TDM-1 news

I have received my first dose last week, Thursday. Yeah!! I have been feeling so much more at peace ever since.

Initially I did not have any side effects. But my platelets have crashed (grade 3 toxicity) so I was unable to do the angioplasty procedure planned yesterday that could potentially help with my puffed up face (planned for next Wednesday so ok about that) they want my platelet to naturally come back up. I have also felt tired but it is hard to tell after all the heparin tribulation/randomisation if I was simply mentally exhausted. I also have had headache. But he ho it will pass.

I am having more problem breathing now tho that I did last week and new pain spot in the lungs (on the back) . I know that my lungs is full of tumours more than 20 with some over 5cm.. but I am hoping that the pain is perhaps it is a good sign as oppose to just things progressing fast. They are perhaps squealing right??

I have started 2 weeks ago a super healthy delicious veg raw food diet (+fish) and I feel better for it, much better. I think the miso I use daily now helped with my oetrogen level too (I am ER- but on menopause). I even got myself a dehydrator: going serious again with it! and enjoying it. I have experienced with it in the past initially when diagnosed.

I will have my first scan in 6 weeks but until then I am feeing quite at peace and continue planning for the summer and getting into my gardening.

superior vena cava blocked: stent but face remains puffy

April 2012: Teresa Trial, randomised to TDM1
Nov 2012 progressing on TDM1
Dec 2012 blockage of my airways by tumours, obliteration of these blocking tumours breathing better but hoping for more- at mo too many tumours to count in the lungs and nodes.

Dec 2012 Starting new trial S-222611 phase 1b dual egfr her2+ inhibitor.

I think you're right...I can hear the cancer beastie's death screams all the way across the ocean! TDM1 is 'da bomb'!

Hope the side effects aren't too troublesome...rest, and get your strength back...let the drugs do the hard work!

Yeah! I am so glad you got the drug last week. Hope your headache is gone. I am saying the cancer is screaming from the treatment. It is raining in Georgia today.

VERY happy to hear that you have prevailed with this trial!
I can hear your sigh of relief with that post. some pain with treatment is normal, especially with mets. Remember that the DM1 in your drug is a mean sister compared to the T. And the tumors are getting hit inside the cells.

Your idea to keep on with your summer plans is a good one, but you have to be close to your treatment center in any case.

Hope the spring weather is also helping to cheer you up, as well as your puppy. Cyberhugs to you.

MY STORY SO FAR ~~~~
Found suspicious lump 9/2000
Lumpectomy, then node dissection and port placement
Stage IIB, 8 pos nodes of 18, Grade 3, ER & PR -
Adriamycin 12 weekly, taxotere 4 rounds
36 rads - very little burning
3 mos after rads liver full of tumors, Stage IV Jan 2002, one spot on sternum
Weekly Taxol, Navelbine, Herceptin for 27 rounds to NED!
2003 & 2004 no active disease - 3 weekly Herceptin + Zometa
Jan 2005 two mets to brain - Gamma Knife on Jan 18
All clear until treated cerebellum spot showing activity on Jan 2006 brain MRI & brain PET
Brain surgery on Feb 9, 2006 - no cancer, 100% radiation necrosis - tumor was still dying
Continue as NED while on Herceptin & quarterly Zometa
Fall-2006 - off Zometa - watching one small brain spot (scar?)
2007 - spot/scar in brain stable - finished anticoagulation therapy for clot along my port-a-catheter - 3 angioplasties to unblock vena cava
2008 - Brain and body still NED! Port removed and scans in Dec.
Dec 2008 - stop Herceptin - Vaccine Trial at U of W begun in Oct. of 2011
STILL NED everywhere in Feb 2014 - on wing & prayer
7/14 - Started twice yearly Zometa for my bones
Jan. 2015 checkup still shows NED
2015 Neuropathy in feet - otherwise all OK - still NED.
Same news for 2016 and all of 2017.
Nov of 2017 - had small skin cancer removed from my face. Will have Zometa end of Jan. 2018.

22 February 2011 - Diagnosed Early Breast Cancer IDBC Stage2b (ER/PR -ve, Her2+ve +++) - 38 years old
(L) skin sparing mastectomy with tissue expander, axilla clearance (2/14 affected) clear margins.
Fec*3, Taxotere and herceptin*2 - stopped due to secondary diagnosis

June 24 2011 Stage IV - Skin met, axilla node, multiple lung lesions

Bolero3 trial - Navelbine, Hereptin weekly, daily Everolimus/Placebo
February 2012 - July 2012 Tykerb and Xeloda - skin mets resolved, Lungs initially dramatically reduced but growing again
August 2012 (turn 40!) tykerb and herceptin (denied compassionate use of TDM1) while holidaying in Italy!
September 2012 - January 2013 TDM1 as part of the Th3resa trial - lymph nodes resolved, lungs slowly progressing.
January 2013 - herceptin, carboplatin and Perjeta (compassionate access)
April 2013 - Some progression in lungs and lymph nodes - Abraxane, Herceptin and Perjeta
July 2013 - mixed response - dramatic reduction of most lung disease, progression of smaller lung nodules and cervical and hilar nodes - ? Add avastin.

I have been feeling a bit better and today it may sound anodyne but I sneezed which 3 days ago I had suddenly lost the ability to do because my lung would not expand enough and was afraid it meant progression. So many thing it could have been from a small pulmonary embolism (had all the signs but with platelet at 25 what could they have done anyway) to small lung collapse (could hear some sound nothing can be done) or even a bloated stomach pressing on a lung scar..all are better than prog.. these things than can resort themselves..fed up of being scanned and hospitals so I let nature takes its course..

and I am a bit better already so the plan:

1-So hopefully get my face sorted soon (once my platelets are up)
2-See some really improvement after next TDM1 IV (in my breathing)
3-Camper in the UK for the moment
4-Go to my best friend wedding in Turkey (very ambitious plan)
5-Write a "un"cook book as Jean suggest!! I have been thinking of organising raw meals at the cancer charity close to me.

9/24/12 CT scan--No new mets. Everything stable.
3/11/13 CT Scan--two small new possible mets and odd looking area in left lung getting larger.
4/2/13--Biopsy of suspicious area in lower left lung. Mets to lung confirmed.
4/30/13 Begin Kadcyla/TDM-1
8/16/13 PET scan "mixed," with some areas of increased uptake, but also some definite improvement, so I'll stay on TDM-1/Kadcyla.
11/11/13 Finally get hormone receptor results from lung biopsy of 4/2/13. My cancer is no longer ER positive.
11/13/13 PET scan mixed results again. We're calling it "stable." Problems breathing on exertion.
2/18/14 PET scan shows a new lesion and newly active lymph node in chest, other progression. Bye bye TDM-1.
2/28/14 Begin Herceptin/Perjeta every 3 weeks.
6/8/14 PET "mixed," with no new lesions, and everything but lower lungs improving. My breathing is better.
8/18/14 PET "mixed" again. Upper lungs & one spine met stable, lower lungs less FDG avid, original tumor more avid, one lymph node in mediastinum more avid.
9/1/14 Begin taking Xeloda one week on, one week off. Will also stay on Herceptin and Perjeta every three weeks.
12/11/14 PET Scan--no new lesions, and everything looks better than it did.
3/20/15 PET Scan--no new lesions, but lower lung lesions larger and a bit more avid.
4/13/15 Increasing Xeloda dose to 10 days on, one week off.
7/1/15 Scan "mixed" again, but suggests continuing progression. Stop Xeloda. Substitute Abraxane every 3 weeks starting 7/13.
10/28/15 PET scan shows dramatic improvement everywhere. All lesions except lower lungs have resolved; lower lungs noticeably improved.
12/18/15 Last Abraxane. Continue on Herceptin and Perjeta alone beginning 1/8/16.
1/27/16 PET scan shows cancer is stable.
5/11/16 PET scan shows uptake in some areas that were resolved on the last two scans.
6/3/16 Begin Kadcyla and Tykerb combination
6/5 - 6/23 Horrible diarrhea from K&T together. Got pneumonia.
7/15/16 Begin Kadcyla only every 3 weeks.
9/6/16 Begin radiation therapy on right lung lesion that caused the pneumonia.
10/3/16 Last of 12 radiation treatments to right lung.
11/4/16 Huffing and puffing, low O2, high heart rate, on tiniest bit of exertion. Diagnosed as radiation pneumonitis. Treated with Prednisone.
11/11/16 PET scan shows significant improvement to radiated part of right lung BUT a bunch of new lung lesions, and the bone met is getting worse.
11/22/16 Begin Eribulin and Herceptin. H every 3 weeks. E two weeks on, one week off.
3/6/17 Scan shows progression in lungs. Bone met a little better.
3/23/17 Lung biopsy. Tumor sampled is ER-, PR+ (5%), HER2+++. Getting Herceptin and Perjeta as a maintenance treatment.
5/31/17 Port placement
6/1/17 Start Navelbine & Tykerb

Diagnosed stage 2b in July 2003 (2.3 cm, HER2+, ER-/PR-, 7+ nodes). Treated with mastectomy (with immediate DIEP flap reconstruction), AC + T/Herceptin (off label). Cancer advanced to lung in Jan. 2007 (1 cm nodule). Started Herceptin every 3 weeks. Lung wedge resection April 2007. Cancer recurred in lung April 2008. RFA of lung in August 2008. 2nd annual brain MRI in Oct. 2008 discovered 2.6 cm cystic tumor in left frontal lobe. Craniotomy Oct. 2008 (ER-/PR-/HER2-) followed by targeted radiation (IMRT). Coughing up blood Feb. 2009. Thoractomy July 2009 to cut out fungal ball of common soil fungus (aspergillus) that grew in the RFA cavity (most likely inhaled while gardening). No cancer, only fungus. Removal of tiny melanoma from upper left arm, plus sentinel lymph node biopsy in Feb. 2016. Guardant Health liquid biopsy in Feb. 2016 showed mutations in 4 subtypes of TP53. Repeat of Guardant Health biopsy in Jana. 2021 showed 3 TP53 mutations, BRCA1 mutation and CHEK2 mutation. Invitae genetic testing showed negative for all of these. Living with MBC since 2007. Stopped Herceptin Hylecta (injection) treatment in March 2020. Recent 2021 annual CT of chest, abdomen and pelvis and annual brain MRI showed NED. Praying for NED forever!!

Congrats FOB !! Did we talk about the platelet issue before ? Lorraines dropped from the beginning, from over 200,000 at start to about 90,000 after 3-4 doses at 3.6 strength. She got 3 more doses at that strength , total of 7. In Hindsight probably should have cut back a bit earlier , after 4-5, depending on plat level. But we didnt know , went on, and plats went to 40,000 after 7 doses. ( had a sepsis issue from old stents in April, 2011, , but that was a unique problem , did drop her plats to 29,000, briefly. didnt help, but speaks to these steps as having good results to come back from there. ) Switched to 3.0 ( every 3 weeks ). And barely hung on , but plats gradually rose starting last July , to 80,000 last week. In May, Lorraine started diet changes sharks liver oil, chlorophyll at health food store, 1,000 mg folic acid, green leafy veggies, spinach , sweet potato, good quality red meat 7 days , and 4 days prior to tx. ( plats have a life in body of 6-8 days ). We favor Porterhous e steak asit has fat in it, roast beef. If your bone marrow is under attack, you have to be a bit of a cavewoman ! Brisk 15' walk just before blood draw ( tip from a Gen. doc ), regular light exercise, regular rest . Doc says not having bone mets may have given L. more bone marrow reserve,

Also, lots of excitement over here w/ the Sloan / Farber trial w/ pertuzumab and t dm-1 at low weekly doses ( 2.4 ). Taxol is given at start , but gets dropped after 3 months or so, if theres quick response . Many are having more dramatic results than Lorraine, and she had a quick response. I've heard of NED after amonth or two ! I think pert. / t dm-1 will be the tx choice of the future. low weekly dosing of t dm-1 seems to protect against plat loss.

Thanks all for the good words. My platelets are on the way back 117 almost 2 weeks after the treatment. Therefore likely to be over 150 by week 3.

Phil how long after the treatment were Lorraine's platelets checked? was that after the 3 weeks prior to the next treatment?

Mine went low 5 days after 1st tdm1 IV but we only found out by accident because of the angioplasty planned procedure.

Having the angiop. this coming Tuesday and really hoping that my face wil be normal again. And surely the fact that more blood will go to the heart as oppose to being pushed to my face will help my body generally and my heart having to pump less.

Good to know that the dose is working at lower level too. I eat plenty of oily fish and veg, I have no doubt that my diet is adequate, if only by the way I feel mentally. 18 months that is nice news to hear, I've always think weekly IV is better: repeat bashing of cancer.. inconvenient but better what ever works

I hope that me and Michka respond to TDM1 too, it is hard to tell between symptoms and everything else going my breathing got worse a week after tdm1 but it seems to have ease up now a bit now or at least it is def not not getting worse since last week. have had blood clot issue so it could have been that.

I had a skin rash coming upon my breast and looked like skin mets but the day after had another patch on my kidney then another patch on my leg with few pimples here and there.. which did not fit the criteria. I was worried )although with eveything that has been happening I was quite calm about it, funny how one adapts) but not anymore. Never heard of skin rash as side effect tho.

That is the news for now next tdm1 next thursday.

Dec 2012 blockage of my airways by tumours, obliteration of these blocking tumours breathing better but hoping for more- at mo too many tumours to count in the lungs and nodes.

Dec 2012 Starting new trial S-222611 phase 1b dual egfr her2+ inhibitor.

Things are really going smoothly thus far here. With T-DM1, platelets take a major hit, but rebound quickly. Keep up with a very healthy diet and rest when you need to. The only side effects with the two drugs I am on is fatigue and a little nausea on infusion day, which is easily controlled with a single Zofran or maybe a cup of black tea.

Thyroid Eye Disease. 2003. Decompression surgery in 2009; eyelid lowering surgery in 2010.
Diagnosed: June 2010, liver mets. ER-/PR+10%; HER2+++.
July 2010: Begin Taxol/Herceptin. Eliminate sugar from diet. No surgery or radiation.
January 2011: NED
April 2011: Progression in liver only. Other previous affected areas eradicated. Stop Taxol/Herceptin after 32 infusions.
May 2011: Brain MRI: clear.
May 2011: Begin Tykerb daily, Xeloda twice per day for one week on, one week off, and Herceptin.
November 2011: Progression in liver. All other tumors remain eradicated.
December 2011: BEGIN TRIAL #09-093 Taxol, MCC-DM1 (T-DM1), Perjeta.
Trial requires scans every six weeks, bloodwork and infusions weekly.
Brain MRI: clear.
January 2012: NED. Liver mets, good riddance!
March 2012: NED. Developed SMA (rare blood clot) in intestinal artery and loss of sight in right eye due to optical nerve neuropathy. Resolved when Taxol removed this month.
Continue Protocol of T-DM1 weekly and Perjeta every 3 weeks.
May 2012: NED.
June 2012: Brain MRI: clear.
June-December 2012: NED.
December 2012: TRIAL CONCLUDED; ENTER TRIAL EXTENSION #09-037. CT, Brain MRI, bone scan: clear. NED.
January-March 2013: NED.
June 2013: Brain MRI: clear. CEA upticking; CT shows new met on liver.
July 3, 2013: DISASTER STRIKES during liver ablation: sloppy surgeon cuts intercostal artery and I bleed out, lose 3.5 liters of blood, have major hemothorax, and collapsed lung requiring emergency resuscitative thoracotomy, lung surgery, rib rearrangement and cutting deep connective tissue, transfusion. Ablation incomplete. This life-saving procedure would end up causing me unforgiving pain with every movement I make, permanently, otherwise known as forever.
July 26, 2013: Try Navelbine/Herceptin. Body too weak after surgery and transfusion. Fever. CEA: Normal.
August 16, 2016: second dose Navelbine/Herceptin; CEA: Normal. Will skip doses. Watching and waiting.
September 2013: NED, Herceptin only. CEA: Normal. Started Arimidex.
October-November 2013: NED. Herceptin and Arimidex. CEA, CA125, 15-3: Normal.
December 2013: Something brewing. PET lights up on little spot on liver; CEA upward trend, just outside normal. PET and triphasic liver scan confirm Little Met. Restart Perjeta with Herceptin, stay on Arimidex. Genomic sequencing completed for future treatments, if necessary.
January 2014: Ablate Little Met on the 6th. Happy New Year.
March 2014: Brain MRI: clear. PET/CT reveal liver mets return; new lung mets. This is not funny.
March 2014: BEGIN TRIAL #10-005 A(11)-Temsirolimus plus Neratinib.
April 2014: Genomic testing indicated they could work, they did not. Very strange drug combo for me, felt weird.
April 2014: Started Navelbine and Herceptin. Needed something tried and true, but had significant progression.
June 2014: Doxil and Herceptin.
July 2014: Progression. Got nothing out of it. Brain: NED.
July 2014: Add integrative medical hematologist-oncologist to my team. Begin supplements. These are tumor-busting, immune system boosters. Add glutathione, lysine and taurine IV infusions every three weeks.
July 2014: Begin Gemzar, Herceptin & Perjeta. Happy.
August 2014: ECHO perfect.
January 2015: Begin weekly Vitamin D Analog infusions. 25 mcg. via port.
February 2015: CT: stable.
April 2015: Gem working, but not 100%. Looking into immunotherapy. Finally, treatments for the 21st century!
April 2015: Penn Medicine. Dendritic cell immunotherapy.
