My 2 year run is over
Hello ladies,
Well, I knew this day was coming but it is still hard to accept. Met with onc yesterday after I insisted I get a PET and it is back in my lungs and a portion of my liver. I also am having rib pain in the lower back. Onc wants to do biopsies if possible. I am not sure I want to.
Can anyone tell me what their experience has been having mets biopsied?
Thanks for any help as I am struggling with this decision.

I am so sorry about the PET...again, good thing you insisted on it.....I am curious, maybe they want biopsies to confirm that the cancer has the same characteristics... I know Chris just underwent a liver biopsy....so many times it seems the makeup of the tumor is different than the original, requiring a different approach....sending prayers and love that you will soon have answers and a plan in place....stay strong, stay positive and know we are all with you in this new journey.

Oh Emilie. Just after I posted I read your message. I had hoped you were on the good road now. I was almost sure. Now you are back at the drawing board like me. Nobody wants a biopsy but we know now that 30% of the tumors change. So if a biopsy can be done, it can be interesting. I don't know what I would do myself to tell the truth. I am sending hugs. I am very sorry to read your news. I tend to agree with Sheila and your oncologist. Sometimes the characteristics of tumors change.
I wonder if your oncologist can also set up some chemo sensitivity testing after the biopsy to see what kind of chemo would work the best?

I am sorry about your PET scan news. I think if your tumor(s) can be biopsied, they should be just in case your cancer is now hormone positive because that brings more weapons to the battle. I am thinking about you.

I am sorry to hear your news.
I hada a lung biopsy last week - still waiting for results - we want to confirm that the cancer has same characteristics and that we are treating it optimally. Unfortunately we didn't get enough tissue for more than the standard testing.

August 2012 (turn 40!) tykerb and herceptin (denied compassionate use of TDM1) while holidaying in Italy!
September 2012 - January 2013 TDM1 as part of the Th3resa trial - lymph nodes resolved, lungs slowly progressing.
January 2013 - herceptin, carboplatin and Perjeta (compassionate access)
April 2013 - Some progression in lungs and lymph nodes - Abraxane, Herceptin and Perjeta
July 2013 - mixed response - dramatic reduction of most lung disease, progression of smaller lung nodules and cervical and hilar nodes - ? Add avastin.

Not the news any of us mets ladies want to hear. But once you get this under control, you will have another run.
I had a lung biopsy. They went in through my back. I was awake in a CT scanner, which is how the interventional radiologist finds the nodule. I coughed up blood during the procedure and was very happy to have first read about that possibility (it doesn't always happen). Otherwise, it would have really frightened me. A slight tickle and then a small cough. I knew what was coming, so it was okay.

Diagnosed stage 2b in July 2003 (2.3 cm, HER2+, ER-/PR-, 7+ nodes). Treated with mastectomy (with immediate DIEP flap reconstruction), AC + T/Herceptin (off label). Cancer advanced to lung in Jan. 2007 (1 cm nodule). Started Herceptin every 3 weeks. Lung wedge resection April 2007. Cancer recurred in lung April 2008. RFA of lung in August 2008. 2nd annual brain MRI in Oct. 2008 discovered 2.6 cm cystic tumor in left frontal lobe. Craniotomy Oct. 2008 (ER-/PR-/HER2-) followed by targeted radiation (IMRT). Coughing up blood Feb. 2009. Thoractomy July 2009 to cut out fungal ball of common soil fungus (aspergillus) that grew in the RFA cavity (most likely inhaled while gardening). No cancer, only fungus. Removal of tiny melanoma from upper left arm, plus sentinel lymph node biopsy in Feb. 2016. Guardant Health liquid biopsy in Feb. 2016 showed mutations in 4 subtypes of TP53. Repeat of Guardant Health biopsy in Jana. 2021 showed 3 TP53 mutations, BRCA1 mutation and CHEK2 mutation. Invitae genetic testing showed negative for all of these. Living with MBC since 2007. Stopped Herceptin Hylecta (injection) treatment in March 2020. Recent 2021 annual CT of chest, abdomen and pelvis and annual brain MRI showed NED. Praying for NED forever!!

Dear emelie,
Although a biopsy is not fun, in the long run it is important to know what you are dealing with. Tumor biology can change, and is the key to finding the best treatment options.
I've had 3 biopsies of my liver, once when first dx with meta (in part because they couldn't believe it was from the "cured" DCIS. Then one last summer to be sure we still knew the her2 and ER/PR status. And one in December which was required as part of a clinical trial.
It is an outpatient procedure. The first two went without hitches, uncomfortable but doable, and I was up and about feeling pretty much back to normal in a day or so. I had some complications with the last one (some bleeding - a perfect storm of low platelets, vascular tumor and large bore needle!) which won me a night in a luxury room with views of SF bay and bridge while being "observed" as a precaution, but was still up and recovered similarly to the other two. Not trying to scare you off, its a small risk, the radiologist hadnt ever had that happen and he was royally p.o.'d, That was the "small risk" thing, guess it was just my turn!
Anyway, I do think it's worth it to get whatever info you can, the better to choose weapons my dear!

Im so sorry to hear your news. I, too, had a liver biopsy a couple months ago. I was awake, and in a CT machine when they did it. They gave me some medication to help relax me before, and they inject you with the numbing medication before the actual biopsy; however, it still was a bit uncomfortable just for a minute. Then I had to lay on my stomach for four hours in a hospital room before they allowed me to go home. I was sore for a couple days, I think, but was up and around and feeling okay. I went back to work the next day...I feel that it is important to get the biopsies done because, as everyone else said, these tumors change characteristics, and you want to be able to get the most effective treatment.

January 2012 after double mastectomy, started taxotere, carboplatin and herceptin.
Clear.
December 2012 was diagnosed with five brain mets, and had whole brain radiation.
Around July 2014 two mets in brain, one a residual spot and one new one growing in size. Received Cyberknife on both areas
Clear/NED
April 2015 remain NED

October 2007 three x 2.5cm lung mets. 8 months Taxol, started Herceptin and continue. Significant reduction in lung mets.
June 2011 3cm x 4cm liver tumour. Started Abraxane and continue with Herceptin.
November 2011. Finished with Abraxane, continue with just Herceptin. Liver tumour now reduced to 15mm x 12mm. Lung tumour now 10mm x 0.5mm
February 2012. Scans show everything stable, and brain scan clear.
July 2012. PET/CT scans show I'm in remission - no active cancer!
]Dec CT brain cllear, lungs stable, liver tumour has increased to 20mm. PET scans showed active liver met and active lung thinglet, and possible bone met.
Jan 2013 recommence Abraxane, continue with Herceptin.
June 2013 finish Cycle 6 Abraxane, continue with Herceptin. 30% reduction in liver tumour, everything stable.
December 2013. CA15-3 on rise.
February 2014. PET and CT scans show single liver tumour has increased to 35mm. No other activity.
March 2014. Planned for SBRT for liver met, but couldn't have treatment as tumour too close to bowel. Continue Herceptin.
April 2014. Surgeon advises that I am a good candidate for liver resection, so will have operation early May (after camping holiday). Tumour now 44mm x 29mm.
May 7, 2014. Two liver tumours surgically removed. Third of liver removed, and gall bladder. Am I NED?May 2014. Pathology of tumour shows it's now ER+ (95% staining).
June 2014. CA15-3 has decreased to 18 from a pre-surgery reading of 59!
June 2014. Started Femara, continue with Herceptin.
July 2014. Stop Femara due to severe Osteoporosis. Commence Tamoxifen, continue Herceptin. Waiting to hear if I can have Aclasta infusion.
August 2014. CA15-3 has decreased further to 12 - YAY!
October 2014. Aclasta infusion for Osteoporosis. November 2014, CA15-3 decreased to 11. Scans of liver all clear, something new showing up on lung, but just watching at the moment.
November 2015. Started SBRT on solitary lung met.
November 2015. Bone density scan showed very good improvement so back on Femara - yay!
December 2016. 6 treatments of SBRT radiation on lung. Seems to have had some effect.
June 2016. CA15-3 still stable and low at 9.
June 2016. Started subcutaneous Herceptin replacing infusion.
Jan 2017. LVEF dropped to 46%. Stopped Herceptin.
Feb 2017. Started ACE Inhibitor and BETA Blocker. Still off Herceptin.
Aug 2017. Two new mets - Portacaval lymph node and mediastinal lymph node.
Aug 2017. Blood tests show extremely elevated liver enzyme levels. Many tests to investigate.
Sept 2017. Portacaval lymph node blocking liver bile duct causing liver enzyme and Bilirubin problems.
Oct 2017. 8cm stent inserted into liver bile duct. Procedure caused pancreatitis, and hospitalised for 3 days. Liver enzymes improving rapidly.
Nov 2017. Commenced 4 weeks of radiation on Portacaval lymph node. 5 week break before chemo.
Jan 2018. CT scan. 11 new small liver mets, and new superclavical lymph node med.
Jan 2018. Start Kadcyla. CA15-3 426.
Apr 2018. First scans since starting Kadcyla. All tumours reducing. CA15-3 dropped to 30 from 426.
Dec 2019. Still on Kadcyla, but two small brain mets have been treated in the past month with SRS. CA15-3 stable for 12 months at 11.

Thank you all for your thoughts, prayers and advice. I am waiting to hear if the sites can be accessed and wish we could move a little faster.
My husband and I have been planning a trip to Paris in May to visit our lovely daughter who moved there in Oct. She is in this picture. I just need to feel a little bit better so I can make this trip. Fingers crossed and I know I have to keep moving forward one foot at a time.
I will keep you all updated as to the treatment plan etc. for anyone else who has similar situation.

Sept. 2011- PET, Mammo, Bone Scan all clear- Herceptin only
March 2012-Recurrence in lungs and tumor around pota hepatis artery. Added Navelbine to herceptin
April 2012- Entered palliative care
June 5, 2012-Entered hospice care

I just thought I would let you know that I saw my oncologist today and got my biopsy results back - unfortunately they "missed" - muscle and normal tissue - they were sure they got the lesion at the time. My larger lung mets are about 11mm and we re trying to get one close to the outer part of the lung as there is less risk of pneumothorax.
I am terribly disappointed but have spoken to a friend tonight who is an interventional radiologist - we are going to try again after my next scan. I really want some tissue as the preliminary results of testing on my initial tumor tissue show that the cancer was resistant to anthracylcines and taxotere - hence progression on adjuvant therapy. As metastatic sites can be different I am keen to know that we are using the best treatments that we can.
I wish you the very best and feel you will have a treatment plan in place very sooner and on your way back to NED.

22 February 2011 - Diagnosed Early Breast Cancer IDBC Stage2b (ER/PR -ve, Her2+ve +++) - 38 years old
(L) skin sparing mastectomy with tissue expander, axilla clearance (2/14 affected) clear margins.
Fec*3, Taxotere and herceptin*2 - stopped due to secondary diagnosis
June 24 2011 Stage IV - Skin met, axilla node, multiple lung lesions
Bolero3 trial - Navelbine, Hereptin weekly, daily Everolimus/Placebo
February 2012 - July 2012 Tykerb and Xeloda - skin mets resolved, Lungs initially dramatically reduced but growing again
August 2012 (turn 40!) tykerb and herceptin (denied compassionate use of TDM1) while holidaying in Italy!
September 2012 - January 2013 TDM1 as part of the Th3resa trial - lymph nodes resolved, lungs slowly progressing.
January 2013 - herceptin, carboplatin and Perjeta (compassionate access)
April 2013 - Some progression in lungs and lymph nodes - Abraxane, Herceptin and Perjeta
July 2013 - mixed response - dramatic reduction of most lung disease, progression of smaller lung nodules and cervical and hilar nodes - ? Add avastin.
