Idc/dcis question

Ok, I admit I should know this, but have never gotten an clear and understandable answer from my onc. Can anyone tell me what this means?? Could it mean the tumor is 1.0 cm that is IDC and 1.0 cm DCIS?

My path report says:

'Poorly differentiated infiltrating ductal carcinoma compatible with 8/9 points according to the SBR grading system (tubule formation 3, nuclear grade 3, mitotic rate 2) This is associated with high grade ductal carcinoma in situ with comedonecrosis. The in situ component constitutes approximately 50% of the tumor volume and focally exceeds the area of invasive carcinoma by several millimeters. The invasive carcinoma focally approaches the painted surgical margin up to 1 mm and the in situ component is present within a fraction of a millimeter of the painted surgical margin.'

Ok, so I get the part down to where it says this is high grade ductal carcinoma in situ etc etc. If the DCIS is 50% of the tumor doesn't that change the size of the tumor? In total it is 2.1 cm. Isn't DCIS normally a better prognosis or does it not matter since there was IDC...I am confused.

Thank you for any answers!!

Re: Idc/dcis question

Hi,

The total (IDC and DCIS) tumor size is similar to mine (2.5 cm IDC in 2007) which was staged at IIB. It looks like yours have been staged based on the IDC portion while taking into consideration of the adjacent DCIS. (multi-focal?)

There is a National Cancer Institute website detailing the staging criteria. I'll add it here when I locate it. Found it: http://ccm.ucdavis.edu/bcancercd/311/tnm_staging.html
