The distinction they make has to do with "heart age." Many bc txs that are part of standard tx (rads and chemo, maybe AI's) are cardiotoxic. My interpretation is that early stage bc patients with "older hearts" are put at greater risk of cardiovascular events as a result of bc tx and are thus more susceptible to cardiac disease than the general population, without knowing whether or not the cardiotoxic tx was either effective or necessary.
