I found the attached website in MAMM magazine while I was waiting for my followup mammogram. I filled it out and thought others would like to (get a free cd). Every bit of info they get will help them in further treatments etc.

It only takes about 2 minutes to fill out and submit the questionaire and I sent it on as well. I think it is a good thing for as many of us who would like to participate to let Genentech know we are out here..as we are, afterall, their most loyal market...smile...
