Re: Kadcyla - Scotland

Just read that we might not get decision from NICE about drug for a month.

Tiffany - think fatigue is a side effect so this is probably what happened. Also I'm sure I read that you can get flu like symptoms in 1st 10 days of treatment.

Re: Kadcyla - Scotland

I’m not surprised that it will take that long for a decision. Let’s hope it’s a good one though!

¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬ ¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬
12/15 – First mammogram
01/16 – Second mammogram and ultrasound.
01/16 – Meet surgeon and go for third mammogram, second ultrasound and biopsy. Surgeon confirms cancer in left breast and lymph nodes and sets surgery date.
01/16 – Chest scan and bone scan done– all looks good.
02/16 – Surgery - left breast mastectomy and 16 lymph nodes removed (8 had cancer).
02/16 – CT scan done – small nodules on lung but Doctor advises it’s post surgical. They will continue to monitor just in case.
03/16 – Meet radiation oncologist and find out results of Pathology Report. I’m told that I have locally advanced breast cancer, based on the size of my tumour (7 cm!) and the fact that they found cancer cells in eight lymph nodes. I’m also told that I’m HER 2 positive, with high levels of estrogen and progesterone and that my cancer is stage 3, grade 2.
03/16 – Meet oncologist and am told that my cancer is actually grade 3, and that I should have done chemo before surgery. Too late now!
03/16 – Start first of six doses of chemo (Carboplatin and Docetaxal) and Herceptin (for 18 months).
04/16 – Have port put in.
04/16 – Get second dose of chemo, but Docetaxal is left out due to liver enzymes being high. I was unable to get a full dose of Docetaxal after my first treatment.
06/16 – Finished chemo! One month off and then I start radiation.
06/16 – Start Tamoxifen.
07/16 – First radiation treatment – 24 more to go!
08/16 – Went for Genetic Testing to see if I have the BRCA gene. Tested negative for BRCA I and II
08/16 – Radiation oncologist biopsies “scar tissue” on my chest wall. I am told that I have a local recurrence and need to have rush surgery.
09/16 – Meet surgeon who advises that I need to meet with a plastic surgeon, as they will need to do a skin graft to close me up after surgery. Meet plastic surgeon and all looks good. A surgery date is set for October 4.
09/16 – Go for rush ultrasound, bone scan, breast MRI and CT scan.
09/16 – Meet oncologist who advises that the ultrasound and bone scan results look good, and that MRI shows three small masses at surgery site, but lymph nodes are clear. Still awaiting the results of the CT scan, but we are positive it will look good.
09/16 – Get a call from my oncologist, who advises that CT scan shows small spots on my lungs, and a large lymph node in the middle of my chest. This means the cancer has spread! She looks into getting me funded for TDM-1 and cancels my surgery.
10/16 – Meet oncologist, who advises that I have to take Perjeta before I can take TDM1. I start Perjeta/Herceptin every three weeks for an indefinite amount of time, and Taxol, which I will take two weeks in a row with one week off and then two weeks in a row for 8-16 treatments. Stop Tamoxifen.
10/16 – Meet surgeon, who reviews my CT scan and advises that the spots on my lungs may not be cancer, and that he doesn’t see a lymph node in my chest. He thinks it’s a spot on my lung. I’m feeling very confused! He advises that my oncologist doesn’t want me to have surgery to remove the three small masses on my scar line, as she wants to use them as a way to determine if the treatment is working. He advises that if they have not shrunk in 6 months, he will revisit surgery.
10/16 – CEA blood test to determine Tumour markers. Results were normal (2.7). My doctor advises that this could mean two things: (1) that the treatment is working, and the tumours are shrinking, or (2), that I'm one of those people who never get elevated CEA levels. Given that some people never get an elevated CEA level, this test doesn’t seem very accurate to me! Asked for PET scan, but am told I don’t qualify.
10/16 – Brain MRI – NED!
11/16 - CA-15-30 blood test – Tumour markers are normal at 19.
11/16 – Second CEA blood test – Tumours markers are still normal at 1.6 Second CA-15-30 blood test – Tumour markers are still normal at 19
11/16 – Develop lymphedema and have to wear a sleeve
12/16 – CT Scan shows that the tumors on my lungs and the lymph node in the middle of my chest are shrinking, and that some have resolved. Also, the small masses along my scar line are no longer visible. This means the medication is working!
12/16 – Small “pimple” shows up where old tumour on chest wall was located. Doctor is going to monitor it for now.
01/17 – A second “pimple” shows up on chest wall, as well as a small lump under the skin. My doctor thinks it’s scar tissue and will monitor it for now.
0/17 – Started to develop severe back pain – worried the cancer has spread to my spine.
03/17 – Third CEA blood test and CA-15-30 blood test – awaiting results.
03/17 – CT Chest scan to see if there’s improvement to chest and lungs – awaiting results. If results are good, I get to stop taking Taxol!

Re: Kadcyla - Scotland

That’s great Juls! It’s nice to know that so many people will now get the treatment they need without having to go into a trial!

¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬ ¬¬¬¬¬¬¬¬¬¬¬¬¬¬¬

03/17 – Third CEA blood test and CA-15-30 blood test – Both normal at 2.5 and 25
03/17 – CT Chest scan to see if there’s improvement to chest and lungs – results are mixed. The good news is, lymph nodes are stable, there is improvement of previous lung changes due to radiation, and the cancer has not spread to my bones, spine or liver, etc. The bad news is that while some lung nodules remained stable, others increased by 1-2 mm and new ones have shown up. All of them were less than 1cm.
03/17 – Received my genetic test results from Foundation One. It found 11 genomic alterations, 3 therapies associated with potential clinical benefit, and 13 clinical trials from which I might benefit.
03/17 – Second brain MRI –NED!
03/17 – Ultrasound done to determine size of “pimples” on my chest wall. They will follow up with a second Ultrasound in three months, to see if they are growing
05/17 – Full body CT scan scheduled to see if I’m still “stable”.

Re: Kadcyla - Scotland

Yes - couldn't agree more.
I would not have had to join this trial if it had been available.
I don't know if you saw post last week about the immunotherapy drug that I may be on. Report was a bit concerning. (links below) I have a 2 in 3 chance of being on Tecentriq!!

Not sure if I would have joined trial if Kadcyla had been available on NHS. But 6 weeks ago it was my only way of getting drug.

Hi Juls, I didn’t see you posts from last week regarding the immunotherapy drug. I’ll check out the links you provided when I have a few minutes to read them. I’m sad to hear that they are a bit concerning though.

Think original post with the 2 links was put up by Nyugen.
Have to say I was concerned when I read it!

Re: Kadcyla - Scotland

I did read an article posted by Nyugen regarding the potential health problems caused by immunotherapy drugs, although I think they were different articles than the links you provided above. I’ve read that people can potentially get type 1 diabetes from immunotherapy drugs, among other heath issues. It’s too bad because I think a lot of us were hoping that immunotherapy drugs would be a home run for most people! Let’s hope that it still works for most of the people getting the drugs.

So happy for everyone in Scotland to be allowed Kadcyla who needs it and hope the same will happen in England.

Re: Kadcyla - Scotland

Hi Janie
Just listened to an MSP talking about Kadcyla on TV - she didn't come across as if she knew much about it and finished with something like- for the hundreds who might need it! Sorry can't remember the exact words used as I was stuck on the hundred or so!
Lets hope it becomes available for everyone.

Re: Kadcyla - Scotland

Juls,

Those stories are kind of scary. If I were you I'd have questions.

I'm glad people in Scotland can get Kadcyla from your health service now. It's pretty darn expensive.

Best wishes,
Amy

Re: Kadcyla - Scotland

Hi Amy
Yes - was a bit concerned when I read them.
I called my Son to discuss & found myself too upset to talk! The thought that I might have put myself in this position was a bit too much.

Kadcyla here is about £90k a year - if you could afford it! So its great that its now available to all.

I see my new Oncologist on Thursday - so intend to go in with lots of questions. Also want to request Foundation One Genetic Test results from Drug Company. Apparently the Drug Co. has the report but doesn't give to Oncologist unless they are requested.
Hope he can reassure me.

Re: Kadcyla - Scotland

Let us know what your oncologist tells you about the articles. I hope he’s able to answer all of your questions. Also, keep us posted regarding your genetic testing. Hopefully the results will help you with your treatment plan down the road.

I was unable to get a full dose of Docetaxal after my first treatment due to liver enzymes being high.
04/16 – Have port put in.
They will follow up with a second Ultrasound in three months, to see if they are growing.
04/17 – Was told that I have developed atelectasis, which is what’s causing my back pain.
05/17 – Full body CT scan scheduled to see if I’m still “stable”.

Re: Kadcyla - Scotland

Hi All
Just an update.
Discussed article re hyper progression. Doctor says it could be pseudo progression which they are aware of. Also they are not expecting any major side effects. Not sure if I am reassured or not!

Also asked for Foundation One report to be requested from Drug Company records. Doctor said it might be useful in the future.

Also got results of 1st Ct report since starting this trial 6 weeks ago. Lesion in liver halved! So all pleased and relieved!!
Not a bad day after all!

Re: Kadcyla - Scotland

That’s great news Juls! I’m glad that the T-DM1 is working for you. I may be changing to T-DM1 as early as next month. I won’t be getting chemo next week, just Herceptin and Perjeta, as I will be going on vacation. I’m supposed to get a chemo break until I get back from my trip in May, but, I think I will be extending that break. If my test results from my May 11 CT scan shows progression, I’m going to tell my doctor that I want to transfer to T-DM1. If not, I will just continue with the Herceptin and Perjeta, and will take a hormone inhibiter like tamoxifen. Until I can get my breathing back to normal, and get rid of this constant back pain, I’m going to have to take a break from chemo. I know that not everyone will agree with this decision ,but, it’s mine to make, and I think it’s the right choice. Hopefully T-DM1 will work for me for a long time, so I don’t need to worry about “what’s next” for a while, as I can’t go back to Herceptin and Perjeta once I stop taking it.

Re: Kadcyla - Scotland

Hi Tiffany
Yes- good result for such a short time. Although last few days have felt terrible. Better today though!
How is back pain? Has it got any easier?
Re Herceptin & Perjeta - is that standard protocol with Perjeta. It is a trial drug here & if you progress while on it - it's immediately removed.

Re: Kadcyla - Scotland

I'm on Herceptin and Perjeta as a maintenance treatment, and it's the third time I've been on it. I also had two go-rounds with Kadcyla. Others on this board have said that once a drug "fails" it's taken off the list. Apparently that's not true everywhere.

When talking about my latest scan with my medical oncologist, she said the scan shows progression. I said, "But it seems to have done some good." She said, "Yes. We'd be looking at a very different scan if you had not had any treatment." I'm on H&P while we wait for more information about my lung tumors.

And I will keep it in the back of my mind that I haven't had progression on Abraxane, so I think it's still in my arsenal. Granted, it wasn't super effective on my lung mets, and the side effects were starting to get to me. Maybe I should try another taxane?

Juls, I'm so glad you got such a good response so far! Wonderful news.

5/24/11 liver met confirmed--Stage IV at diagnosis
5/27/11 Begin weekly Taxol & Herceptin for 3 months (standard of care at the time of my DX)
7/18/11 Switch to weekly Abraxane & Herceptin due to Taxol allergy
8/29/11 CT scan shows no new lesions & old lesions shrinking
9/27/11 Finish Abraxane. Start Herceptin every 3 weeks. Begin taking Arimidex
10/17/11--Brain MRI--No Brain mets
12/5/11 PET scan--Almost NED
5/15/12 PET scan shows progression-breast/chest/spine (one vertebra)
5/22/12 Stop taking Arimidex; stay on Herceptin
6/11/12 Started Tykerb and Herceptin on clinical trial (w/no chemo)
9/24/12 CT scan--No new mets. Everything stable.
3/11/13 CT Scan--two small new possible mets and odd looking area in left lung getting larger.
4/2/13--Biopsy of suspicious area in lower left lung. Mets to lung confirmed.
4/30/13 Begin Kadcyla/TDM-1
8/16/13 PET scan "mixed," with some areas of increased uptake, but also some definite improvement, so I'll stay on TDM-1/Kadcyla.
11/11/13 Finally get hormone receptor results from lung biopsy of 4/2/13. My cancer is no longer ER positive.
11/13/13 PET scan mixed results again. We're calling it "stable." Problems breathing on exertion.
2/18/14 PET scan shows a new lesion and newly active lymph node in chest, other progression. Bye bye TDM-1.
2/28/14 Begin Herceptin/Perjeta every 3 weeks.
6/8/14 PET "mixed," with no new lesions, and everything but lower lungs improving. My breathing is better.
8/18/14 PET "mixed" again. Upper lungs & one spine met stable, lower lungs less FDG avid, original tumor more avid, one lymph node in mediastinum more avid.
9/1/14 Begin taking Xeloda one week on, one week off. Will also stay on Herceptin and Perjeta every three weeks.
12/11/14 PET Scan--no new lesions, and everything looks better than it did.
3/20/15 PET Scan--no new lesions, but lower lung lesions larger and a bit more avid.
4/13/15 Increasing Xeloda dose to 10 days on, one week off.
7/1/15 Scan "mixed" again, but suggests continuing progression. Stop Xeloda. Substitute Abraxane every 3 weeks starting 7/13.
10/28/15 PET scan shows dramatic improvement everywhere. All lesions except lower lungs have resolved; lower lungs noticeably improved.
12/18/15 Last Abraxane. Continue on Herceptin and Perjeta alone beginning 1/8/16.
1/27/16 PET scan shows cancer is stable.
5/11/16 PET scan shows uptake in some areas that were resolved on the last two scans.
6/3/16 Begin Kadcyla and Tykerb combination
6/5 - 6/23 Horrible diarrhea from K&T together. Got pneumonia.
7/15/16 Begin Kadcyla only every 3 weeks.
9/6/16 Begin radiation therapy on right lung lesion that caused the pneumonia.
10/3/16 Last of 12 radiation treatments to right lung.
11/4/16 Huffing and puffing, low O2, high heart rate, on tiniest bit of exertion. Diagnosed as radiation pneumonitis. Treated with Prednisone.
11/11/16 PET scan shows significant improvement to radiated part of right lung BUT a bunch of new lung lesions, and the bone met is getting worse.
11/22/16 Begin Eribulin and Herceptin. H every 3 weeks. E two weeks on, one week off.
3/6/17 Scan shows progression in lungs. Bone met a little better.
3/23/17 Lung biopsy. Tumor sampled is ER-, PR+ (5%), HER2+++. Getting Herceptin and Perjeta as a maintenance treatment.
5/31/17 Port placement
6/1/17 Start Navelbine & Tykerb

Re: Kadcyla - Scotland

Hi Amy
That's good to know. I might be wrong but it seems here that once tried and failed - that's it!

Perjeta is still a trial drug here -only 2 out of 15 left on this trial which started 4 1/2 years ago. The 1st Lady ( numerous mets) on trial still doing ok but now getting really bad side effects. The other Lady ( bone mets only) doing really well at nearly 4 years. It will be interesting to see if Health Board take this drug on!
Abraxane is paclitaxel here - another I've not heard much about.
I beginning to wonder which drugs are actually available here!

Re my Ct results- I was surprised by response. Didn't expect that result!
