Here's a link to the National Comprehensive Cancer Network guidelines for treating breast cancer, which can give us an idea why some oncs in the United States refuse to give certain treatments and insurance companies sometimes refuse to pay for them:

Notice how oncs from the comprehensive cancer centers participate in setting the standards, which may be why their facilities often refuse to give particular treatments that other survivors have received, since they set the standards.
