I've had the same issue with my big toe nails (and crappy nails in general) and it got so really painful that I went to a podiatrist. He cut the nail surgically and it has healed beautifully and I am pain free. The worst part was getting needles in the big toe to numb it up, but with the relief I have gotten was well worth it. You might want to consider this. They can cauterize the nail so it does not grow pack in that painful spot.
