TCH or FEC?
 

We have our first oncologist consultation tomorrow so I've been studying what I can tonight, based on my wife's diagnosis I'm assuming her oncologist will recommend either TCH or FEC (plus herceptin), but I haven't been able to find anything that would indicate why one or the other. Any assistance is appreciated.

If you google ACTH (adriamycin, cytoxin, taxol, herceptin) vs TCH (taxol, carboplatin, herceptin) you will find a good bit of information. The long term studies are not in yet, but the stats thus far look fairly equal. FEC is similar to ACTH with the Epirubicin being a less cardiotoxic version of Adriamycin and the Cytoxin is the same. I do not know much about 5FU as it is not used for breast cancer much here in the States. I do know that it is not advisable for the Eprirubicin to be administered in conjunction with the Herceptin, sequentially is the way to go with Herceptin being given upon the completion of the Eprirubicin.

I was given FEC x 3, then Taxotere x 3, then I had my Herceptin, every 3 weeks for a year. My onc. strongly recommended the Taxotere along with the FEC, said it was very good fighting Her2+ BC.
FEC is commonly used in Canada, England and other commonwealth countries.
TCH is a newer combo, I don't know if it is given in Canada yet, as I am over 3 years post chemo, and over 2 years post Herceptin.

Hi,

I had FEC for my first diagnosis in 2003. I think FEC was used in place of AC because of my lower than normal heart function. When TCH became the standard, I had TCH in 2007 for the local regional recurrence.

Since Herceptin is 'the' cure for Her2 breast cancer, I think TCH should be the choice for your wife. There are studies showing each drug targets certain part of the cancer cell (skeleton, etc.) Maybe that's why the newer protocol uses both. If you look at other newly diagnosed, seems several of them are doing 4 AC (=FEC) and then TH, then Herceptin only.

What seems to be used most often...or known as the standard of care is "ACTH" as Laurel mentioned, & TCH. If you have heard of Dr. Slamon out at UCLA...he is the one that made Herceptin. His clinic will only treat you with TCH. The studies between ACTH and TCH showed very little difference. The main difference is lower risk of heart damage with TCH. So I'm pretty sure your wife will be offered one of those two choices. (It's so nice of you to research online and help your wife out...she is so blessed to have a wonderful husband to support her through this.) You both take care and keep us updated. thanks to everyone for the information. Our oncologist has recommended 6 courses of Taxotere / Carboplatin, and a year of Herceptin every three weeks. She will receive a port next week and start treatment the week after that. From everything I've read that seems like a pretty common course of treatment for her diagnosis.
