to be a good combo of drugs and pray that all of you get a chance to get on it if you come to that point.

Dx March 2004 HER2+ er/pr+ 8/16 + lymphnodes mets to the liver Stage IV / 6 cycles of TCH and 33 rads . NED Continued Herceptin 3 years w/ Femara. Tumor markers start to rise in Jan of 2007 and swiched to Tamoxifen. August of 2007 found more mets to liver and lymphnodes outside of liver. Went on Tykerb/Xeleoda--spots on liver gone found large mass between intestines. March 2008 started TCH. Stopped Carboplatin due to allergy. Still on Taxotere and Herceptin. Nov 2008 mass is gone. 3 new spot on liver and one on spine. Radiation on spine. tykerb/gemzar for liver mets. PET in April 09 showed shrinkage still a little activity, continue tykerb/gemzar until Aug 09. PET showed new spot on my spine more on my liver and a bunch of enlarged lymphnodes in upper chest. Start Adriamycin Sep 09. more radiation on spine . April 2010- Still on Adriamycin 3wks on 1 wk off and zometa every 28 days. PET -May 2010 showed progression. Starting Herceptin/Navelbine on June '10

I've been waiting (patiently!) for your update as I knew it was time for scans...
It sounds like good news and I am delighted! I would also, if I were you, push for a PET scan, especially if it is difficult to get good views with a CT.
I hope this news has you feeling better. I can relate to the fever and feeling yucky. Although it sounds like it is hitting you even harder, I do feel crummy for several days and even worse, I feel emotionally down - probably because not feeling well physically makes me thing about the enormousity (is that a word) of the battle.

Hi Irene,
I'm so glad to hear that the trial is working. It's interesting that you and Chrisy are reporting fevers. I am somewhat tired after spending so much time at the Dana Farber after teaching all day. I don't get home until about 8:00 in the evening. The next day I am somewhat tired at school, but I still feel fit enough to work. Although my dosage is higher than the phase two trial, I wonder if the weekly trial is easier to handle.

Sept'09~ new 6mm & 1 cm spots in pituitary/optic chiasm area. Rx= 25 days of 3D conformal fractionated targeted IMRT to the tumors.
Oct'09~ 25 days of low dose 3D conformal fractionated targeted IMRT to the bone mets spot on rt. iliac crest that have been watching for 2 years. Added daily Aromasin back into treatment regimen.
Apr'10~ Brain MRI clear! But, see new small spot on adrenal gland. Change from Aromasin back to Tamoxifen.
June'10~ Tumor markers (CA15.3) dropped from 37 to 23 after one month on Tamoxifen. Continue to monitor adrenal gland spot. Remain on Tykerb/Herceptin/Tamoxifen.
Nov'10~ Radiate positive mediastinal node that was pressing on recurrent laryngeal nerve, causing paralyzed larynx and a funny voice.
Jan'11~ MRI shows possible activity or perhaps just scar tissue/necrotic increase on 3 previously treated brain spots and a pituitary spot. 5 days of IMRT on 4 spots.
Feb'11~ Enrolled in T-DM1 EAP in Denver, first treatment March 25, 2011.
Mar'11~ Finally started T-DM1 EAP in Denver at Rocky Mountain Cancer Center/Rose on Mar. 25... hallelujah.

I'm very happy to hear that your tumor markers have dropped significantly and that your tumor and neck node are shrinking.
I'm not sure what a PET/CT would show, but you'd have to get more clarification from your onc. The PET part lights up a malignant nodule at about 1 cm, to be reliable although in some cases it could be as small as 7 mm). The CT scan shows nodules as small as 2-3 mm, but I understand your point about the contrast. Also, for a combined PET/CT, which is nuclear imaging with the FDG isotope, the CT part is without contrast anyway. It serves as a roadmap for the PET part, giving more definition to all the organs.
I know you've had a lot of treatments and it's a lot to tolerate. It's wonderful that the smart bomb seems to be working really well.

Diagnosed stage 2b in July 2003 (2.3 cm, HER2+, ER-/PR-, 7+ nodes). Treated with mastectomy (with immediate DIEP flap reconstruction), AC + T/Herceptin (off label). Cancer advanced to lung in Jan. 2007 (1 cm nodule). Started Herceptin every 3 weeks. Lung wedge resection April 2007. Cancer recurred in lung April 2008. RFA of lung in August 2008. 2nd annual brain MRI in Oct. 2008 discovered 2.6 cm cystic tumor in left frontal lobe. Craniotomy Oct. 2008 (ER-/PR-/HER2-) followed by targeted radiation (IMRT). Coughing up blood Feb. 2009. Thoractomy July 2009 to cut out fungal ball of common soil fungus (aspergillus) that grew in the RFA cavity (most likely inhaled while gardening). No cancer, only fungus. Removal of tiny melanoma from upper left arm, plus sentinel lymph node biopsy in Feb. 2016. Guardant Health liquid biopsy in Feb. 2016 showed mutations in 4 subtypes of TP53. Repeat of Guardant Health biopsy in Jana. 2021 showed 3 TP53 mutations, BRCA1 mutation and CHEK2 mutation. Invitae genetic testing showed negative for all of these. Living with MBC since 2007. Stopped Herceptin Hylecta (injection) treatment in March 2020. Recent 2021 annual CT of chest, abdomen and pelvis and annual brain MRI showed NED. Praying for NED forever!!

If your body is tired of chemos and treatment that would not be a big surprise!
You have been the epitome of the Energizer Bunny all these years, plus continuing to work.
Now you have shared some great news. That TM number is dropping like a rock and I know the new numbers sound good to you, too!
How often are your tumor markers checked?? When I was on active treatment for my liver they checked my markers with my bloodwork every week. The good numbers comforted me a lot when I was feeling whipped from the treatment.

MY STORY SO FAR ~~~~
Found suspicious lump 9/2000
Lumpectomy, then node dissection and port placement
Stage IIB, 8 pos nodes of 18, Grade 3, ER & PR -
Adriamycin 12 weekly, taxotere 4 rounds
36 rads - very little burning
3 mos after rads liver full of tumors, Stage IV Jan 2002, one spot on sternum
Weekly Taxol, Navelbine, Herceptin for 27 rounds to NED!
2003 & 2004 no active disease - 3 weekly Herceptin + Zometa
Jan 2005 two mets to brain - Gamma Knife on Jan 18
All clear until treated cerebellum spot showing activity on Jan 2006 brain MRI & brain PET
Brain surgery on Feb 9, 2006 - no cancer, 100% radiation necrosis - tumor was still dying
Continue as NED while on Herceptin & quarterly Zometa
Fall-2006 - off Zometa - watching one small brain spot (scar?)
2007 - spot/scar in brain stable - finished anticoagulation therapy for clot along my port-a-catheter - 3 angioplasties to unblock vena cava
2008 - Brain and body still NED! Port removed and scans in Dec.
Dec 2008 - stop Herceptin - Vaccine Trial at U of W begun in Oct. of 2011
STILL NED everywhere in Feb 2014 - on wing & prayer
7/14 - Started twice yearly Zometa for my bones
Jan. 2015 checkup still shows NED
2015 Neuropathy in feet - otherwise all OK - still NED.
Same news for 2016 and all of 2017.
Nov of 2017 - had small skin cancer removed from my face. Will have Zometa end of Jan. 2018.

Thanks for updating us - it sounds like you are responding well to the treatment.
I'm sure it's a drag for you, no wonder you have been in a funk for awhile - but brighter days are ahead, I'm sure.
