I hope you are still reading this board - the difficulty you are having could very well be just a uterine polyp, that can be found and removed during a relatively easy D&C - please continue with follow ups. A uterine biopsy is even easier than a D&C - they are done in the office, at least my 2 were, and you pop a couple tylenol afterwards and that's it; this procedure alone might pick up the polyp - don't know if it can actually be removed during just the office biopsy but at least it'd give you an answer. I know it's frustrating and I wish you well.
