Iron deficiency produces reduced MCH, MCV, and reduced Hgb. In iron deficiency, the serum iron level and ferritin level are low. The total iron binding capacity is elevated, and the percent saturation is low (<20%). Ferritin is an acute-phase reactant and may be falsely normal in patients with cancer. Thus, the transfer iron saturation level of <20% may be used as an alternative measure of iron deficiency.
In addition to common clinical findings, the presence of “pica,” a craving for nonfood substances, is a sign
of iron deficiency
