Question Re: Mortality In Nearly Half Of Cancer Survivors Due To Conditions Other Than Cancer

If a cancer patient who was treated in the past with chemo that caused heart problems then dies of cardiac failure, since the cardiac failure was the "immediate cause of death", is it reported as a cancer-related death, or only as cardiac-related?

If they are receiving palliative care as the cancer progresses, but die of cardiac failure as the "immediate cause of death", are they reported as dying of cancer, or as having died of cardiac failure?
