Thumbs up CEA: How low can you go?!

Quick update from Ceesun. Recent bloodwork shows cea down from 78 in April to 2.7 now! So the plan is one more treatment with Xempra and then a Pet scan. If that looks good to my oncologist he is sending me back to radiation oncology for right lung and chest nodes. Still no word on herceptin. The beat goes on....anyone with lung mets, I have been battling mine since 2006. Hopeful. Love Cathy

Re: CEA: How low can you go?!

This is good to know Cathy! I was told last month that I have lung mets and a lymph node in my chest, and have been taking Perjeta with Herceptin and Taxol since the end of September. I’m really hoping that the Perjeta works! My first CEA levels were 2.7, which are normal, so this test does not seem to be a good indication as to whether the drugs are working for me. Best of luck to you with radiation!

¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬ ¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬
12/15 – First mammogram
01/16 – Second mammogram and ultrasound.
01/16 – Meet surgeon and go for third mammogram, ultrasound and biopsy. Surgeon confirms cancer in left breast and lymph nodes and sets surgery date.
01/16 – Chest scan and bone scan done– all looks good.
02/16 – Surgery - left breast mastectomy and 16 lymph nodes removed (8 had cancer).
02/16 – CT scan done – small nodules on lung but Doctor advises it’s normal. They will continue to monitor just in case.
03/16 – Meet radiation oncologist and find out results of Pathology Report. I’m told that I have locally advanced breast cancer, based on the size of my tumour (7 cm!) and the fact that they found cancer cells in eight lymph nodes. Also told that I’m HER 2 positive, and that my cancer is stage 3, grade 2.
03/16 – Meet oncologist and am told that my cancer is actually grade 3, and that I should have done chemo before surgery. Too late now!
03/16 – Start first of six doses of chemo (Carboplatin and Docetaxal) and Herceptin (for one year).
04/16 – Have port put in.
04/16 – Get second dose of chemo, but Docetaxal is left out due to liver enzymes being high. I was unable to get a full dose of Docetaxal after my first treatment.
06/16 – Finished chemo! One month off and then I start radiation.
06/16 – Start Tamoxifen.
07/16 – First radiation treatment – 24 more to go!
08/16 – Went for Genetic Testing to see if I have the BRCA gene.
08/16 – Radiation oncologist biopsies “scar tissue” on my scar.
08/16 – I am told that I have a “local recurrence” and need to have rush surgery.
09/16 – Meet surgeon who advises that I need to meet with a plastic surgeon, as they will need to do a skin graft to close me up after surgery.
09/16 – Go for rush ultrasound, bone scan, breast MRI and CT scan.
09/16 – Meet plastic surgeon and all looks good. A surgery date is set for October 4.
09/16 – Meet oncologist who advises that ultrasound and bone scan look good, and that MRI shows three small masses at surgery site, but lymph nodes are clear. Still awaiting the results of the CT scan, but we are positive it will look good.
09/16 – Get a call from my oncologist, who advises that CT scan shows small spots on my lungs, and a large lymph node in the middle of my chest. This means the cancer has spread! She looks into getting me funded for TDM1 and cancels my surgery.
10/16 – Meet oncologist, who advises that I have to take Perjeta before I can take TDM1. I start Perjeta/Herceptin every three weeks for an indefinite amount of time, and Taxol, which I will take two weeks on, one off for 8-16 treatments.
10/16 – Stop Tamoxifen.
10/16 – Meet surgeon, who reviews my CT scan and advises that the spots on my lungs may not be cancerous, and that he doesn’t see a lymph node in my chest. He thinks it’s a spot on my lung. I’m feeling very confused! He advises that my oncologist doesn’t want me to have surgery to remove the three small masses on my scare line, as she wants to use them as a way to determine if the treatment is working. He advises that if they have not shrunk in 6 months, he will revisit surgery.
10/16 – CEA blood test to determine Tumour markers. Results were normal (2.7). My doctor advises that this could mean two things: (1) that the treatment is working, and the tumours are shrinking, or (2), that I'm one of those people who never get elevated CEA levels. Given that some people never get an elevated CEA level, this test doesn’t seem very accurate to me! Asked for PET scan, but am told I don’t qualify.
10/16 – Brain MRI – NED!
11/16 – CT Scan scheduled for November 22. I can’t wait to find out if the Perjeta is working!

Re: CEA: How low can you go?!

Cathy--

That's awesome!! Like Tiffany, I don't have tumor markers in my blood. After a few years they finally quit wasting money testing for them. But yours are fantastic.

I'm going to keep Xempra in the back of my mind. I haven't been walking around with confirmed lung mets as long as you, but mine have been stubborn too. Last year Herceptin, Perjeta, and Abraxane had knocked out all measurable cancer in my body except in my lungs. It didn't hold on the other cancer, and the lungs started getting worse again, even causing pneumonia in my right lung.

I'm so happy for you, friend. Thank you for giving us this wonderful news.

4/29/11 CT scan shows suspicious lesions on liver and lungs
5/17/11 liver biopsy
5/24/11 liver met confirmed--Stage IV at diagnosis
5/27/11 Begin weekly Taxol & Herceptin for 3 months (standard of care at the time of my DX)
7/18/11 Switch to weekly Abraxane & Herceptin due to Taxol allergy
8/29/11 CT scan shows no new lesions & old lesions shrinking
9/27/11 Finish Abraxane. Start Herceptin every 3 weeks. Begin taking Arimidex
10/17/11--Brain MRI--No Brain mets
12/5/11 PET scan--Almost NED
5/15/12 PET scan shows progression-breast/chest/spine (one vertebra)
5/22/12 Stop taking Arimidex; stay on Herceptin
6/11/12 Started Tykerb and Herceptin on clinical trial (w/no chemo)
9/24/12 CT scan--No new mets. Everything stable.
3/11/13 CT Scan--two small new possible mets and odd looking area in left lung getting larger.
4/2/13--Biopsy of suspicious area in lower left lung. Mets to lung confirmed.
4/30/13 Begin Kadcyla/TDM-1
8/16/13 PET scan "mixed," with some areas of increased uptake, but also some definite improvement, so I'll stay on TDM-1/Kadcyla.
11/11/13 Finally get hormone receptor results from lung biopsy of 4/2/13. My cancer is no longer ER positive.
11/13/13 PET scan mixed results again. We're calling it "stable." Problems breathing on exertion.
2/18/14 PET scan shows a new lesion and newly active lymph node in chest, other progression. Bye bye TDM-1.
2/28/14 Begin Herceptin/Perjeta every 3 weeks.
6/8/14 PET "mixed," with no new lesions, and everything but lower lungs improving. My breathing is better.
8/18/14 PET "mixed" again. Upper lungs & one spine met stable, lower lungs less FDG avid, original tumor more avid, one lymph node in mediastinum more avid.
9/1/14 Begin taking Xeloda one week on, one week off. Will also stay on Herceptin and Perjeta every three weeks.
12/11/14 PET Scan--no new lesions, and everything looks better than it did.
3/20/15 PET Scan--no new lesions, but lower lung lesions larger and a bit more avid.
4/13/15 Increasing Xeloda dose to 10 days on, one week off.
7/1/15 Scan "mixed" again, but suggests continuing progression. Stop Xeloda. Substitute Abraxane every 3 weeks starting 7/13.
10/28/15 PET scan shows dramatic improvement everywhere. All lesions except lower lungs have resolved; lower lungs noticeably improved.
12/18/15 Last Abraxane. Continue on Herceptin and Perjeta alone beginning 1/8/16.
1/27/16 PET scan shows cancer is stable.
5/11/16 PET scan shows uptake in some areas that were resolved on the last two scans.
6/3/16 Begin Kadcyla and Tykerb combination
6/5 - 6/23 Horrible diarrhea from K&T together. Got pneumonia.
7/15/16 Begin Kadcyla only every 3 weeks.
9/6/16 Begin radiation therapy on right lung lesion that caused the pneumonia.
10/3/16 Last of 12 radiation treatments to right lung.
11/4/16 Huffing and puffing, low O2, high heart rate, on tiniest bit of exertion. Diagnosed as radiation pneumonitis. Treated with Prednisone.
11/11/16 PET scan shows significant improvement to radiated part of right lung BUT a bunch of new lung lesions, and the bone met is getting worse.
11/22/16 Begin Eribulin and Herceptin. H every 3 weeks. E two weeks on, one week off.
3/6/17 Scan shows progression in lungs. Bone met a little better.
3/23/17 Lung biopsy. Tumor sampled is ER-, PR+ (5%), HER2+++. Getting Herceptin and Perjeta as a maintenance treatment.
5/31/17 Port placement
6/1/17 Start Navelbine & Tykerb

Wink Re: CEA: How low can you go?!

Wonderful new on the CEA trend!

My CEA is normal at .7 or so. It trended upwards with my brain mets, so we keep using it as that was the marker that moved at that time.

Re: CEA: How low can you go?!

We actually don’t know yet if I get elevated CEA levels, as I’ve only had one test and have nothing to compare it to. I find it odd though that the first blood test I take is normal, considering that I am pretty sure that there’s still cancer in the body. I should be taking my second CEA blood test tomorrow, and those results should determine if the test works for me. As this test doesn’t seem very reliable to me, I will also be pushing for regular scans, once we know if the Perjeta is working or not.

Amy, I really hope that radiation worked on your stubborn lung mets.

Re: CEA: How low can you go?!

I found when I was at the hospital yesterday that they took a CA-15-3 blood test at my last treatment to determine my tumour markers. The results of this blood test were normal at 19. I will be taking this test again next week, so that we can compare the results, and I took a second CEA blood test yesterday. We’ll see if these test results come back normal, like they did the first time around. I am glad that the results of the CA-15-3 blood test is normal, but I'm not putting a lot of faith in these test results given that they are not accurate for everyone.

He advises that my oncologist doesn’t want me to have surgery to remove the three small masses on my scar line, as she wants to use them as a way to determine if the treatment is working.
11/16 - CA-15-3 blood test – Tumour markers are normal at 19.
11/16 – CT Scan scheduled for November 22. I can’t wait to find out if the Perjeta is working!

Re: CEA: How low can you go?!

I have had progression in my lungs (especially the right lung) on Eribulin and Herceptin. So I looked at Ixempra. I'm allergic to Taxol, and I've never had anthracycline, so I am not a candidate.

I'm sure there are other options.

Re: CEA: How low can you go?!

Amy, I was reading your signature line, and it looks like the Erbulin and Herceptin are not working for you, as your March scan shows progression in your lungs. I’m sorry to hear that, and I hope they find a new treatment plan for you soon. At least your bone met is a little better. Let us know what your next step is once you know. You don’t seem to get much of a break to you? Five years of treatment, and you still have lung and bone mets. I hope you find a treatment plans that gets and keeps you stable soon.

¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬ ¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬
12/15 – First mammogram
01/16 – Second mammogram and ultrasound.
01/16 – Meet surgeon and go for third mammogram, second ultrasound and biopsy. Surgeon confirms cancer in left breast and lymph nodes and sets surgery date.
01/16 – Chest scan and bone scan done– all looks good.
02/16 – Surgery - left breast mastectomy and 16 lymph nodes removed (8 had cancer).
02/16 – CT scan done – small nodules on lung but Doctor advises it’s post surgical. They will continue to monitor just in case.
03/16 – Meet radiation oncologist and find out results of Pathology Report. I’m told that I have locally advanced breast cancer, based on the size of my tumour (7 cm!) and the fact that they found cancer cells in eight lymph nodes. I’m also told that I’m HER 2 positive, with high levels of estrogen and progesterone and that my cancer is stage 3, grade 2.
03/16 – Meet oncologist and am told that my cancer is actually grade 3, and that I should have done chemo before surgery. Too late now!
03/16 – Start first of six doses of chemo (Carboplatin and Docetaxal) and Herceptin (for 18 months).
04/16 – Have port put in.
04/16 – Get second dose of chemo, but Docetaxal is left out due to liver enzymes being high. I was unable to get a full dose of Docetaxal after my first treatment.

11/16 – Second CEA blood test – Tumours markers are still normal at 1.6 Second CA-15-30 blood test – Tumour markers are still normal at 19
11/16 – Develop lymphedema and have to wear a sleeve
12/16 – CT Scan shows that the tumors on my lungs and the lymph node in the middle of my chest are shrinking, and that some have resolved. Also, the small masses along my scar line are no longer visible. This means the medication is working!
12/16 – Small “pimple” shows up where old tumour on chest wall was located. Doctor is going to monitor it for now.
01/17 – A second “pimple” shows up on chest wall, as well as a small lump under the skin. My doctor thinks it’s scar tissue and will monitor it for now.
0/17 – Started to develop severe back pain – worried the cancer has spread to my spine.
03/17 – Third CEA blood test and CA-15-30 blood test – awaiting results.
03/17 – CT Chest scan to see if there’s improvement to chest and lungs – awaiting results. If results are good, I get to stop taking Taxol!
03/17 – Second brain MRI scheduled for end of month.

Re: CEA: How low can you go?!

Hi Tiffany,

Thank you so much for your words of concern and encouragement. I appreciate it. And I'll definitely share what we decide to do next, and report on results.

I would not say Eribulin didn't work. My onc agrees. She said, "If you hadn't been on that treatment we'd be looking at a very different scan today." It, like many of my prior treatments, controlled the cancer relatively well. In fact, when I re-read the report I think I might have called it "stable."

I have had some physical impairment from the amount of cancer activity that I currently have. Also there's a sense of slowly losing ground. So it would be nice to find a treatment that unambiguously "resolves" more of my lesions, or, better yet, gets me to "NED." But my personal definition of "stable" is "no new lesions" and "cancer activity is about the same as last time." I have been in that zone for almost all the time I've been in treatment.

Some people get a robust remission on their first round of treatment and it lasts a long time. With respect to my liver mets, that happened for me. But then the bone met popped up, and later I got tumors in my lungs. We haven't yet knocked those out and kept them down.

I think in your case you stand a good chance of being in the first group, because you're getting the new first-round standard of care. I certainly hope so. But if you don't get a complete response, don't despair. Lots of us are harboring measurable amounts of cancer in our bodies, but it's not getting in the way too much. I'm alive and I can function. I call that a success.

All the best,
Amy

Re: CEA: How low can you go?!

Hi Amy,

I’m sorry that you’re having some physical impairment from the cancer. Is your doctor able to give you something to help with that? I hope that your doctor finds a treatment plan that will work for you. You’ve been fighting this for years now, and it seems like every time you see some improvement, it doesn’t last for long. Did you read the article Cathy posted under the heading “Cracking Cancer....A Cdn production”. Would this be something available to you where you live? If so, you should ask your doctor to send you for genetic testing. It may help her to identify why you are not responding to certain treatments. At least it can’t hurt.

Keep us posted regarding your updated treatment plan.

Re: CEA: How low can you go?!

Oh, hi Juls,

Thank you for asking.

My onc wants me to get a biopsy, and I agree. It's been awhile, and my cancer might have mutated into a different type. (Though if it has changed it won't be for the better. Such is life with "cancer as a chronic illness.")

My biopsy is scheduled for April 4. In the meantime, I got Herceptin only last week, and next time we'll add Perjeta and Xeloda. I think my one bone met is already triple negative, so I asked for "some kind of chemo." I've been on this combination before and it is probably a good choice for a maintenance treatment while we sort things out.

I didn't mention that my right lower jaw lit up on the scan. There was no identifiable lesion, just some glucose uptake. When the Dr. told me about that I said I thought it was a dental issue. I had been having tooth and jaw pain at that spot. Sure enough, I had to have a tooth pulled yesterday. For some reason it broke and split all the way down, and it was infected. The dentist showed it to me after he extracted it. I'm sorry I lost the tooth, but I'm glad it's gone now and I'm super glad it's not cancer progression.

I'll start a new thread when we come up with a new plan. It'll be another 4 weeks or so, I think.

Amy

Re: CEA: How low can you go?!

Thanks for the update Amy. Good luck to you and keep us posted regarding your new plan once you’ve gotten your test results.

On another note, what do you mean by triple negative? When I was diagnosed, I was told that I was HER II positive, with high levels of estrogen and progesterone, and, when I read my pathology report, it said the same thing however, it had a percentage assigned to each hormone. Does this mean I’m triple positive? I still don’t understand the difference between being double positive/negative and triple positive/negative, etc.

The good news is, lymph nodes are stable, there was improvement of previous lung changes due to radiation, and the cancer has not spread to my bones, spine or liver, etc. The bad news is that while some lung nodules remained stable, others increased by 1-2 mm and new ones have shown up All of them were less than 1cm.
03/17 – Second brain MRI scheduled for end of month.
03/17 – Ultrasound scheduled to determine size of “pimples” on my chest wall. They will follow up with a second Ultrasound in three months, to see if they are growing

Re: CEA: How low can you go?!

Tiffany--

They test breast cancer for hormone receptors and HER2. If all three (ER, PR, and HER2) are present, then a cancer is "triple positive." If none, then it's triple negative.

I don't actually know for a fact that my bone met is triple negative. It appeared while I was on an estrogen blocker, and it always progresses when I'm on just HER2 drugs, so I'm deducing that it is. I'm told it's hard to biopsy bone and get a good result.

Re: CEA: How low can you go?!

Thanks Amy. I guess that means that I’m triple positive.

I hope they are able to get a good result from your bone biopsy. Keep us posted.

06/16 – Finished chemo! One month off and then I start radiation.
06/16 – Start Tamoxifen.
07/16 – First radiation treatment – 24 more to go!
08/16 – Went for Genetic Testing to see if I have the BRCA gene. Tested negative for BRCA I and II
08/16 – Radiation oncologist biopsies “scar tissue” on my chest wall. I am told that I have a local recurrence and need to have rush surgery.
09/16 – Meet surgeon who advises that I need to meet with a plastic surgeon, as they will need to do a skin graft to close me up after surgery. Meet plastic surgeon and all looks good. A surgery date is set for October 4.
09/16 – Go for rush ultrasound, bone scan, breast MRI and CT scan.
09/16 – Meet oncologist who advises that the ultrasound and bone scan results look good, and that MRI shows three small masses at surgery site, but lymph nodes are clear. Still awaiting the results of the CT scan, but we are positive it will look good.
09/16 – Get a call from my oncologist, who advises that CT scan shows small spots on my lungs, and a large lymph node in the middle of my chest. This means the cancer has spread! She looks into getting me funded for TDM-1 and cancels my surgery.
10/16 – Meet oncologist, who advises that I have to take Perjeta before I can take TDM1. I start Perjeta/Herceptin every three weeks for an indefinite amount of time, and Taxol, which I will take two weeks in a row with one week off and then two weeks in a row for 8-16 treatments. Stop Tamoxifen.
10/16 – Meet surgeon, who reviews my CT scan and advises that the spots on my lungs may not be cancer, and that he doesn’t see a lymph node in my chest. He thinks it’s a spot on my lung. I’m feeling very confused! He advises that my oncologist doesn’t want me to have surgery to remove the three small masses on my scar line, as she wants to use them as a way to determine if the treatment is working.
11/16 - CA-15-30 blood test – Tumour markers are normal at 19.
03/17 – Third CEA blood test and CA-15-30 blood test – Both normal at 2.5 and 25
03/17 – CT Chest scan to see if there’s improvement to chest and lungs – results are mixed. The good news is, lymph nodes are stable, there was improvement of previous lung changes due to radiation, and the cancer has not spread to my bones, spine or liver, etc. The bad news is that while some lung nodules remained stable, others increased by 1-2 mm and new ones have shown up. All of them were less than 1cm.
03/17 – Second brain MRI scheduled for end of month.

Re: CEA: How low can you go?!

Hi Amy
Thanks for update.

Had issue with a tooth ( cracked etc) as well - went on for months because I was on Zometa - Nurse telling me it was not to be removed - dentist saying ok to remove! - I just wanted it out and dealt with!
Nothing is simple any more!!

Good luck re biopsy and with new plan.
