Question What drug combo will be next?

I did a year of herceptin following a metastatic breast cancer in my right lung(2006). Then a tumor was found in my left lung. I did 9 months of Tykerb and Xeloda (2007). Another tumor was found just outside my upper left lung (June 2008); my onc is studying the pathology report and trying to decide the next medicine to try.

Has anyone else gone back on herceptin after once being herceptin resistant? What other drug did they give you along with it?

Did you do Herceptin with Taxol? or by itself? I think the new wisdom is that your cancer doesn't always become Herceptin resistant (but can to Taxol/other chemos), and that it is valid to go back to Herceptin, potentially in combo with other chemos or with Tykerb.

Feb'07~ Genetic testing, BRCA 1&2 neg

Apr'07~
MRI - two 9mm brain mets & 5 punctates, new left chest met, & small increase of bone spot C3 neck, Stopped Aromasin
May'07~
Started Tykerb/Xeloda, no WBR for now
June'07~
MRI - stable brain mets, no new mets, 9mm spots less enhanced, CA15.3 down 45.5 to 9.3 in 10 wks, Ty/Xel working magic!
Aug'07~
MRI - brain mets shrunk half, NO NEW BRAIN METS!!, TMs stable @ 9.2
Oct'07~
PET/CT & MRI show NED
Apr'08~
scans still show NED in the head, small bone spot on right iliac crest (rear pelvic bone)
Sept'08~
MRI shows activity in brain mets, completed 5 fractions/5 consecutive days of IMRT to zap the pesky buggers
Oct'08~
dropped Xeloda, switched to tri-weekly Herceptin in combo with Tykerb, extend to tri-monthly Zometa infusion
Dec'08~
Brain MRI- 4 spots reduced to punctate size, large spot shrunk by 3mm, CT of torso clear/pelvis spot stable
June'09~
new 3-4mm left cerrebellar spot zapped with IMRT targeted rads
Sept'09~
new 6mm & 1 cm spots in pituitary/optic chiasm area. Rx= 25 days of 3D conformal fractionated targeted IMRT to the tumors.
Oct'09~
25 days of low dose 3D conformal fractionated targeted IMRT to the bone mets spot on rt. iliac crest that have been watching for 2 years. Added daily Aromasin back into treatment regimen.
Apr'10~ Brain MRI clear! But, see new small spot on adrenal gland. Change from Aromasin back to Tamoxifen.
June'10~ Tumor markers (CA15.3) dropped from 37 to 23 after one month on Tamoxifen. Continue to monitor adrenal gland spot. Remain on Tykerb/Herceptin/Tamoxifen.
Nov'10~ Radiate positive mediastinal node that was pressing on recurrent laryngeal nerve, causing paralyzed larynx and a funny voice.
Jan'11~ MRI shows possible activity or perhaps just scar tissue/necrotic increase on 3 previously treated brain spots and a pituitary spot. 5 days of IMRT on 4 spots.
Feb'11~ Enrolled in T-DM1 EAP in Denver, first treatment March 25, 2011.
Mar'11~ Finally started T-DM1 EAP in Denver at Rocky Mountain Cancer Center/Rose on Mar. 25... hallelujah.

I've seen a lot of people do well with Navelbine and Herceptin for lung mets.

12/08 Start TDM1 trial
1/09 Scans show stable
12/09 1 year on TDM1 still stable
10/10 progression in chest and liver
11/10 false positive of liver mets; tykerb and herceptin
4/11 Tykerb/Herceptin/Xgeva
4/11 Rads to Sternum
5/12/12 NED Herceptin/Zometa
3/16/19 still NED Herceptin/Zometa very 6months

The year I did Herceptin, the only thing I was taking was Herceptin.

Is Navelbine given through an IV or is it an oral chemo?

Navelbine is an IV chemo, and I also know of people who have done well with this + Herceptin for lung mets.

As for redoing Herceptin, that is very common to continue with Herceptin and just add a chemo because you can still get synergy by adding Herceptin.

Now that Tykerb is approved, I expect you could alternate herceptin and tykerb combinations as well as use them together (there are trials ongoing for this). The point is, continue to target the Her2 with something and boost it with chemo as needed.

Brenda's question is also important, if you have used a taxane, because these work well with Herceptin. But so does Navelbine.

What drug combo will be next?

Janet, Genentech just about completed a 100+ patient Herceptin-DM1 trial, it was conducted at 40 some sites and recruited from MBC patients that had progressed on Herceptin. Company sounds very excited about results they seen so far. A new phase II is scheduled to start this month, an additional inclusion item is that candidates have been exposed to Tykerb and/or Xeloda.

Side effects from drug are minimal! Mention the trial to your oncologist.

you can also email michele fernandes at genentech to find the nearest location to you.. these trials are scheduled to open in july sometime and will be run worldwide..
roche is operating this trial in munich ,germany and other european cities for those across the pond...

Hi Janet,

Definitely give Navilbene with Herceptin or Tykerb some serious consideration. I had lung and liver mets, started Navilbene and Herceptin in January. I have since had to switch due to liver progression, but I haven't seen hide nor hair of those lung mets for the past 7 months since Navilbene wiped them off the face of the CAT scan. It's also a relatively easy combo to tolerate.

Here is the plan....I am going back on weekly Herceptin IV infusions as well as a Faslodex shot every 4 weeks to continue fighting the estrogen that feeds my cancer cells. Does anyone have any Faslodex experience?
