Can someone explain what metastic disease means?

Hi. I am so confused right now. I called my doctor to find out more information about the Neratinib Trial. One of the nurses called me back because she said that I didn't qualify because I had metastic disease. I explained that I was confused because I thought I was at stage IIIA. I had 7/12 nodes involved but it hadn't spread to any other organs. The nurse said that because I had it in my lymph nodes, it probably was in my blood stream and then considered it metastic. I finished Herceptin in February of 2007.

Can someone help me?

I suppose what needs to be determined is what the difference between metastatic disease and Stage 4 is. For example, like you, I was also node positive and nodes are not breast so... theorectically any woman with node positive disease or even just angio/lymphaic invasion has metastatic disease - cancer in nodes or vessels. However, I have always considered metastatic disease as cancer in other organs but then, the lymph system and nodes are different organs than breast.

I suppose you should call back and ask for this clarification - are they looking only for Stage 1 women without or without angiolymphatic invasion (or those Stage 1s with no invasion at all perhaps?!??)

If it is Stage 1 women, then they don't want anyone who had nodes involved and thus, they are considering positive nodes as "metastatic" (which strictly defined is true but not when the rest of the world defines metastatic (aka "mets") as distant organ involvement.

A NURSE said that? That's wrong, and she should know better. It's pretty universally accepted that "early" breast cancer is anything short of stage IV. Metastatic breast cancer, advanced breast cancer, and stage IV breast cancer are the same thing, and they do not describe stage IIIA.

Any breast cancer can have spread through the blood stream (including stage I), which is why they are looking for better ways to evaluate individual breast cancers. But unless those cells take up residence somewhere else (typically bones, liver, lungs), there's no way to know if they're there or not, and you don't say that the breast cancer is metastatic just because you think it "might" eventually be that. Jeeze! I hope that you talk to the onc and that he straightens out his nurse. If she didn't know for sure, she would have been much better off to say nothing.

Now, it is possible that the neratinib trial has some specifications that exclude you (like maybe they only want stage I or II). But that still doesn't mean your breast cancer is metastatic.

There is a gray area sometimes, in the language, because pathology reports will describe lymph node involvement as "metastatic to lymph node" (or something like that, using the mets word) but that is not the same as distant (stage IV) metastatic disease. I'm in a rush and hoping this makes good enough sense. I didn't want you to be fretting one second more than necessary.

3/01 ~ Age 49, occult primary announced by large axillary node found by my husband. Multiple CBE's, mammogram, U/S could not find anything in the breast. Axillary node biopsy - pathology said + for "mets above diaphragm, probably breast".
4/01 ~ Bilateral mastectomies (LMRM, R simple) - 1.2cm IDC was found at pathology.
5 of 11 axillary nodes positive, largest = 6cm. Stage IIIA
ERPR 5%/1% (re-done later at Baylor, both negative at zero).
HER2neu positive by IHC and FISH (8.89).
Lymphovascular invasion, grade 3, 8/9 modified SBR.
TX: Control of arm of NSABP B-31's adjuvant Herceptin trial (no Herceptin): A/C x 4 and Taxol x 4 q3weeks, then rads. Arimidex for two years, stopped after second patholgy opinion.

There are stage II and III women posting there who are getting into this trial. That nurse sounds more like an aide who does not have much education. Was that person actually your onc's NURSE or someone else in the office??

MY STORY SO FAR ~~~~
Found suspicious lump 9/2000
Lumpectomy, then node dissection and port placement
Stage IIB, 8 pos nodes of 18, Grade 3, ER & PR -
Adriamycin 12 weekly, taxotere 4 rounds
36 rads - very little burning
3 mos after rads liver full of tumors, Stage IV Jan 2002, one spot on sternum
Weekly Taxol, Navelbine, Herceptin for 27 rounds to NED!
2003 & 2004 no active disease - 3 weekly Herceptin + Zometa
Jan 2005 two mets to brain - Gamma Knife on Jan 18
All clear until treated cerebellum spot showing activity on Jan 2006 brain MRI & brain PET
Brain surgery on Feb 9, 2006 - no cancer, 100% radiation necrosis - tumor was still dying
Continue as NED while on Herceptin & quarterly Zometa
Fall-2006 - off Zometa - watching one small brain spot (scar?)
2007 - spot/scar in brain stable - finished anticoagulation therapy for clot along my port-a-catheter - 3 angioplasties to unblock vena cava
2008 - Brain and body still NED! Port removed and scans in Dec.
Dec 2008 - stop Herceptin - Vaccine Trial at U of W begun in Oct. of 2011
STILL NED everywhere in Feb 2014 - on wing & prayer
7/14 - Started twice yearly Zometa for my bones
Jan. 2015 checkup still shows NED
2015 Neuropathy in feet - otherwise all OK - still NED.
Same news for 2016 and all of 2017.
Nov of 2017 - had small skin cancer removed from my face. Will have Zometa end of Jan. 2018.

Thank you both so much ... I am not sure why I felt like I was kicked in the gut and panicked. I am three years and 11 months out and I still get freaked by what people say.

After a barrage of questions the nurse quickly told me that she had to call me back!

I agree with Debbie L: You do not have metastatic disease if you are stage III and being 3 yrs 11 months out is a good sign. Stupid nurse! How could she say that? She ought to be flogged!

It's always a gut wrencher when we hear description (rightly or wrongly stated) outside what we hang on to and the vernacular we choose to use. It happened to me after my 2nd recurrence/relapse/metastatic progression, etc... which were the words I was using. Then I spoke to a person qualifying patients for a research interview (not a trial, just an interview) who said that they had acquired all of their stage IV patients, but she would keep me in mind. I almost had a heart attack... I had never been described as stage IV up until then, although, indeed, I was. And this was 3.5 years ago. It took me a while to wrap my head around it, but then it dawned on me that there are at least 50 stages in my world...

I understand the emotional sensitivity all too well Lisa. And that nurse (if indeed it was a 'nurse') needs someone to unscrew her head and screw it back on straight.

MRI shows activity in brain mets, completed 5 fractions/5 consecutive days of IMRT to zap the pesky buggers

dropped Xeloda, switched to tri-weekly Herceptin in combo with Tykerb, extend to tri-monthly Zometa infusion
Dec'08~
Brain MRI- 4 spots reduced to punctate size, large spot shrunk by 3mm, CT of torso clear/pelvis spot stable
June'09~
new 3-4mm left cerrebellar spot zapped with IMRT targeted rads
Sept'09~
new 6mm & 1 cm spots in pituitary/optic chiasm area. Rx= 25 days of 3D conformal fractionated targeted IMRT to the tumors.
Oct'09~
25 days of low dose 3D conformal fractionated targeted IMRT to the bone mets spot on rt. iliac crest that have been watching for 2 years. Added daily Aromasin back into treatment regimen.
Apr'10~ Brain MRI clear! But, see new small spot on adrenal gland. Change from Aromasin back to Tamoxifen.
June'10~ Tumor markers (CA15.3) dropped from 37 to 23 after one month on Tamoxifen. Continue to monitor adrenal gland spot. Remain on Tykerb/Herceptin/Tamoxifen.
Nov'10~ Radiate positive mediastinal node that was pressing on recurrent laryngeal nerve, causing paralyzed larynx and a funny voice.
Jan'11~ MRI shows possible activity or perhaps just scar tissue/necrotic increase on 3 previously treated brain spots and a pituitary spot. 5 days of IMRT on 4 spots.
Feb'11~ Enrolled in T-DM1 EAP in Denver, first treatment March 25, 2011.
Mar'11~ Finally started T-DM1 EAP in Denver at Rocky Mountain Cancer Center/Rose on Mar. 25... hallelujah.

Thank you again everyone! I actually got a call from a different nurse apologizing for Dr. Tellez and the nurse practioner I had talked to. She told me the nurse misunderstood the doctors message and was not familiar with my case. Unfortunately I do not qualify for the trial!

Where are you in Chicago? I can tell you that in the past few weeks on my journey with the insurance issues and clinical trials, it depends on who you talk to, the answer you get...does your hospital/center have a clinical trials specialist, or better yet, call and talk to someone where this trial is being conducted. It can make a huge difference.

Lisa,

It looks like you qualify for the early stage Neratinib trial, as the trial inclusion criteria says stages I through IIIC. But there may be other reasons why your doctor doesn't think you qualify. Here's the link the the trial information. You can also contact them directly.

Diagnosed stage 2b in July 2003 (2.3 cm, HER2+, ER-/PR-, 7+ nodes). Treated with mastectomy (with immediate DIEP flap reconstruction), AC + T/Herceptin (off label). Cancer advanced to lung in Jan. 2007 (1 cm nodule). Started Herceptin every 3 weeks. Lung wedge resection April 2007. Cancer recurred in lung April 2008. RFA of lung in August 2008. 2nd annual brain MRI in Oct. 2008 discovered 2.6 cm cystic tumor in left frontal lobe. Craniotomy Oct. 2008 (ER-/PR-/HER2-) followed by targeted radiation (IMRT). Coughing up blood Feb. 2009. Thoractomy July 2009 to cut out fungal ball of common soil fungus (aspergillus) that grew in the RFA cavity (most likely inhaled while gardening). No cancer, only fungus. Removal of tiny melanoma from upper left arm, plus sentinel lymph node biopsy in Feb. 2016. Guardant Health liquid biopsy in Feb. 2016 showed mutations in 4 subtypes of TP53. Repeat of Guardant Health biopsy in Jana. 2021 showed 3 TP53 mutations, BRCA1 mutation and CHEK2 mutation. Invitae genetic testing showed negative for all of these. Living with MBC since 2007. Stopped Herceptin Hylecta (injection) treatment in March 2020. Recent 2021 annual CT of chest, abdomen and pelvis and annual brain MRI showed NED. Praying for NED forever!!

Staged lll (A, B, or C) is called locally advanced breast cancer. It is not the same as metastatic, or stage lV breast cancer.

There are others on this board who are stage lll who have already qualified for this study. If you want to be a part of it I would investigate carefully., and perhaps get a 2nd opinion from an oncologist at a research center who is actively involved with these studies.
