Surgeon and Investigator: Constantinos Sofocleous This is a new study, and I believe a significant one. When surgery is performed on the tumor, the specimen is sent to the lab and one learns soon enough if clear margins were achieved. With ablation, the surgeon would like to keep at least 1 cm of clean tissue around ablation, and that should include clear margins. The strategy is: take two biopsies during ablative therapy: one pre-ablation and one post (same surgery). Results will determine whether clean margins were had, that is, if tumor cells are not found in the second biopsy after the ablation. It is believed that they GOT IT ALL according to the PET/CT, you know, dancing eyes, glowing faces, relieved demeanor/s, but the real truth lies in the lab. The results are in. Pre-ablation liver biopsy yielded breast cancer tumor cells. Post-ablation biopsy yielded some cells as well in the charred-out part. That part is not "cut" out but should disintegrate, fade out, go away. If I had a PET today, it would be clear. If I didn't look for these cells, we wouldn't find them. We had clear margins. So the question is...and the whole point of the trial is...if cancer cells are found in the ablation area, is the patient more likely to have a higher recurrence? We just don't know yet. So, it's all good, I suppose---until something develops or never does.
Hope something fruitful comes out of the $25 million grant to Weill Cornell. Now, that would be news. Still sore. But not like before. Continue Perjeta, Herceptin, Arimidex. Thanks for your support and words of encouragement, Karen |
