Oh Lily, I am so sorry, and so surprised - it seemed like your doctor has so far been very creative and willing to try the "new and different". I think it sounds like HE may be out of ideas - but someone else may see different options. Maybe as he has seen several treatments fail or work only a short while, he believes something completely different will be a better shot.

You have gotten good input so far - another opinion, take someone along. I'd add, write down all your questions and try to get more adequate explanations. Specifically, WHY you cannot try Herceptin with another agent - my doctor specifically still considers Herceptin and Tykerb as options to pair with future chemos, even though both have "failed" to hold the cancer without chemo. You may still get some synergy adding Herceptin.

And WHY no taxanes, especially if taxotere worked for you in the past. There is no "lifetime max" on those as with Adriamycin, and it actually is very common to try another taxane (taxol or abraxane) in your situation.



On the other hand, remember that Navelbine is strong medicine, even as a single agent. Glad you are at least getting that - and that you found out quickly that the Tykerb/Xeloda was not doing the trick.

I do not have much knowledge on the estrogen stuff...but would be interested in you seeing "the good data" that it interferes with chemo.

Lily, I think of you constantly. You are a strong fighter and advocate for yourself. I know you are frustrated (with good reason) but do not give up. There are still options that can work for you and get that #$*($&^cancer back in control.

Dear Lily

I am so sorry that I didn't read this sooner. I am not even going to read everyone else's responses - I am justing responding.

First, breathe. Then call the cancer center you go to and ask them to make you a full copy of your chart. Second, call your oncologist. Even IF Herceptin or Tykerb don't seem to be working for you, maybe they will help the Navelbine along. I think you should get another opinion on this but Herceptin won't hurt you.

Also, if a drug has worked for you before (like Taxotere), it can work again if it has been 6 months since the last infusion - so you can try it again.

Make another appointment with another oncologist as soon as you can. You need to find someone who you can be a partner with. Someone who understands that anti Her2 therapies and antihormonals work together with or without chemo to keep cancer in check. You should be trying Faslodex shots.

Perhaps it is not the Herceptin or the tykerb that wasn't working but that Tamoxifen or no antihormonal was working and you need a change there (like Faslodex). But above all, you need someone who will try to work with you and work with you logically. If you are Her2+ you need a drug for that. If you are hormone positive, you need a drug for that. And sometimes you need chemo to smack back the cancer harder.

You are stronger than the cancer and stronger than your current oncologist and you CAN do this. You can, you can, you can and you know you can.

I like Becky's advice. And many others gave really good feedback and very good ideas. Me personally - I'd like to call your oncologist and have a little chat fest with him. What an idiot.

So - you are going to make a plan and move forward. Dr Glasby at UCLA explained to me that if Tykerb didn't work he thought it should be combined with herceptin. He's a big believer in synergy and putting more than one roadblock in the way of her2.

I will say that I feel the one thing you MUST do is have someone with you who understands cancer-world. It will make a HUGE difference in how you feel and also it will eliminate any language issues. It's so hard not to panic - try to remember that we are all here with you.

1996 cancer WTF?! 1.3 cm lumpectomy Er/Pr neg. Her2+ (20nodes NEGATIVE) did CMF + rads. NED.
2002 recurrence. Bilateral mastectomy w/TFL autologous recon. Then ACx2. Skin lymphatic rash. Taxotere w/Herceptin x4. Herceptin/Xeloda. Finally stops spreading.
2003 - Back to surgery, remove skin mets, and will have surgery one week later when pathology can confirm margins.
‘03 latisimus dorsi flap to remove skin mets. CLEAN MARGINS. Continue single agent Herceptin thru 4/04. NED.
‘04 '05 & 06 tiny recurrences - scar line. surgery to cut out. NED each time.
1/2006 Rads again, to scar line. NED.

3/07 Heartbreaking news - mets! lungs.sternum. Try Tykerb/Xeloda. Tykerb/Carbo/Gemzar. Switch Oncs.
12/07 Herceptin.Tykerb. Markers go stable.
2/8/08 gamma knife 13mm stupid brain met.
3/08 Herceptin/tykerb/avastin/zometa.
3/09 brain NED. Lungs STABLE.
4/09 attack sternum (10 daysPHOTONS.5 days ELECTRONS)
9/09 MARKERS normal!
3/10 PET/CT=manubrium intensely metabolically active but stable. NEDhead.
Wash out 5/10 for tdm1 but 6/10 CT STABLE, PET improving. Markers normal. Brain NED. Resume just Herceptin plus ZOMETA
Dec 2010 Brain NED, lungs/sternum stable. markers normal.
MAR 2011 stop Herceptin/allergy! Go back on Tykerb and switch to Xgeva.
May-Aug 2011 Tykerb Herceptin Xgeva.
Sept 2011 Tykerb, Herceptin, Zometa, Avastin.
April 2012 sketchy drug trial in NYC. 6 weeks later I’m NED!
OCT 2012 PET/CT shows a bunch of freakin’ progression. Back to LA and Herceptin.avastin.zometa.
12/20/12 add in PERJETA!
March 2013 – 5 YEARS POST continue HAPZ
APRIL 2013 - 6 yrs stage 4. "FAILED" PETscan on 4/2/13
May 2013: rePetted - improvement in lungs, left adrenal stable, right 6th rib inactive, (must be PERJETA avastin) sternum and L1 fruckin'worsen. Drop zometa. ADD Xgeva. Doc says get rads consultant for L1 and possible biopsy of L1. I say, no thanks, doc. Lets see what xgeva brings to the table first. It's summer.
June-August 2013HAPX Herceptin Avastin Perjeta xgeva.
Sept - now - on chemo hold for calming tummy we hope. Markers stable for 2 months.
Nov 2013 - Herceptin-Perjeta-Avastin-Xgeva (collageneous colitis, which explains tummy probs, added Entocort)
December '13 BRAIN MRI ned in da head.
Jan 2014: CONTINUING on HAPX…
FEB 2014 PetCT clinical “impression”: 1. newbie nodule - SUV 1.5 right apical nodule, mildly hypermetabolic “suggestive” of worsening neoplastic lesion. 2. moderate worsening of the sternum – SUV 5.6 from 3.8
3. increasing sclerosis & decreasing activity of L1 met “suggests” mild healing. (SUV 9.4 v 12.1 in May ‘13)
4. scattered lung nodules, up to 5mm in size = stable, no increased activity
5. other small scattered sclerotic lesions, one in right iliac and one in thoracic vertebral body similar in appearance to L1 without PET activity and not clearly pathologic
APRIL 2014 - 6 YRS POST GAMMA ZAP, 7 YRS MBC & 18 YEARS FROM ORIGINAL DX!
October 2014: hold avastin, continue HPX
Feb 2015 Cancer you lost. NEDHEAD 7 years post gamma zap miracle, 8 years ST4, +19 yrs original diagnosis.
Continue HPX. Adding back Avastin
Nov 2015 pet/ct is mixed result. L1 SUV is worse. Continue Herceptin/avastin/xgeva. Might revisit Perjeta for L1. Meantime going for rads consult for L1
December 2015 - brain stable. Continue Herceptin, Perjeta, Avastin and xgeva.
Jan 2016: 5 days, 20 grays, Rads to L1 and continue on HAPX. I’m trying to "save" TDM1 for next line. Hope the rads work to quiet L1. Sciatic pain extraordinaire :((
Markers drop post rads.
2/24/16 HAP plus X - markers are down
SCIATIC PAIN DEAL BREAKER.
3/23/16 Laminectomy w/coflex implant L4/5. NO MORE SCIATIC PAIN!!! Healing.
APRIL 2016 - 9 YRS MBC
July 2016 - continue HAP plus Xgeva.
DEC 2016 - PETCT: mets to sternum, lungs, L1 still about the same in size and PET activity. Markers not bad. Not making changes if I don't need to. Herceptin/Perjeta/Avastin/Xgeva

December 2017 - Progression - gonna switch it up
FEB 2018 - Kadcyla 3 cycles ---->progression :(
MAY30th - bronchoscopy, w/foundation1 - her2 enriched
Aug 27, 2018 - start clinical trial ZW25
JAN 2019 - ZW25 seems to be keeping me stable
APRIL 2019 - ONE DOZEN YEARS LIVING METASTATIC
MAY 2019 - progression back on herceptin add xeloda
JUNE 2019 - "6 mos average survival" LMD & CNS new single brain met - one zap during 5 days true beam SBRT to cord met
10/30/19 - stable brain and cord. progression lungs and bones. washing out. applying for ds8201a w nivolumab. hope they take me.
12/27/19 - begin ds8401a w nivolumab. after 2nd cycle nodes melt away. after 3rd cycle chest scan shows Improvement, brain MRI shows improvement, resolved areas & nothing new. switch to plain ENHERTU. after 4th cycle, PETscan shows mostly resolved or improved results. Markers near normal. I'm stunned but grateful.
10/26/20 - June 2021 Tucatinib/xeloda/herceptin - stable ish.

Lily, I really don't post that often, but I really think the time has come, when you have to kick a** and take names later. Dump this Onc and find some one who will go to bat for you. I hope nobody takes offense at the language, but it totally pi**es me off when we are treated like this. You go get a new onc. Lily you are a wonderful woman and should not be treated this way. I wasn't listed as a "smart girl" either, even though Sassy said I was an honorary Sister. In retrospect, maybe it's because I'm neither smart with chemo. or a girl, so that makes sense. Lily, you stay strong!!!

Thank you all! It has made us feel stronger about our decision to go with another Onc. We've been considering it for some time and, with the recent setup back, you gals (and guys [bill]) have helped us feel better about our choice. The language is not really the barrier here but the attitude that it's another patient or that the patient is to much trouble. Or knows to much.

Furthermoore - Thank you for your time in replies, every single one, it means alot to me. I love you all of you from the bottom of my heart.

mastectomy right breast chemo before surgery herceptin/carboplatin/taxotere ,clear and surgery have radiation 20, `& then herceptin and tamoxifen
NED until Aug/07 body only then 'n June 04-06-07 .1 lesion of 1.6 cm on cerebellum ...novalis ,open sugery
5m.m brain met again novalis, 4mm.In the liver. Waiting 2 months now 3 tumors enroll on T-MCC trial start first infusion Nov 5/07 at Dec 17 scan show one tumor despair the 2nd and 3th diminish Doc said great results until March/08 ct scan show progression
03-05-08 start tykerb & xeloda

Sweet TigerLily, We all love you. I just wanted to add my name to the petition! I second everything said. GET A NEW ONC (maybe Sassy's). KICK HIM TO THE CURB. I am so displeased with his attitude and his seemingly poor opinions, I have no words... And I speak English pretty well. Get over him. Listen to Sheila and Becky! And everyone on the long list of smart girls (and bros). Pretty much everyone who chimed in. I won't reiterate. Just want you to know I ECHO, ECHO, ECHO *everything* they all said!!!!!!!

Sending prayers and love to one of the dearest people it's ever been my honor to meet... Do not despair. It is time to move on. Please keep us posted, as we are all with you and your beloved family in this fight, Lily.

'95 post-meno dx Invasive LOBULAR w/9cm tumor! YIKES + 2/21 nodes. Clear mammo 10 mnths earlier. Mastec/tram flap reconst/PORT/8 mnths chemo (4Adria/8CMF). Borderline ER/PR. Tamoxifen 2 yrs. Felt BLESSED. I could walk and talk, feed and bathe myself! I KNEW I would survive...

'98 -- multiple mets to liver. HER2+ 80%. ER/PR- Raging, highly aggressive tumors spreading fast. New PORT. 9 mnths Taxotere Fought fire w/fire! Pronounced in cautious remission 5/99. Taxotere weekly for 6 wks, 2 wks off -- for 9 mnths. TALK ABOUT GRUELING! (I believe they've altered that protocol since those days -- sure hope so!!)
+ good old Vit H wkly for 1st 3 yrs, then triple dosage ev 3 wks for 7 yrs more... The "easy" chemo, right?! Not a walk in the park, but not a freight train coming at 'ya either...

Added Herceptin Nov '98 (6 wks after FDA fast-tracked it for met bc). Stayed w/Vit H till July '08! Now I AM FREE! Humbly and eternally grateful for this life-saving drug! NED since '99 and planning on keeping it that way. To hell w/poor prognosis and nasty stats! STOPPED VIT H JULY '08...! REMAIN STABLE... Eternally grateful...Yes is a world & in this world of yes live (skillfully curled) all worlds ... (e e cummings) EVERY DAY I BEAT MY PREVIOUS RECORD FOR # OF CONSECUTIVE DAYS I'VE STAYED ALIVE. Smile KNOWING you too can be a miracle. Up to me and God now...

I agree with the group....you need someone who wants to try harder! I'm angry at the doctor for not treating you better. I also understand your frustration - I'm feeling the same way about my oncologist. I read about all the people on here that have doctors who listen to them and really seem to care. Please keep us updated on how you make out.

The onc I recommended is the one Sheila, Joe, Christine and others met in San Antonio. When he left me, he told me that everything that we had done was exactly what he would have wanted his wife to do had she been in my shoes. So you know he will treat YOU. You will be a team. He will also talk to you, explain, discuss etc. My visits with him were always long, because we talked about any new BC treatment developments since my last visit. When I shared info from this site with him, he followed up on it. As a bonus, his wife is an endocrinologist, and they must surely share knowledge.

3/17 Stop Evista--unwelcome side effects!

You have already had great advices from our sisters and brother. I am glad you decided to look for another oncologist because yours sounds like my former oncologist with whom I had a lot of problems and who asked me 'Who is the doctor?" when I insisted on having Herceptin after mastectomy instead or with radiation. I did what he wanted, and I regret so much, because I stayed without chemo for 3 months, and I went from Stage III to Stage IV as the cancer spread to my liver and lungs while I was on radiation treatment. Nowadays I would run away from any oncologist who had this kind of attitude.
I also had a great response to Taxotere when I was diagnosed, so I asked my oncologist if I could use it again in case I needed it, and he said he would try Abraxane/Herceptin instead (another Taxane). Why can't your doctor try it too? Also, I had Nalvabine for 18 months combined with Herceptin for my liver and lungs mets with great results. I do not see why your doctor is not willing to try this combination. It does not make sense to me. If you are Hers2+, you do need more than just Nalvabine.
I am praying that soon you will find a great oncologist, maybe Sassy's, with a good treatment plan that will fight for you.
