Follow up care after chemo complete/ scans

Hello ladies,
Questions... I will be having my last chemo tx on May 6th, then the mastectomy,
Rads and 1 year herceptin . Ct scan in May before surgery. In discussing my follow up care with oncologist (pending results of path report after surgery) she said my next scan will be yearly and that she doesn't do blood checks for tumor markers. She does basic labs CBC, bmp. So unless I become symptomatic no follow labs or scans.( will still see her once every 3 weeks for herceptin).Her reasoning is that with the ct scans she doesn't want to introduce any unnecessary radiation into my body as it will put me at higher risk of other cancers . As for the labs ,she doesn't do tumor markers as in my words that I can understand they don't always tell the whole story and everyone is different plus does not want undue stress on patient. With having ibc plus her2 neu I truly am concerned about the waiting periods between scans, and yet I don't want to risk developing other cancer . Advice please. And I am sure my oncologist will do a compromise with me to make sure I am comfortable with my tx.

7/11/14 emergency surgery to left breast . Wound refused to heal and broke open. New TE placed
7/28/14 emergency medical procedure now to right breast( post op 6 weeks mastectomy ) wound opens.
8/25/14 - 10/07/14 Radiation
11/24/14 CT of Brain clear
01/23/15 One year Herceptin Complete
02/04/15 Diep done
5/01/15 1 year Ct scan- mass in thymus
6/23/15 repeat CT mass in thymus still present
6/30/15 biopsy thymus 7/03/15 mass Benign! Diagnoses Thymic hyperplasia rebound ( chemo at fault, no long term health effects)
7/08/15 phase 2 diep
11/23/15 phase 2b ( reconstructive surgery with lipo)
03/05/18 Vinnie Myer 3 D nipple tattoos complete.
11/19/2018 5 years since Dx of IBC stage 3c remain NED.
11/19/21- 8 years today I officially remain NED and in 12 days I will be having a lymphnode transfer to help decrease some mild lymphedema! Still working, living life and soon to see a day I thought never would happen and that’s becoming a grandparent June 22nd.
11/19/2023 - 10 years since diagnosis and I remain NED

Hi this is Joanne, I just joined in April had my first neoadjuvant TCH-P April 17th. I have worked in my Oncologist's office since 1990. Unless you have active disease or symptoms the current opinion is less radiation from scans is better the chemo already makes you susceptible to a secondary malignancy. The following paragraph was in Uptodate.com physician web sit but you have to have a login so I copied and pasted the paragraph.

"Screening studies — New screening diagnostic procedures have been developed, including coronary CT screening, whole body CT screening, and chest CTs for lung cancer screening [68-70]. For these patients who are either asymptomatic or vaguely symptomatic, the increased risks, such as radiation-induced cancer, may outweigh the benefits of their radiologic procedures, and a full discussion of risks, benefits, and alternatives is of particular relevance"

This probably carries over to screening for for recurrence in asymptomatic cancer patients post treatment. Physicians don't want to cause more harm!

I'll be checking in with you if you don't mind, as I just started chemo. I will finish the first week of August.

May we find ourselves looking forward to renewed health 1 year from now!!

HX: left breast IDC T1a node negative 1999. Lumpectomy, sentinel node biopsy, radiation therapy, 5 years of Tamoxifen, 1 year of Femara discontinued due to intolerance.
3/21/14 almost 15 years later abnormal mammogram 2 cm right breast mass deep in the breast by the chest wall, mammogram last year was normal. Ultra sound confirmed highly suspicious for malignancy.
3/26/14 needle biopsy proven IDC grade 3, ER+ PR+ Ki67 75%, Her 2 in process.
3/31/14 Consult with Breast Surgeon. Bilateral mastectomy discussed along with referral to plastic surgeon for planned surgery with reconstruction.
4/4/14 Her 2 +. My Oncologist explained that this is a new primary and unlike before in 1999 very aggressive. Surgery put on hold.
BRCA testing done. Results were negative for gene mutation.
4/7/14 PET CT for staging for possible neoadjuvant treatment. Results indicate 1.3 cm tumor and a positive lymph node.
4/9/14 lymph node biopsy. Pathology documented metastasis.
4/14/14 cardiac echo result Ejection Fraction 60%
4/16/14 Port placement
4/17/14 1st chemotherapy treatment
with Herceptin, Perjeta, taxotere and carboplatin.
4/18/14 1st treatment done yesterday with no events whatsoever!
4/23-4/25 Dreaded D hit really hard.
Daily visits to the Oncologists office for hydration and neupogen injections.
4/24/14 Started on Cipro for seriously low WBC count.
4/25/14 Results of blood count at MD office so low (ANC 0.08) I am hospitalized in isolation for neutropenic precautions. One treatment wiped out my immune system!
4/28/14 home recovering. 4/30/14 MD's new plan of treatment: reduced dose weekly taxotere, carboplatin, Herceptin with Perjeta every third week.
5/7/14 Cardiac echo > than 60% and the breast ultra sound showed 2 cm lymph node now only .5 cm. mass in breast is unable to be palpated!! treatment is a go.
5/9/14 Partial 2nd treatment, full dose Herceptin and Perjeta 1/3 dose taxotere and carboplatin.
5/13/14 lab check and fluid infusion.
5/14/14 Appointment with my Oncologist. The lab results are great and the dreaded "D" not as severe as 1st round. Then I mention that my left calf and foot are swollen and painful to the point of limping, could be the drugs but to be safe I had a venous doppler study. I have a clot in my left leg. It wasn't totally occluding the vessel and she really didn't want me in the hospital. Now I will be on Arixtra shots in my tummy till the chemo is complete sometime in August, 2014
5/16/14 continuation of 2nd treatment taxotere carboplatin only.
5/23/14 continuation of 2nd
treatment taxotere, carboplatin only
Amazingly no horrid "D" or nausea!!
5/28/14 visit with the oncologist, great news she can no longer feel a breast mass or the malignant lymph node Yippee!!
5/30/14 started 3rd chemotherapy cycle had full dose Heceptin, Perjeta ans 1/3 dose taxotere and carboplatin.
6/6/14 next dose of taxotere carboplatin for 3rd cycle
6/13/14 last dose taxotere carboplatin 3rd cycle
6/16/14 I am scheduled again for another breast ultrasound to see extent of response to chemotherapy. results will direct continued chemotherapy of directly to surgery!
6/20/14 tentative 4th chemotherapy cycle

Congratulations on getting the hard part done in just a few days.

What your doctor had said is very standard. Routine scans don't necessary catch recurrence sooner. My scans had been misread (as scar tissues) for three years straight before I went to the original surgeon and confirmed the recurrence. So, just pay attention to your body and bring any new concerns to your doctor's attention.

My oncologist also did not believe in routine scans. During the year I received herceptin, I had routine blood work, including MUGA scans. Tumor markers were done during this time. I continued to see my oncologist quarterly for years 3+4 post treatment...with blood work, including tumor markers. Year 5 I saw her twice, and then was "released" . It felt like my lifeline was severed!!! My oncologist told me that I could make an appointment any time, she would order any scans, blood work, etc. As time goes on I am more comfortable with no oncologist, scans, etc. At my last regular doctor's visit I asked, any had, tumor markers done...normal. At first it was hard to wrap my head around the idea that I shouldn't worry as long as I was asymptomatic...after all, I was asymptomatic when I was first diagnosed!!! And as far as not "worrying", HA!!!I still worry about every little twinge. And I have gone to the ER on more than one occasion with symptoms...dizziness and nausea (sure it was recurrence to brain) was really a bad case of vertigo. Burning pain and numbness ( sure it was tumor pressing on spine) was really SHINGLES. You get the picture! That is another reason I love this support group...we all "get it"!!!

The more I read about doing scans after tx risk vs benefits the more I just may wait for the yearly scan. My hope is softer surgery my path report is good/great and that will ultimately help me with my final decision.

I see my oncologist every 3 months, get complete blood work (cmp, CBC, ca15 tumor marker).

Had my first breast MRI since bmx a couple of weeks ago, all clear (yea!). Have never gotten any kind of pet scan and don't think my doc will order one unless I am symptomatic of something. I will get an mri every year.

A word on tumor markers - when my new primary was found, my tumor marker was 17. Go figure

Nov 29, 2012: Begin TCH/6x/every 3 wks, H for 1 year/every 3 weeks.
March 14, 2013: Finished chemo
April 9, 2013: Begin radiation 28x
May 22, 2013: Finished rads
June 1st, 2013: Started Aromasin for 5 yrs.
July 15, 2013: Switched to Letrozole (Femara). Probably for the rest of my life
October 16, 2013: Exchange surgery
October 31, 2013: Finished Herceptin
December 5, 2013: Port removed
