Getting Off Herceptin

To all my HER2 Sisters,
After 9 years of treatment and endless ECHO-cardiograms, I made a personal decision to discontinue Herceptin last February.
Although my heart was doing well, my ELVF was slowly declining. Initially it was measured at 70%, the last one showed it at 50-55%. I discussed this with Dr. Slamon at the spring AACR meeting. He supported my decision with the provision that I be closely monitored. I mentioned that I had been using the HER2 Serum test and that my oncologist would continue to include that test along with other standard markers. Dr. Slamon was completely comfortable with my plan. I will also be measured using the CEA tumor markers as it was only the CEA test that had indicated my recurrence in 1999. Even then it was only slightly elevated @ 2.59
My HER2 Serum test has always been in the 5.2 – 7.8 range which is well within the normal range. I will continue to be monitored with MRI brain scans.
I have been feeling very well lately. My vision has markedly improved after having cataract surgery last year on both eyes and I have returned to driving again.
I do not recommend discontinuing Herceptin to everyone. When I was first tested for the HER2 gene in 1999, the pathology report stated that I was strongly positive by IHC 3+++ and also stated “strongly 3+”. My initial response to Herceptin was nothing less than a miracle. I personally know of only few others who were as highly receptive as myself.

Diagnosed stage 2b in July 2003 (2.3 cm, HER2+, ER-/PR-, 7+ nodes). Treated with mastectomy (with immediate DIEP flap reconstruction), AC + T/Herceptin (off label). Cancer advanced to lung in Jan. 2007 (1 cm nodule). Started Herceptin every 3 weeks. Lung wedge resection April 2007. Cancer recurred in lung April 2008. RFA of lung in August 2008. 2nd annual brain MRI in Oct. 2008 discovered 2.6 cm cystic tumor in left frontal lobe. Craniotomy Oct. 2008 (ER-/PR-/HER2-) followed by targeted radiation (IMRT). Coughing up blood Feb. 2009. Thoractomy July 2009 to cut out fungal ball of common soil fungus (aspergillus) that grew in the RFA cavity (most likely inhaled while gardening). No cancer, only fungus. Removal of tiny melanoma from upper left arm, plus sentinel lymph node biopsy in Feb. 2016. Guardant Health liquid biopsy in Feb. 2016 showed mutations in 4 subtypes of TP53. Repeat of Guardant Health biopsy in Jana. 2021 showed 3 TP53 mutations, BRCA1 mutation and CHEK2 mutation. Invitae genetic testing showed negative for all of these. Living with MBC since 2007. Stopped Herceptin Hylecta (injection) treatment in March 2020. Recent 2021 annual CT of chest, abdomen and pelvis and annual brain MRI showed NED. Praying for NED forever!!

Christine, you and Andi are both brave ladies. I applaud your decisions to get off Herceptin. It's a big move. I wish the best for both of you.

p.s. My path report also indicated "strongly positive" Her2 +++3.
I consider myself extremely lucky to get Herceptin from the start.

Congratulations! 1990 was the year when I had my first brain (tumor) surgery. And the recurrence has been well controlled by the Gamma knife radiosurgery in 2001.

Your MUGA scores are amazing! Guess you have had a good heart to begin with.

And you look so good in your picture!

One of her secrets - WALKING!

Glad to hear that you are still holding your own and so well!

What Christine failed to mention is that she is a dedicated WALKER. She joined a walking group and has a great trail near her home. I think you said you walk 3 miles at a time? Or is it ?? miles, 3 days per week?

Anyway, as they say: "The proof is in the pudding."

I need to follow this example myself to try and get my MUGA back up!

MY STORY SO FAR ~~~~
Found suspicious lump 9/2000
Lumpectomy, then node dissection and port placement
Stage IIB, 8 pos nodes of 18, Grade 3, ER & PR -
Adriamycin 12 weekly, taxotere 4 rounds
36 rads - very little burning
3 mos after rads liver full of tumors, Stage IV Jan 2002, one spot on sternum
Weekly Taxol, Navelbine, Herceptin for 27 rounds to NED!
2003 & 2004 no active disease - 3 weekly Herceptin + Zometa
Jan 2005 two mets to brain - Gamma Knife on Jan 18
All clear until treated cerebellum spot showing activity on Jan 2006 brain MRI & brain PET
Brain surgery on Feb 9, 2006 - no cancer, 100% radiation necrosis - tumor was still dying
Continue as NED while on Herceptin & quarterly Zometa
Fall-2006 - off Zometa - watching one small brain spot (scar?)
2007 - spot/scar in brain stable - finished anticoagulation therapy for clot along my port-a-catheter - 3 angioplasties to unblock vena cava
2008 - Brain and body still NED! Port removed and scans in Dec.
Dec 2008 - stop Herceptin - Vaccine Trial at U of W begun in Oct. of 2011
STILL NED everywhere in Feb 2014 - on wing & prayer
7/14 - Started twice yearly Zometa for my bones
Jan. 2015 checkup still shows NED
2015 Neuropathy in feet - otherwise all OK - still NED.
Same news for 2016 and all of 2017.
Nov of 2017 - had small skin cancer removed from my face. Will have Zometa end of Jan. 2018.

Red face her2 serum test

Hi Leesun
The HER2 serum test has been FDA approved for metastatic breast cancer for the past few years. I feel it is a great monitor test that for her2neu overexpreassers. With a simple blood draw, the laB,Quest) has the assay test that was formulated by Bayer.
It is a very sensitive test that can show if your here2 cancer cells are progressing or decreasing. A baseline test comes first, then every 2-3 months to see if your chemo has worked or to see if they are progressing. Iam no longer taking herceptin and will be monitor any progression withis test. It has been on target forme over the past 2years. Normal goes fom 4 to 15. My average with hrceptin was arround 5 to 7. my recent test results w/o herceptin is 7.8. Iam having this test every 2-3 months as planned. I also take Cea tumor marker test that helped me in 1999 DX my relapse.
Ask your onc if he is intersted in this test for her2 cancer patients. Ifind it to work for me, while other tm tests sre and invalid, except for the cea tm test that has been my backup. Sending you well wishes, Hugs Christine

MRI - stable brain mets, no new mets, 9mm spots less enhanced, CA15.3 down 45.5 to 9.3 in 10 wks, Ty/Xel working magic!

MRI - brain mets shrunk half, NO NEW BRAIN METS!!, TMs stable @ 9.2
Oct'07~
PET/CT & MRI show NED
Apr'08~
scans still show NED in the head, small bone spot on right iliac crest (rear pelvic bone)
Sept'08~
MRI shows activity in brain mets, completed 5 fractions/5 consecutive days of IMRT to zap the pesky buggers
Oct'08~
dropped Xeloda, switched to tri-weekly Herceptin in combo with Tykerb, extend to tri-monthly Zometa infusion
Dec'08~
Brain MRI- 4 spots reduced to punctate size, large spot shrunk by 3mm, CT of torso clear/pelvis spot stable
June'09~
new 3-4mm left cerrebellar spot zapped with IMRT targeted rads
Sept'09~
new 6mm & 1 cm spots in pituitary/optic chiasm area. Rx= 25 days of 3D conformal fractionated targeted IMRT to the tumors.
Oct'09~
25 days of low dose 3D conformal fractionated targeted IMRT to the bone mets spot on rt. iliac crest that have been watching for 2 years. Added daily Aromasin back into treatment regimen.
Apr'10~ Brain MRI clear! But, see new small spot on adrenal gland. Change from Aromasin back to Tamoxifen.
June'10~ Tumor markers (CA15.3) dropped from 37 to 23 after one month on Tamoxifen. Continue to monitor adrenal gland spot. Remain on Tykerb/Herceptin/Tamoxifen.
Nov'10~ Radiate positive mediastinal node that was pressing on recurrent laryngeal nerve, causing paralyzed larynx and a funny voice.
Jan'11~ MRI shows possible activity or perhaps just scar tissue/necrotic increase on 3 previously treated brain spots and a pituitary spot. 5 days of IMRT on 4 spots.
Feb'11~ Enrolled in T-DM1 EAP in Denver, first treatment March 25, 2011.
Mar'11~ Finally started T-DM1 EAP in Denver at Rocky Mountain Cancer Center/Rose on Mar. 25... hallelujah.
