Mammogram insurance requirements

Back in the early 1990's, there was a very sucessful movement to obtain state enactment of mandated insurace coverage for screening mammograms. Today, the only state lacking such a mandate is Utah. This means that private health insurers regulated in all states except Utah are required to include this coverage in their policies. The change in recommendations does not alter state laws which mandate coverage. The only plans exempted are self-insured plans.

Medicare, covers mammograms but is a creature of federal, not state law, so there is not mandatory coverage. Medicaid covers mammograms according to guidelines from the Centers for Disease Control and Prevention, and is also not subject to state law.

Just thought this would give some context to the discussion.
