Hi from Adriana- Brain MRI- mixed results +

Hi everyone,

Just checking in with everyone.

Here's the latest:

Had Brain MRI about a week ago, doctor said mixed results with marked improvement in one area but some worsening in the other--don't know which part of the brain he's taking about? Doc said not to worry since it could be necrosis. Will see Doc/Radiology Oct 31st and will post results.

October 24 had a guided needle biopsy; it went well, minimum bleeding, felt great afterwards, went home had dinner with family. Yesterday doctor called and said the surgeon did not get to the tumor, just the lung tissue, so it came back negative for cancer. I will have to have yet another biopsy done or a bronchoscopy-----he mentioned the latter is a much better procedure since the tumor is down the pipe near the trachea. I have been upset since yesterday, I felt sooo good after the biopsy and confident that the surgeon had gotten a big sample off my lung. It's not the end of the world but can't help to feel a little angry!! Now, the biopsy is much needed in order to have the sample sent to Foundation One for the tumor profiling test. The one I had at home and sent to them wasn't big enough for Foundation One to run the test. So, what do you guys think? Am I wasting my time and energy? Should I proceed with the biopsy. I need some TLC, I feel so depressed today, and this is not me. Any comments will be appreciated, even if you have no opinion either way about my getting the biopsy. 2013- March Bone density shows small spot on 5th rib.
2013 - April 4th appt with onc. will post after discussing course of treatment.
2013-March-April Cyber knife to brain and radiation to rib. Chest --base line before chemo-CT-Scan stable for lung issue. CA2729 Normal.
2013 April Herceptin- TDMI
2013 Sept Herceptin + Perjeta . CA2729 within normal range. Brain and Pet scans October 31st. will post results.
2013 October Brain MRI- mixed response. Will see Onc/rad on Halloween.
2013 October/November Brain-MRI nothing new. Repeat MRI next year in May.

2013 December Continue Herceptin and Perjeta. Stable at the moment.
2014 February Brain MRI -clear!
2014 January Added Taxotere to Perjeta+Herceptin.
2014 March Stopped chemo-chest ct-scan next.

2014- March Scans shows tumor's larger, CA2729 higher. Discontinue Herceptin.
2014 April Perjeta+ Halaven
2014 April CA2729 went down 60 points after one cycle. Cough does not want to go away.
2014 June Continue on Perjeta + Halaven-- no more cough. Stable
2014 June Back on Herceptin + abraxane

Adriana, the biopsy was a bit of bad luck, but perhaps after you have "de-stressed" you will feel able to try again. It should rest on how important you and your docs regard the tumour profiling.

Diagnosed 2004: Lumpectomy - 2 tumours, both grade 1 infiltrating duct carcinoma, about 12mm. ER+,
C-erbB-2 status 3+.
Clear margins, no nodal involvement.
Radiotherapy, i year Tamoxifen, 4 years Arimidex.
Rediagnosed 2012: Multiple bone metastases.
3/12: began on Marianne trial - T-DM1 + Pertuzamab/Placebo.
5/12:Unexpected development of numerous bilateral liver mets. Came off trial.
Started Docetaxol/ Herceptin + Zometa.
8/12:Bones stable +major regression in liver (!)
9/12:Can't take any more Docetaxol! Start on Herceptin and Tamoxifen. Cross fingers!
Changed to Denosumab.
11/12: Scan shows stable - yay!
11/13: Still stable :-) !!!
1/16: All stable, but lowered calcium, so switched to Zometa 3 monthly.
2/19: Happily still stable on Herceptin, Letrozole and 3 monthly Zometa.

Sorry to hear that news -- one of the things I hate the most is the non-definitive test result/biopsy/scan etc. I never even knew this was a possibility until I wasted a contrast MRI super-sexed up ultra brain scan (I can't recall the actual test name) and it was non-definitive. So frustrating!!! I know you will regroup and hang in there. Keep us posted. Sending a huggggg!!!!

1996 cancer WTF?! 1.3 cm lumpectomy Er/Pr neg. Her2+ (20nodes NEGATIVE) did CMF + rads. NED.
2002 recurrence. Bilateral mastectomy w/TFL autologous recon. Then ACx2. Skin lymphatic rash. Taxotere w/Herceptin x4. Herceptin/Xeloda. Finally stops spreading.
2003 - Back to surgery, remove skin mets, and will have surgery one week later when pathology can confirm margins.
‘03 latisimus dorsi flap to remove skin mets. CLEAN MARGINS. Continue single agent Herceptin thru 4/04. NED.
‘04 '05 & 06 tiny recurrences - scar line. surgery to cut out. NED each time.
1/2006 Rads again, to scar line. NED.

3/07 Heartbreaking news - mets! lungs.sternum. Try Tykerb/Xeloda. Tykerb/Carbo/Gemzar. Switch Oncs.
12/07 Herceptin.Tykerb. Markers go stable.
2/8/08 gamma knife 13mm stupid brain met.
3/08 Herceptin/tykerb/avastin/zometa.
3/09 brain NED. Lungs STABLE.
4/09 attack sternum (10 daysPHOTONS.5 days ELECTRONS)
9/09 MARKERS normal!
3/10 PET/CT=manubrium intensely metabolically active but stable. NEDhead.
Wash out 5/10 for tdm1 but 6/10 CT STABLE, PET improving. Markers normal. Brain NED. Resume just Herceptin plus ZOMETA
Dec 2010 Brain NED, lungs/sternum stable. markers normal.
MAR 2011 stop Herceptin/allergy! Go back on Tykerb and switch to Xgeva.
May-Aug 2011 Tykerb Herceptin Xgeva.
Sept 2011 Tykerb, Herceptin, Zometa, Avastin.
April 2012 sketchy drug trial in NYC. 6 weeks later I’m NED!
OCT 2012 PET/CT shows a bunch of freakin’ progression. Back to LA and Herceptin.avastin.zometa.
12/20/12 add in PERJETA!
March 2013 – 5 YEARS POST continue HAPZ
APRIL 2013 - 6 yrs stage 4. "FAILED" PETscan on 4/2/13
May 2013: rePetted - improvement in lungs, left adrenal stable, right 6th rib inactive, (must be PERJETA avastin) sternum and L1 fruckin'worsen. Drop zometa. ADD Xgeva. Doc says get rads consultant for L1 and possible biopsy of L1. I say, no thanks, doc. Lets see what xgeva brings to the table first. It's summer.
June-August 2013HAPX Herceptin Avastin Perjeta xgeva.
Sept - now - on chemo hold for calming tummy we hope. Markers stable for 2 months.
Nov 2013 - Herceptin-Perjeta-Avastin-Xgeva (collageneous colitis, which explains tummy probs, added Entocort)
December '13 BRAIN MRI ned in da head.
Jan 2014: CONTINUING on HAPX…
FEB 2014 PetCT clinical “impression”: 1. newbie nodule - SUV 1.5 right apical nodule, mildly hypermetabolic “suggestive” of worsening neoplastic lesion. 2. moderate worsening of the sternum – SUV 5.6 from 3.8
3. increasing sclerosis & decreasing activity of L1 met “suggests” mild healing. (SUV 9.4 v 12.1 in May ‘13)
4. scattered lung nodules, up to 5mm in size = stable, no increased activity
5. other small scattered sclerotic lesions, one in right iliac and one in thoracic vertebral body similar in appearance to L1 without PET activity and not clearly pathologic
APRIL 2014 - 6 YRS POST GAMMA ZAP, 7 YRS MBC & 18 YEARS FROM ORIGINAL DX!
October 2014: hold avastin, continue HPX
Feb 2015 Cancer you lost. NEDHEAD 7 years post gamma zap miracle, 8 years ST4, +19 yrs original diagnosis.
Continue HPX. Adding back Avastin
Nov 2015 pet/ct is mixed result. L1 SUV is worse. Continue Herceptin/avastin/xgeva. Might revisit Perjeta for L1. Meantime going for rads consult for L1
December 2015 - brain stable. Continue Herceptin, Perjeta, Avastin and xgeva.
Jan 2016: 5 days, 20 grays, Rads to L1 and continue on HAPX. I’m trying to "save" TDM1 for next line. Hope the rads work to quiet L1. Sciatic pain extraordinaire :((
Markers drop post rads.
2/24/16 HAP plus X - markers are down
SCIATIC PAIN DEAL BREAKER.
3/23/16 Laminectomy w/coflex implant L4/5. NO MORE SCIATIC PAIN!!! Healing.
APRIL 2016 - 9 YRS MBC
July 2016 - continue HAP plus Xgeva.
DEC 2016 - PETCT: mets to sternum, lungs, L1 still about the same in size and PET activity. Markers not bad. Not making changes if I don't need to. Herceptin/Perjeta/Avastin/Xgeva

December 2017 - Progression - gonna switch it up
FEB 2018 - Kadcyla 3 cycles ---->progression :(
MAY30th - bronchoscopy, w/foundation1 - her2 enriched
Aug 27, 2018 - start clinical trial ZW25
JAN 2019 - ZW25 seems to be keeping me stable
APRIL 2019 - ONE DOZEN YEARS LIVING METASTATIC
MAY 2019 - progression back on herceptin add xeloda
JUNE 2019 - "6 mos average survival" LMD & CNS new single brain met - one zap during 5 days true beam SBRT to cord met
10/30/19 - stable brain and cord. progression lungs and bones. washing out. applying for ds8201a w nivolumab. hope they take me.
12/27/19 - begin ds8401a w nivolumab. after 2nd cycle nodes melt away. after 3rd cycle chest scan shows Improvement, brain MRI shows improvement, resolved areas & nothing new. switch to plain ENHERTU. after 4th cycle, PETscan shows mostly resolved or improved results. Markers near normal. I'm stunned but grateful.
10/26/20 - June 2021 Tucatinib/xeloda/herceptin - stable ish.

Sheesh!
Can't things ever be easy when we need it most?

I know it's easier said than done but... try not to worry ahead of the news.
Stress is bad for us on our best days... and girl!...
I can't remember my last "best" day... how 'bout you?!

I have no knowledge about lung mets or the biopsies that go with them.
But necrosis?! I feel like I could write a book.
The shadows and images on those MRIs are so nebulous.
This, from the woman whose brain stem looks just like the Mickey Mouse Club emblem.
For 3 scans in a row, the docs kept saying new disease... and I kept saying "I don't see it".
Now, they agree it's necrosis.

Hang in there. Sending prayers for peace of mind, and healing.
And for a healthy dose of good judgement for the members of the team that's cared for you so well this far.

I think you should go for the biopsy. I understand there are times and things that make us unhappy, specially when it is not our fault, but somebodyelse's. If you have the biopsy, you will get additional information that if you do not, so, better get it. Relajate y no pienses en ello, hasta el dia que tengas que ir a hacerlo. Cuidate mucho!! Love you!

Sending hugs and good wishes for Halloween to be happy. I think we all understand the frustration of mistakes and take it hard because it is so critical to our health but give it a little time and see how you feel about the bronchoscopy after the anger and frustration wear off. You may see it differently and more clearly especially if the results will affect treatment etc.

Hey Addie!

I'm a big believer in necrosis. Stable since '99 with some hiccups this past yr, but benign was a gift and the bottom line. The agitation of prep and recovery which lasted mnths has mostly subsided for me.

I did feel anger for you, Addie! Failed biopsy???!!! How infuriating. Yet, you must move on. And I know you know that.

The needed tissue is a thing I believe in beyond hearing benign, finding additional info is critical.

I asked my surgeon last June for some extra tissue just in case I had cancer, which I believed 99.9% I did NOT, and he immediately, without hesitation, waved his hand and said, I'm not getting any extra tissue! Dismissed.

Last time I checked, we were talking about MY body. Bastard. No discussion.

Well, you vent away. I find it incredibly relieving myself. And this is the place to do that to your heart's content. We get it. All too well. No deaf ears here.

Just love -- and HUGS too of course!!

'95 post-meno dx Invasive LOBULAR w/9cm tumor! YIKES + 2/21 nodes. Clear mammo 10 mnths earlier. Mastec/tram flap reconst/PORT/8 mnths chemo (4Adria/8CMF). Borderline ER/PR. Tamoxifen 2 yrs. Felt BLESSED. I could walk and talk, feed and bathe myself! I KNEW I would survive...

'98 -- multiple mets to liver. HER2+ 80%. ER/PR- Raging, highly aggressive tumors spreading fast. New PORT. 9 mnths Taxotere Fought fire w/fire! Pronounced in cautious remission 5/99. Taxotere weekly for 6 wks, 2 wks off -- for 9 mnths. TALK ABOUT GRUELING! (I believe they've altered that protocol since those days -- sure hope so!!)
+ good old Vit H wkly for 1st 3 yrs, then triple dosage ev 3 wks for 7 yrs more... The "easy" chemo, right?! Not a walk in the park, but not a freight train coming at 'ya either...

Added Herceptin Nov '98 (6 wks after FDA fast-tracked it for met bc). Stayed w/Vit H till July '08! Now I AM FREE! Humbly and eternally grateful for this life-saving drug! NED since '99 and planning on keeping it that way. To hell w/poor prognosis and nasty stats! STOPPED VIT H JULY '08...! REMAIN STABLE... Eternally grateful...Yes is a world & in this world of yes live (skillfully curled) all worlds ... (e e cummings) EVERY DAY I BEAT MY PREVIOUS RECORD FOR # OF CONSECUTIVE DAYS I'VE STAYED ALIVE. Smile KNOWING you too can be a miracle. Up to me and God now...

Vent to your hearts content and know that we are all in your corner holding you up with love and support. Give yourself a day or so to calm your emotions then make your decision on how to proceed.

7-10-2019 CT to check lung nodule.
1-10-2020 8mm stable nodule on R Lung, two 6mm new ones on L Lung, a possible lymph node involvement in inter fissule.

I'm so sorry you are going through this, the "wait and then wait some more" games we hae to go through is truly harder than just getting the cold facts right away. I am not very patient and I would want a conclusive biopsy for myself. I will be praying for you....sherryg

Lately it seems like I am a day late and a dollar short keeping up with responding to all the threads on the site. Im sorry I didnt respond to your thread sooner. I hope you are feeling some better now, but Im sending you big cyber hugs anyway and lots of prayers. Always remember how much everyone here cares about you, and that you are never alone.
I agree with Sherry in that I would get the biopsy redone so I could get the very best directed treatment based on the results. I had a couple nodules in my left lung that turned out to be nothing cancerous at all, so we can all hope and pray that your results will be similar.
I had two lesions left in my brain a few weeks out from WBR, and they are thinking now that they were necrosis and not tumors after all. And like Denise said, she could write a whole book on necrosis. I think that it must be a pretty common misdiagnosed finding. I also had an area of necrosis in my liver that they thought was a lingering tumor.
I went to nursing school and worked in the medical field for over 15 years, and my doctor seems to not recognize that and explain everything to me on a preschool level. Sometimes it makes me angry, and sometimes I just have to laugh because its so silly. But one day he sat and looked very serious at me and said...the thing in your liver is like a dead horse. The horse is dead, but the carcass is still there decaying. o.O Even though I felt like karate chopping him in the adam's apple and hollering..I know what necrosis is!!!!! I just smiled and nodded and said, good deal. So...here's hoping for horse carcasses in your brain!

Diagnosed in October 2011 Stage IV with metastasis to liver.
January 2012 after double mastectomy, started taxotere, carboplatin and herceptin.
Clear.
December 2012 was diagnosed with five brain mets, and had whole brain radiation.
Around July 2014 two mets in brain, one a residual spot and one new one growing in size. Received Cyberknife on both areas
