How long for T-Dm1 induced neuropathy to go away ...

Hello,

Does anyone happen to know/experience how long for T-Dm1 induced neuropathy to go away after stopping the drug?

I still have neuropathy after being off Kadcyla for 4 years. I am assuming it is permanent.

How bad was your neuropathy after you ended treatment? Was it to the point that you feel weakness such as nearly couldn't open juice bottle with plastic cap due to either gripping the bottle or twisting the cap?

By the way what is/was your next treatment after T-Dm1?

My neuropathy is mainly numbness in both hands and feet. I have been receiving Perjeta and Herceptin since stopping TDM-1

I have had some degree of neuropathy since my first treatment, but it definitely ebbs and flows. I had some neuropathy after TDM-1, though less severe than with taxanes.

I recently stopped getting Eribulin, and my neuropathy is less severe.

The impression I get is each person is different. I have noticed decreased grip strength, but I can open jars. I can knit, color, use a knife, fasten necklaces, put in ear rings. I always have at least some numbness and tingling in my hands and feet.

One thing I learned is that, for me at least, environmental factors exacerbate neuropathy and other chemo side effects. I react to mold, chlorine, diesel, gasoline, fragrances, incense, and other airborne contaminants.

I also think exercise helps (including fine motor activities like knitting, writing, and coloring). I also think Glutamine is helpful.

9/24/12 CT scan--No new mets. Everything stable.
3/11/13 CT Scan--two small new possible mets and odd looking area in left lung getting larger.
4/2/13--Biopsy of suspicious area in lower left lung. Mets to lung confirmed.
4/30/13 Begin Kadcyla/TDM-1
8/16/13 PET scan "mixed," with some areas of increased uptake, but also some definite improvement, so I'll stay on TDM-1/Kadcyla.
11/11/13 Finally get hormone receptor results from lung biopsy of 4/2/13. My cancer is no longer ER positive.
11/13/13 PET scan mixed results again. We're calling it "stable." Problems breathing on exertion.
2/18/14 PET scan shows a new lesion and newly active lymph node in chest, other progression. Bye bye TDM-1.
2/28/14 Begin Herceptin/Perjeta every 3 weeks.
6/8/14 PET "mixed," with no new lesions, and everything but lower lungs improving. My breathing is better.
8/18/14 PET "mixed" again. Upper lungs & one spine met stable, lower lungs less FDG avid, original tumor more avid, one lymph node in mediastinum more avid.
9/1/14 Begin taking Xeloda one week on, one week off. Will also stay on Herceptin and Perjeta every three weeks.
12/11/14 PET Scan--no new lesions, and everything looks better than it did.
3/20/15 PET Scan--no new lesions, but lower lung lesions larger and a bit more avid.
4/13/15 Increasing Xeloda dose to 10 days on, one week off.
7/1/15 Scan "mixed" again, but suggests continuing progression. Stop Xeloda. Substitute Abraxane every 3 weeks starting 7/13.
10/28/15 PET scan shows dramatic improvement everywhere. All lesions except lower lungs have resolved; lower lungs noticeably improved.
12/18/15 Last Abraxane. Continue on Herceptin and Perjeta alone beginning 1/8/16.
1/27/16 PET scan shows cancer is stable.
5/11/16 PET scan shows uptake in some areas that were resolved on the last two scans.
6/3/16 Begin Kadcyla and Tykerb combination
6/5 - 6/23 Horrible diarrhea from K&T together. Got pneumonia.
7/15/16 Begin Kadcyla only every 3 weeks.
9/6/16 Begin radiation therapy on right lung lesion that caused the pneumonia.
10/3/16 Last of 12 radiation treatments to right lung.
11/4/16 Huffing and puffing, low O2, high heart rate, on tiniest bit of exertion. Diagnosed as radiation pneumonitis. Treated with Prednisone.
11/11/16 PET scan shows significant improvement to radiated part of right lung BUT a bunch of new lung lesions, and the bone met is getting worse.
11/22/16 Begin Eribulin and Herceptin. H every 3 weeks. E two weeks on, one week off.
3/6/17 Scan shows progression in lungs. Bone met a little better.
3/23/17 Lung biopsy. Tumor sampled is ER-, PR+ (5%), HER2+++. Getting Herceptin and Perjeta as a maintenance treatment.

Amy, I bought some Glutamine to help with my neuropathy and diarrhea, however my oncologist told me not to take it. I’m going to follow up with her about it again, as most people on this board says it’s helps with the neuropathy. Since I will be getting Taxol for an indefinite amount of time, I need to do something about the neuropathy in my feet. Does it also help with foot/leg spasms?.

Kathy, I want to transfer from Perjeta/Herceptin/Taxol to T-DM1, however, the fellow at my clinic told me that if I transfer to T-DM1, and it doesn’t work, I can’t go back to Perjeta/Herceptin/Taxol. I’m surprised that you have been able to take Perjeta and Herceptin since taking T-DM1. I take it you live in the US? I’m wondering why you can take Perjeta and Herceptin after T-DM1, however, I can’t.

01/16 – Second mammogram and ultrasound.
01/16 – Meet surgeon and go for third mammogram, second ultrasound and biopsy. Surgeon confirms cancer in left breast and lymph nodes and sets surgery date.
01/16 – Chest scan and bone scan done– all looks good.
02/16 – Surgery - left breast mastectomy and 16 lymph nodes removed (8 had cancer).
02/16 – CT scan done – small nodules on lung but Doctor advises it’s post surgical. They will continue to monitor just in case.
03/16 – Meet radiation oncologist and find out results of Pathology Report. I’m told that I have locally advanced breast cancer, based on the size of my tumour (7 cm!) and the fact that they found cancer cells in eight lymph nodes. I’m also told that I’m HER 2 positive, with high levels of estrogen and progesterone and that my cancer is stage 3, grade 2.
03/16 – Meet oncologist and am told that my cancer is actually grade 3, and that I should have done chemo before surgery. Too late now!
03/16 – Start first of six doses of chemo (Carboplatin and Docetaxal) and Herceptin (for 18 months).
04/16 – Have port put in.
04/16 – Get second dose of chemo, but Docetaxal is left out due to liver enzymes being high. I was unable to get a full dose of Docetaxal after my first treatment.
06/16 – Finished chemo! One month off and then I start radiation.
06/16 – Start Tamoxifen.
07/16 – First radiation treatment – 24 more to go!
08/16 – Went for Genetic Testing to see if I have the BRCA gene. Tested negative for BRCA I and II
08/16 – Radiation oncologist biopsies “scar tissue” on my chest wall. I am told that I have a local recurrence and need to have rush surgery.
09/16 – Meet surgeon who advises that I need to meet with a plastic surgeon, as they will need to do a skin graft to close me up after surgery. Meet plastic surgeon and all looks good. A surgery date is set for October 4.
09/16 – Go for rush ultrasound, bone scan, breast MRI and CT scan.
09/16 – Meet oncologist who advises that the ultrasound and bone scan results look good, and that MRI shows three small masses at surgery site, but lymph nodes are clear. Still awaiting the results of the CT scan, but we are positive it will look good.
09/16 – Get a call from my oncologist, who advises that CT scan shows small spots on my lungs, and a large lymph node in the middle of my chest. This means the cancer has spread! She looks into getting me funded for TDM-1 and cancels my surgery.
10/16 – Meet oncologist, who advises that I have to take Perjeta before I can take TDM1. I start Perjeta/Herceptin every three weeks for an indefinite amount of time, and Taxol, which I will take two weeks in a row with one week off and then two weeks in a row for 8-16 treatments. Stop Tamoxifen.
10/16 – Meet surgeon, who reviews my CT scan and advises that the spots on my lungs may not be cancer, and that he doesn’t see a lymph node in my chest. He thinks it’s a spot on my lung. I’m feeling very confused! He advises that my oncologist doesn’t want me to have surgery to remove the three small masses on my scar line, as she wants to use them as a way to determine if the treatment is working. He advises that if they have not shrunk in 6 months, he will revisit surgery.
10/16 – CEA blood test to determine Tumour markers. Results were normal (2.7). My doctor advises that this could mean two things: (1) that the treatment is working, and the tumours are shrinking, or (2), that I'm one of those people who never get elevated CEA levels. Given that some people never get an elevated CEA level, this test doesn’t seem very accurate to me! Asked for PET scan, but am told I don’t qualify.
10/16 – Brain MRI – NED!
11/16 - CA-15-30 blood test – Tumour markers are normal at 19.
11/16 – Second CEA blood test – Tumours markers are still normal at 1.6 Second CA-15-30 blood test – Tumour markers are still normal at 19
11/16 – Develop lymphedema and have to wear a sleeve
12/16 – CT Scan shows that the tumors on my lungs and the lymph node in the middle of my chest are shrinking, and that some have resolved. Also, the small masses along my scar line are no longer visible. This means the medication is working!
12/16 – Small “pimple” shows up where old tumour on chest wall was located. Doctor is going to monitor it for now.
01/17 – A second “pimple” shows up on chest wall, as well as a small lump under the skin. My doctor thinks it’s scar tissue and will monitor it for now.
0/17 – Started to develop severe back pain – worried the cancer has spread to my spine.
03/17 – Third CEA blood test and CA-15-30 blood test – Both normal at 2.5 and 25
03/17 – CT Chest scan to see if there’s improvement to chest and lungs – results are mixed. The good news is, lymph nodes are stable, there was improvement of previous lung changes due to radiation, and the cancer has not spread to my bones, spine or liver, etc. The bad news is that while some lung nodules remained stable, others increased by 1-2 mm and new ones have shown up All of them were less than 1cm.
03/17 – Second brain MRI scheduled for end of month.
03/17 – Ultrasound scheduled to determine size of “pimples” on my chest wall. They will follow up with a second Ultrasound in three months, to see if they are growing

Tiffany, I am not sure why that policy is in place. Perjeta and Herceptin have been helpful to me.

I’m glad it’s been working for your Kathy. It’s not working for me as well as I would like it to anymore, and I would like to try T-DM1, however, like I said above, if I try it and it doesn’t work, I can’t go back. I guess I’ll just have to stay with Perjeta/Herceptin/Taxol for now.

I take B6 to prevent neuropathy.
Read about it on a site specifically for neuropathy. Sorry can't remember which site.
So far so good!

Hi Juls, the pharmacist at my clinic told me to stop taking all B vitamins when I started treatment again, as there was a suggestion that B vitamins could speed up cancer growth. I did some research on it myself, and there are some studies that suggest that B vitamins could cause cancer. You should mentione this to your oncologist and look into it yourself.

Hi Tiffany

Geez - never heard that before!

Oncologists know I take them and have never said I shouldn't - but they don't know much about vitamins!
Haven't taken any B vitamins since starting TDM-1, 3 weeks ago ( no other reason than I'd finished them). Maybe I should leave them out!
Will continue with Vit D though!

My pharmacist is the one who told me about this, not my oncologist. At my clinic, we are all assigned a pharmacist, social worker, and nutritionist, and if we need to speak any of them, they will come by during treatment. If you have a pharmacist, you may want to follow up with him/her. Based on what she told me, studies are only suggesting that B vitamins could speed up cancer, but they still don’t know for sure. That being said, why take the risk?

How were your side-effects from the T-DM1? I was talking to a women at my clinic yesterday who had four treatments of T-DM1. Not only did it not work for her, but she said her side-effects where horrible. I was so surprised to hear this, as I have heard nothing but good things about T-DM1, and I had heard that the side-effects were less severe than they are from chemo. It just goes to show how different we all are.

12/15 – First mammogram
01/16 – Second mammogram and ultrasound.
01/16 – Meet surgeon and go for third mammogram, second ultrasound and biopsy. Surgeon confirms cancer in left breast and lymph nodes and sets surgery date.
01/16 – Chest scan and bone scan done– all looks good.
02/16 – Surgery - left breast mastectomy and 16 lymph nodes removed (8 had cancer).
02/16 – CT scan done – small nodules on lung but Doctor advises it’s post surgical. They will continue to monitor just in case.
03/16 – Meet radiation oncologist and find out results of Pathology Report. I’m told that I have locally advanced breast cancer, based on the size of my tumour (7 cm!) and the fact that they found cancer cells in eight lymph nodes. I’m also told that I’m HER 2 positive, with high levels of estrogen and progesterone and that my cancer is stage 3, grade 2.
03/16 – Meet oncologist and am told that my cancer is actually grade 3, and that I should have done chemo before surgery. Too late now!
03/16 – Start first of six doses of chemo (Carboplatin and Docetaxal) and Herceptin (for 18 months).
04/16 – Have port put in.
04/16 – Get second dose of chemo, but Docetaxal is left out due to liver enzymes being high. I was unable to get a full dose of Docetaxal after my first treatment.
06/16 – Finished chemo! One month off and then I start radiation.
06/16 – Start Tamoxifen.
07/16 – First radiation treatment – 24 more to go!
08/16 – Went for Genetic Testing to see if I have the BRCA gene. Tested negative for BRCA I and II
08/16 – Radiation oncologist biopsies “scar tissue” on my chest wall. I am told that I have a local recurrence and need to have rush surgery.
09/16 – Meet surgeon who advises that I need to meet with a plastic surgeon, as they will need to do a skin graft to close me up after surgery. Meet plastic surgeon and all looks good. A surgery date is set for October 4.
09/16 – Go for rush ultrasound, bone scan, breast MRI and CT scan.
09/16 – Meet oncologist who advises that the ultrasound and bone scan results look good, and that MRI shows three small masses at surgery site, but lymph nodes are clear. Still awaiting the results of the CT scan, but we are positive it will look good.
09/16 – Get a call from my oncologist, who advises that CT scan shows small spots on my lungs, and a large lymph node in the middle of my chest. This means the cancer has spread! She looks into getting me funded for TDM-1 and cancels my surgery.
10/16 – Meet oncologist, who advises that I have to take Perjeta before I can take TDM1. I start Perjeta/Herceptin every three weeks for an indefinite amount of time, and Taxol, which I will take two weeks in a row with one week off and then two weeks in a row for 8-16 treatments. Stop Tamoxifen.
10/16 – Meet surgeon, who reviews my CT scan and advises that the spots on my lungs may not be cancer, and that he doesn’t see a lymph node in my chest. He thinks it’s a spot on my lung. I’m feeling very confused! He advises that my oncologist doesn’t want me to have surgery to remove the three small masses on my scar line, as she wants to use them as a way to determine if the treatment is working. He advises that if they have not shrunk in 6 months, he will revisit surgery.
10/16 – CEA blood test to determine Tumour markers. Results were normal (2.7). My doctor advises that this could mean two things: (1) that the treatment is working, and the tumours are shrinking, or (2), that I'm one of those people who never get elevated CEA levels. Given that some people never get an elevated CEA level, this test doesn’t seem very accurate to me! Asked for PET scan, but am told I don’t qualify.
10/16 – Brain MRI – NED!
11/16 - CA-15-30 blood test – Tumour markers are normal at 19.
11/16 – Second CEA blood test – Tumours markers are still normal at 1.6 Second CA-15-30 blood test – Tumour markers are still normal at 19
11/16 – Develop lymphedema and have to wear a sleeve
12/16 – CT Scan shows that the tumors on my lungs and the lymph node in the middle of my chest are shrinking, and that some have resolved. Also, the small masses along my scar line are no longer visible. This means the medication is working!
12/16 – Small “pimple” shows up where old tumour on chest wall was located. Doctor is going to monitor it for now.
01/17 – A second “pimple” shows up on chest wall, as well as a small lump under the skin. My doctor thinks it’s scar tissue and will monitor it for now.
0/17 – Started to develop severe back pain – worried the cancer has spread to my spine.
03/17 – Third CEA blood test and CA-15-30 blood test – Both normal at 2.5 and 25
03/17 – CT Chest scan to see if there’s improvement to chest and lungs – results are mixed. The good news is, lymph nodes are stable, there was improvement of previous lung changes due to radiation, and the cancer has not spread to my bones, spine or liver, etc. The bad news is that while some lung nodules remained stable, others increased by 1-2 mm and new ones have shown up. All of them were less than 1cm.
03/17 – Second brain MRI –awaiting results
03/17 – Ultrasound scheduled to determine size of “pimples” on my chest wall. They will follow up with a second Ultrasound in three months, to see if they are growing
05/17 – Full Body CT Scan Scheduled to see if I’m still stable

Don't have a Pharmacist as such. I could ask a local Pharmacist but doubt if they would be much help. I'll need to investigate this!!

Re TDM-1

Get 2nd treatment tomorrow so still early days. So far have felt a bit tired and last few days a tiny bit low which is unlike me! Also some muscle stiffness. Maybe I am wrong but 4 treatments seems quite a short time to judge results on? I had echo on Monday (all ok) & next CT due on 11th April (after 2 cycles). Hoping its working because I have no idea what else is available here!

It’s too bad you don’t have a pharmacist assigned to you at your clinic. I met my during my first treatment, and she went over all of the chemos/drugs I was taking, and asked me about all vitamins I was taking and told me what foods/vitamins I shouldn’t be taking while on chemo, etc. Since then, she’s come by to see me a few times to make sure everything was okay, and to ask if I had any questions regarding the drugs I was taking. I just assumed this was standard care everywhere, but I guess not.

I’m glad your side-effects are not that bad. The women I spoke to told me there her tumour grew, even after getting four treatments with T-DM1, so the doctor told her it wasn’t working for her and put her in a trial. I guess if it has remained stable, they would have kept her on TDM-1, but if it’s growing, then I guess they think it’s not working. What’s odd is that my lung nodules are growing, even though I’m on Perjeta, and yet my doctor said that it’s still working as it’s slowing the growth. I guess there are different standards to determine whether a medication is working or not.

I hope your CT scan results comes back positive! Keep us posted.

Hi Tiffany
Hoping it works as liver met grew 1.2 cm in a few weeks when I was on no treatment. This concerned me but Oncologist said "if it grows, it grows." Not really what you want to hear!
My Nurse commented, re previous treatment that I had just failed on - "well Xeloda must have been doing something!"
It was a real concern to be taken of all treatment just when cancer progressing at what seemed to me to be at a quick rate.

I think you are right about the different standards!

Lorraine's foot neuropathy got worse on t dm-1. her onc says it's permanent . she had it before t dm-1 from many chemos - taxanes, xeloda. cramping and a " weird " feeling in the ball of her feet. and she has been off over 2 yrs. she's had some falls that could be related. no finger neuropathy .
i would try t dm-1 , not wait. if you are straight her2. for the chance for a super response like lorraine's . perjeta track record is clear - for a govt to block it is an outrage

Hi Phil, I’m glad T-DM1 worked so well for Lorraine, and I hope it works just as well for Juls. I’m still getting Perjeta/Herceptin/Taxol, however, if my next scan shows progression, which is in May, I’ll be changing to T-DM1. My doctor wants me to stay on Perjeta for as long as possible though, as, once I change drugs I can’t go back. I was hoping to get at least a year out of Perjeta, but, it doesn’t look like that will be the case.

03/17 – Received my genetic test results from Foundation One. It found 11 genomic alterations, 3 therapies associated with potential clinical benefit, and 13 clinical trials from which I might benefit.
