5/12 Brain,cervical lumbar clear/thoracic slight decrease
10/12 t 2-4 shrunk t-9 grew start Xeloda, 02/13 stop xeloda,5/13 on metformin, decadron, Tykerb, iv and IT herceptin 5/30/13 total #11 #2 of 80mg dose weekly.
9/13 100mg of IT her, IV hern, 750 mg tykerb, 3mg dec.
last Mri T--3 SHRUNK t7-9 shrunk no edema. Left shift in CBC bone marrow BX negative.
10/13 Ct has shown Double left ureters with stones/cysts in them, after 3 births and lots and U/S iit takes cancer to figure out you have 2 smaller ureters going into 1 kidney!
12/13 Mri brain no new lesions, cervical and lumbar arthritis.
Tspine lesion at T3 stable, T 7-9 GROWTH lots of pain
1/29/14 HIHO HIHO its off to Neuro surgery I go

Vogel, NY is a very welcome contributor to question sessions after presentations at ASCO meetings
He is insightful and down-to-earth and never forgets that results only means as much as they help those suffering.
Yes, Roche (purposely it semms) semms not to have choseb the best treatment for the alternate arm to compare TDM1 to
I hope this does not stop TDM1 from being approved for those it could help

It is a really interesting article but I felt quite dismissive of the plight of women with metastatic herr2 disease for whom herceptin has not been the golden panacea. I don't think any of us really imagine that TDM1 is that and in that regard pertuzamab is so promising but... For this to be a chronic disease as he says, we need weapons in our arsenal and TDM1 with less side effects is certainly worthwhile at that. It worries me that they see the numbers of her2 women getting metastatic diseases shrinking as adjuvant therapies are more successful that those of us for whom they a not will be left behind. I guess though that they always seem to test the new stuff on us (nothing to lose hey?) first so that will help - apart from most of the vaccines it seems. While Emilia was the first study, Marianne surely must have some prelim results coming and th3resa too.
Still a very interesting and insightful article.

(L) skin sparing mastectomy with tissue expander, axilla clearance (2/14 affected) clear margins.
Fec*3, Taxotere and herceptin*2 - stopped due to secondary diagnosis
June 24 2011 Stage IV - Skin met, axilla node, multiple lung lesions
Bolero3 trial - Navelbine, Hereptin weekly, daily Everolimus/Placebo
February 2012 - July 2012 Tykerb and Xeloda - skin mets resolved, Lungs initially dramatically reduced but growing again
August 2012 (turn 40!) tykerb and herceptin (denied compassionate use of TDM1) while holidaying in Italy!
September 2012 - January 2013 TDM1 as part of the Th3resa trial - lymph nodes resolved, lungs slowly progressing.
January 2013 - herceptin, carboplatin and Perjeta (compassionate access)
April 2013 - Some progression in lungs and lymph nodes - Abraxane, Herceptin and Perjeta
July 2013 - mixed response - dramatic reduction of most lung disease, progression of smaller lung nodules and cervical and hilar nodes - ? Add avastin.

"NEW" weapons in arsenal, indeed very important, maybe even more important is technology incorporated in these state-of-the-art weapons continuous to be improved upon; so new agents of today,become the foundation for even more efficacious treatments in the future.
According to article, T-DM1 once approved, will be prescribed by many of the ASCO attending oncologists.

01/12 ~ Malignant tumors found in uterus, cervix, fallopian tubes and lymph nodes. Dx as endometrial cancer. Stage III2c
02/12 ~Hysterectomy(all tumors removed). Back to NED.
02/12 ~ Final Herceptin treatment.
03/12 ~ Began Cisplatin/Adriamycin tx for endometrial cancer.
03/12 ~ Tumors dx her2 metastisis, not entdometrial cancer. Back to BC tx.
03/12 ~ CT scan shows NED. :)
04/12/~ Began Tykerb/Xeloda.
06/12 ~ Ended Xeloda. Continuing Tykerb. Still NED.
09/12 ~ PET/CT scans show NED. : )
04/13 ~ Rash on original breast biopsied as cancer.
05/13 ~ Surgery to remove skin and tissue around rash. Continue Tykerb.
06/13 ~ PET/CT scans show NED : )
11/13 ~ Jaundice eyes and skin. CT scan show mets to liver as well as peritoneal carcinomatosis with malignant ascites. Began Abraxane/Herceptin tx.
02/14 ~ CT scan shows NED. :)

Glad to read that MJ's latest scan showed NED. Now that you have read this article I hope you discuss with the Onc adding Herceptin to the Tykerb, when you are stage IV the article drives home the point that we should never go without it.

weekly taxotere,herceptin, xeloda
Sept 2004 NED-3 herceptin, zoladex,aromasin
Dec 2006 recurrence in liver
Feb. 2007 liver resection left lobe removed-herceptin, zoladex, switch to Arimidex
NED 16 months added zometa
May 2008 new lesion in liver 15mm Tykerb/Xeloda/Herceptin
July 2008 stable...yeah!
Sept 2008 NED again !!!
Jan 2009 fell off the wagon again spot back in the liver and fell out of menopause.
Feb 2009 RFA and 2nd liver resection to remove spot ---back on the NED wagon again continue Tykerb, Herceptin.
March 2009- oophrectomy added Femara and bi-annual Zometa
May 2009- scans clear but suspect lung nodule
June 2009- Lung VAT wedge resection to remove nodule---fungus ball not cancer!! phew
Aug 2009- NED
Nov 2009-scans again clear YAHOO!
March 2010- scans clear continue Tykerb, Herceptin, Femara, Zometa Q6mo
Nov 2009-Nov 2019 scans clear done every 6 months
Feb 2020 - Fell out of the NED wagon hard! single liver lesions and large infect cyst. 3 weeks in ICU.
March 2021- 5 cycles perjeta, herceptin, navelbine. lesion stable.
June 2021 - 3rd liver resection to remove single liver lesion. Continued perjeta, herceptin.
Nov. 2021- PET scan show 5 hot nodes near liver. 9 cycles Kadcyla
June 2021- PET scan shows progression. nodes size unchanged but even more SUV uptake.
July 2021- start ENHERTU

This doc is a prime example of a nitpicker or bean -counter, forever analyzing , even drugs they are not familiar with. So " scientific ' they end up being " un -scientific ', ignoring the facts . The Internet is both a blessing and a curse , as we try to wade through every ' tom , dick and harrys " opinion. Tyk/xel. is the latest APPROVED combo for Late Stage IV Her2 + Survivors, and makes great sense as a comparator to T DM-1. he wants to do more trials ? w/ taxol ?, navelbine ? how about gemcitabine, platinum salts ?,
This " NY Doc " ( that puts him in the company of thousands , right ? ), sounds just like This FDA. If he isn't using T DM-1 in his practice by this time next year, he won't have many metatstatic Her2 Pts in his practice. Mark my words.
