insurance paying for early stage herceptin

Do any of you have Anthem BC/BS paying for early stage herceptin?

My onc office checked with my insurance before starting herceptin and got approval. Now they are sending me letters saying they need more information about dx, treatment, response etc then they will reopen. Anybody else running into this?

I have Anthem BC/BS in Maine. I have an "insurance caseworker" who has called me every few weeks since my dx to ask what treatment I am receiving. I'm stage 1, asked about herceptin and she said it was covered, no hassles. I've received two treatments. I also asked about BRCA testing and she said that needed to go through a Review Panel to be approved. I haven't done that yet.

I don't know if Anthem has a national policy or makes decisions on a state by state basis.
