				MBC Triple positive's here's an exciting possibility
			 
 Background:  I have recently been diagnosed with stage 4 mbc and to the lungs.  Whereas I was triple positive previously I am now her2-.  As such I am taking a new CDK4/6 inhibitor Palbociclib (Ibrance) along with Letrozole.

I have been lurking and reading posts by members on the community.breastcancer.com Ibrance topic and I saw a woman who was originally diagnosed in 2002 who has stage 4 bone mets which are Her2+.  Imagine my surprise.  I texted her to inquire as to how she is taking a drug only recommended for her2-'s and here is her reply which I found very exciting 
 
"hi Cathy - I used to hang out at her2support.org the first couple of years after my original dx in 2002. i was so sad to see that christine passed. 😢 She is the one who inspired me to get herceptin off label 4 years before it was approved for early stage BC.
 
I’m so sorry to read about your MBC diagnosis. I am only a few months into this with bone mets. So regarding Ibrance (I take it with Faslodex) my doctor said that it was only studied in Her2- people to rule out the Her2 variable and fast track it. They are starting trials now for Her2+ and he said we have no reason to believe that it won’t also work for the triple positive people as it really is targeting the hormone positive side of the equation. So he said let’s try to run it through insurance and a see if they notice.... they did not so we are rolling with it!"

 To begin new drug Palbociclib (Ibrance) along with Letrozole May, 2018.
 Genetic testing of ovarian tumour and this new lung met will take months.
 To see geneticist to be retested for BRCA this week....still BRCA VUS

				__________________11/17/10 - Diagnosed with 4.5cm tumor in right breast, IDC, Stage 2, Nuclear grade 2, ER+ PR+ HER2+
 12/13/10 - Lymph node biopsy - negative
 12/28/10 - Started neo-adjuvant treatment along with clinical trial with 4 rounds of AC chemo every 3 weeks
 3/15/11 - Began weekly Taxol/Herceptin infusions along with 750mg of Tykerb taken by mouth daily
 6/28/11 - Finished last cycle of Taxol
 7/27/11 - Breast MRI shows tumor has dissolved, remarkable reaction to chemo
 8/31/11 - Lumpectomy, Sentinel Node biopsy. Node negative, clear margins, 7mm of cancer left over from chemo.
 10/05/11 - Started radiation, 5 days a week for 7 weeks.
 11/8/11 - Finished radiation
 3/21/12 - Last Herceptin!
 3/26/12 - Port removed!
 Tamoxifen for 5 years
 8/4/15 - Hysterectomy & bilateral salpingo-oophorectomy. Due to large fibroids. No cancer!

 2002 recurrence. Bilateral mastectomy w/TFL autologous recon. Then ACx2. Skin lymphatic rash. Taxotere w/Herceptin x4. Herceptin/Xeloda.  Finally stops spreading.
 2003 - Back to surgery, remove skin mets, and will have surgery one week later when pathology can confirm margins.
 ‘03 latisimus dorsi flap to remove skin mets. CLEAN MARGINS. Continue single agent Herceptin thru 4/04. NED.
 ‘04 '05 & 06 tiny recurrences - scar line. surgery to cut out. NED each time.
 1/2006 Rads again,  to scar line. NED.
 
 3/07 Heartbreaking news - mets! lungs.sternum. Try Tykerb/Xeloda. Tykerb/Carbo/Gemzar. Switch Oncs.

 Sept - now - on chemo hold for calming tummy we hope. Markers stable for 2 months.

 3. increasing sclerosis & decreasing activity of L1 met “suggests” mild healing. (SUV 9.4 v 12.1 in May ‘13)
 4. scattered lung nodules, up to 5mm in size = stable, no increased activity
 5. other small scattered sclerotic lesions, one in right iliac and one in thoracic vertebral body similar in appearance to L1 without PET activity and not clearly pathologic

 Jan 2016: 5 days, 20 grays, Rads to L1 and continue on HAPX.  I’m trying to "save" TDM1 for next line. Hope the rads work to quiet L1. Sciatic pain extraordinaire :((

 10/30/19 - stable brain and cord. progression lungs and bones. washing out. applying for ds8201a w nivolumab. hope they take me.
 12/27/19 - begin ds8401a w nivolumab. after 2nd cycle nodes melt away. after 3rd cycle chest scan shows Improvement, brain MRI shows improvement, resolved areas & nothing new. switch to plain ENHERTU. after 4th cycle, PETscan shows mostly resolved  or improved results.  Markers near normal. I'm stunned but grateful.
 10/26/20 - June 2021 Tucatinib/xeloda/herceptin - stable ish.

 Hi all - had my pet scan today and surprisingly, I got results just before closing time at 5 today. I haven't read the report yet but after 5 months of Ibrance and radiation to my single rib met, I have no new cancer and the rib appears to be healing bone Not consistent with active cancer so i am elated!

				__________________July 24, 2013: "Infected" Right Nipple and benign cyst removed, pathology report revealed Paget's, DCIS, and ILC 1.25 cm, ER+/Pro+/HER2 equivocal, Grade 2 under benign cyst, previous diagnostic mammo/ultrasound said I was perfectly healthy in both breasts.
 
 Aug 18, 2013: MRI report says Left breast is perfectly healthy "consistent with previous studies".
 
 Sept 2013: I insist on a bilateral mastectomy anyway. Too nervewracking to let left breast remain with higher risk after 3 cancers in right, nipple in right is already gone anyhow.
 
 Sept 18, 2013: Bilateral mastectomy, 11 right nodes removed, ALL negative BUT -- ER+/PRO+/HER2+ tumor, 1.0 cm, Grade 2 found in a piece of "grossly unremarkable" breast tissue from prophylactic mastectomy of left breast, no nodes removed.
 
 Oct 25, 2013: 13 left side nodes removed, ALL negative, Stage 1 across the board, NO RADS needed, YAAAAY! Port also installed.
 
 Nov 25, 2013 Begin 6 rounds TCH.
 
 March 10, 2014 Just finished 6th and LAST Chemo today, YAAAAAY!
 
 March 24, 2014 Echocardiogram to make sure I'm still good for Herceptin every 3 weeks.
 
 March 31, 2014 Echo results NORMAL, first Herceptin all by itself. Now if only my eyes would stop streaming from the Taxotere ... :)
 
 April 21, 2014 Started Arimidex and therapy for "mild" lymphedema in left hand and arm
 
 May 2014 Therapy completed, I have sleeves and gloves for both arms, a Flexi touch lymph pump to hook up to for an hour every day, and I've become an arm bandaging expert. :)
 
 June 2014 Begin Fosamax to prevent osteoporosis; bone scan revealed osteopenia
 
 Nov 17, 2014 FINAL Herceptin!
 
 Dec 4, 2014 My right thigh muscle has been extra achy for days ... I discover a blister rash cluster on the side of my right thigh while taking a shower. Port appointment cancelled until Dec 17, my doc is working me in tomorrow afternoon to see me and the rash. My muscle at least feels less achy.
 
 Dec 5, 2014 Yep, I have shingles. Boo! I start acyclovir and also have a prescription for a painkiller just in case for over the weekend.
 
 Dec 17, 2014 Port is OUT!
 
 January 2016 Shingles again and this time it started where my left breast (where the hidden HER2+++ tumor was!) used to be. My onc nurse got me a same day appointment to see my doc when I called and told her I had a rash on the site. The antiviral meds are working once again, though, so that is good news. :)

 I posted this under another topic but wanted to be sure to post my results after posting Laurens who is her2+.  My tumour has turned her2- but this is being used for both.  There apparently will be a paper on this in December.
 
 I'm very happy to report that the Ibrance/Letrozole appears to be working. I understand that you have to give this time so after three months (I just finished my 3rd cycle today) I am super stoked to see these results. I'll post the results below. My abdominal CT with contrast was reported separately and NED there so don't think ovarian spread is an issue....although I haven't spoken to my oncologist yet. I see him Aug 2nd. Here we go:
 
 COMPARISON: February 23, 2018.
 LUNGS AND TRACHEOBRONCHIAL TREE: Previously 6 mm nodule anteriorly in left
 upper lobe has decreased to 4 mm in diameter on image #114. No new pulmonary
 nodule.
 LYMPH NODES AND MEDIASTINUM: No enlarged thoracic lymph node.
 PLEURA AND PERICARDIUM: Previously seen left-sided pleural nodules have either
 significantly decreased or resolved. For example
 Previously 25 mm anterior paramediastinal nodule has decreased to 12 mm in
 diameter.
 Previously 27 x 11 mm pleural nodule at upper posterior aspect has decreased to
 13 x 4 mm in diameter.

 April 20, 2018 BC mets confirmed, ER/PR+ now Her2-
 Questions about the possibility of ovarian spread and mets to bones so will be tested and monitored for these.
 To begin new drug Palbociclib (Ibrance) along with Letrozole May, 2018.
 Genetic testing of ovarian tumour and this new lung met will take months.
 To see geneticist to be retested for BRCA this week....still BRCA VUS
 CA125 has declined from 359 to 12 as of Aug.23/18

				__________________July 24, 2013: "Infected" Right Nipple and benign cyst removed, pathology report revealed Paget's, DCIS, and ILC 1.25 cm, ER+/Pro+/HER2 equivocal, Grade 2 under benign cyst, previous diagnostic mammo/ultrasound said I was perfectly healthy in both breasts.
 
 Aug 18, 2013: MRI report says Left breast is perfectly healthy "consistent with previous studies".
 
 Sept 2013: I insist on a bilateral mastectomy anyway. Too nervewracking to let left breast remain with higher risk after 3 cancers in right, nipple in right is already gone anyhow.
 
 Sept 18, 2013: Bilateral mastectomy, 11 right nodes removed, ALL negative BUT -- ER+/PRO+/HER2+ tumor, 1.0 cm, Grade 2 found in a piece of "grossly unremarkable" breast tissue from prophylactic mastectomy of left breast, no nodes removed.
 
 Oct 25, 2013: 13 left side nodes removed, ALL negative, Stage 1 across the board, NO RADS needed, YAAAAY! Port also installed.
 
 Nov 25, 2013 Begin 6 rounds TCH.
 
 March 10, 2014 Just finished 6th and LAST Chemo today, YAAAAAY!
 
 March 24, 2014 Echocardiogram to make sure I'm still good for Herceptin every 3 weeks.
 
 March 31, 2014 Echo results NORMAL, first Herceptin all by itself. Now if only my eyes would stop streaming from the Taxotere ... :)
 
 April 21, 2014 Started Arimidex and therapy for "mild" lymphedema in left hand and arm
 
 May 2014 Therapy completed, I have sleeves and gloves for both arms, a Flexi touch lymph pump to hook up to for an hour every day, and I've become an arm bandaging expert. :)
 
 June 2014 Begin Fosamax to prevent osteoporosis; bone scan revealed osteopenia
 
 Nov 17, 2014 FINAL Herceptin!
 
 Dec 4, 2014 My right thigh muscle has been extra achy for days ... I discover a blister rash cluster on the side of my right thigh while taking a shower. Port appointment cancelled until Dec 17, my doc is working me in tomorrow afternoon to see me and the rash. My muscle at least feels less achy.
 
 Dec 5, 2014 Yep, I have shingles. Boo! I start acyclovir and also have a prescription for a painkiller just in case for over the weekend.
 
 Dec 17, 2014 Port is OUT!
 
 January 2016 Shingles again and this time it started where my left breast (where the hidden HER2+++ tumor was!) used to be. My onc nurse got me a same day appointment to see my doc when I called and told her I had a rash on the site. The antiviral meds are working once again, though, so that is good news. :)

 2 micromets to sentinel node
 Stage 2A
 left 3mm DCIS - low grade ER+PR+Her2 neg
 lumpectomies 9/7/04
 4DD AC followed by 4 DD taxol
 Used Leukine instead of Neulasta
 35 rads on right side only
 4/05 started Tamoxifen
 Started Herceptin 4 months after last Taxol due to
 trial results and 2005 ASCO meeting & recommendations
 Oophorectomy 8/05
 Started Arimidex 9/05
 Finished Herceptin (16 months) 9/06
 Arimidex Only
 Prolia every 6 months for osteopenia
 
 NED 18 years!
