Sharing Cyberknife to Sternum experience

Hello to all.
I had a small bone met in the sternum detected by Pet/scan and confirmed by MRI. Around 8 mm. I decided to have a Cyberknife procedure more than 2 months ago. I found few women who had done that. In France there are just a few Cyberknife centers and none in Paris. (There will be 3 next year). The centers I contacted said it was not in their priorities yet. They are first working on brains, lungs, spines and starting on livers. They are overbooked. So I looked for experienced centers in other countries. I wrote to centers in the States and to one of the most experienced center in Europe which is in Munich, Germany. All centers said they could do it and I opted for Germany. I am really happy I organized this trip because these last weeks my sternum all of a sudden started hurting more and more and I was taking pain medicine.

I entered the hospital in the morning to have 2 gold fiducials (2mm?) placed in my sternum. They put you in a scanner and the surgeon comes in the room with the nurses while the scanner is on. They put on lead shields. My body was half out of the scanner and the ray was just on my chest. The surgeon could see on a screen, just at the entrance of the scanner, where the tumor was. With his hands in the scanner, he did a local anesthesia by injection and then implanted the 2 gold seeds. The entire operation lasted less than 30mn. They recorded the scan and they gave it to the neurosurgeon in charge of the Cyberknife because it is with the scan that the machine does all the calculation. The gold seeds also allow the machine to follow your breathing. You don't need gold seeds now if you do cyberknife in the spine because the breathing issue is not as important. They can track the spot with only software.I was able to leave the hospital in the afternoon and sleep at the hotel.
I was not afraid of being ridiculous and I asked if the seeds would be an issue for MRIs or airports and they said no because they are tiny gold specks. Better ask before...

The next day I had the Cyberknife procedure. It lasted 50 mn. I was dressed and just put a special jacket on. I had to lay on the table and not move. A big articulated arm turned around me, shooting at the tumor from every angle. The machine does everything based on the calculation it did before. There is no man intervention at this stage.

The worse part was the local anesthesia because my sternum was hurting and I had already been irradiated in that zone so it was always very sore.
It hurt 3 or 4 minutes and then I did not feel anything. And nothing during the Cyberknife part.
They told me it would take 6 months to really see the result on a scanner because of the small inflammation the procedure would create. They were very confident about the result because the tumor was small and only in the bone.

Today my sternum is a little a blue from the seed implementation and it hurts a little when I move but nothing to do with the deep pain I had before.

This procedure is just palliative but if it can spare me pain and worse, big problems if the hole gets bigger, it is worth it.

I wanted to share this positive experience.

Thanks for the report. Especially useful is your information regarding where they are/are not treating with Cyberknife especially in extracranial sites.

Your first-hand description should help others considering similar treatment.

Thank you for getting the info out on the web for others to use as "ammunition"

Yes Rich, I will continue Zometa but my onc thinks once every 2 months is enough. I asked for Denosumab but it is not available in France yet other than by exception. My onc is investigating.

AM posting that FDA in US has just approved denosumab for antihormonal treatment related osteoporsosis in bc, prostate cancer patients

05/09/2013: Discussing SBRT with Radiology due to inability of bone marrow to recover from chemo.

07/03/2013: Chemo discontinued, on Perjeta, Herceptin and Zometa alone
07/25/2013: SBRT (gamma knife) begins
08/01/2013: SBRT completed
08/15/2013: STABLE! continuing with Perjeta, Herceptin, Zometa
06/18/2014: ***** NED!!!!***** continuing with Perjeta, Herceptin, Zometa
01/29/2014: Still NED. continuing with Perjeta, Herceptin. Zometa lowered to every 3 months instead of monthly.
11/08/2015: Progression throughout abdomen and lungs. Started TDM-1, aka Kadcyla. Other meds discontinued. Remission was nice while it lasted.

5/27/18: Stable. Kadcyla put me right back in the barn. I have two teeny spots on my lungs that are metabolically inactive, and liver is clean.

I’m beating this MFer. I was 51 when this started and had two kids, 22 and 12. Now I’m 60. My oldest got married and trying to start s family. My youngesg graduates from Caltech this June. My stepdaughter gave me grandkids. Life is fantastic.

Diagnosed stage 2b in July 2003 (2.3 cm, HER2+, ER-/PR-, 7+ nodes). Treated with mastectomy (with immediate DIEP flap reconstruction), AC + T/Herceptin (off label). Cancer advanced to lung in Jan. 2007 (1 cm nodule). Started Herceptin every 3 weeks. Lung wedge resection April 2007. Cancer recurred in lung April 2008. RFA of lung in August 2008. 2nd annual brain MRI in Oct. 2008 discovered 2.6 cm cystic tumor in left frontal lobe. Craniotomy Oct. 2008 (ER-/PR-/HER2-) followed by targeted radiation (IMRT). Coughing up blood Feb. 2009. Thoractomy July 2009 to cut out fungal ball of common soil fungus (aspergillus) that grew in the RFA cavity (most likely inhaled while gardening). No cancer, only fungus. Removal of tiny melanoma from upper left arm, plus sentinel lymph node biopsy in Feb. 2016. Guardant Health liquid biopsy in Feb. 2016 showed mutations in 4 subtypes of TP53. Repeat of Guardant Health biopsy in Jana. 2021 showed 3 TP53 mutations, BRCA1 mutation and CHEK2 mutation. Invitae genetic testing showed negative for all of these. Living with MBC since 2007. Stopped Herceptin Hylecta (injection) treatment in March 2020. Recent 2021 annual CT of chest, abdomen and pelvis and annual brain MRI showed NED. Praying for NED forever!!

Hello from south of France from StephN. Thank you for such a blow by blow account of your procedure. Plus I am so happy the pain is receding.

Hoping this means we can meet when I get to the Paris region. My husband thinks that I will be walked out and Versailles will be too much, but we will see how everyone is doing next week!
