We will be replacing our list of recommended breast care practitioners shortly.

I have developed a form to use to input the information and need to test it out for functionality. Please fill out and submit the following form:

I submitted my form without too much trouble. Title and Suffix were confusing but maybe others are smarter and will get it.

What about a question section where we are asked to either rate things or check off things that apply.
Or maybe we should add a data base section that you could use to view or compare services of the practice.

Even for me it was hard to stay on the important points and I left many off. I should have mentioned the size of the office - the treatment room on site - the pet/ct scanner in-house & on site - labs on site - radiation treatment also in house - across the street. Free parking. NICE COMPASSIONATE office culture. Professional staff. Etc. I left off the website address.

we will be creating a database shortly. We are currently in Saltillo, Coah, Mexico to attend a wedding and will not be returning home until the middle of next week.

Until then I have very limited connectivity.
