I have a plan!
...and it does NOT include cancer.
Waiting for a treatment plan was nearly more torturous for me than waiting for a diagnosis.
I got my plan!
Tomorrow PET, MRI, MUGA. Monday, Chemo class. Wed consult with BS to get Port. Friday, consult with MO to go over diagnostics. Tues Port in. Friday, CHEMO! Taxere, Carboplatin and Herceptin infusion (6 hours) q21d x6 txs. Start radiation 33 tx daily M-F for six weeks concurrent with additional Herceptin infusion (2h) q21d for 5 additional txs. Quarterly MUGA, PET, MRI scans. Because it cannot be determined if the DCIS I have is a separate cancer event (it is focally positive <1% for ER) or if it's what my IDC looked like before it went nuts and eradicated ER (highly unlikely but not implausible), I will take oral Tamoxifen for five years to protect my other breast. I told my MO that I didn't necessarily want my other breast and she seemed quite upset with me.... Hey, I don't lactate anymore, what to I need it for?

1/2/14 Happy New Year, you have a Goiter? Muga down to 59%.
1/17/14 Hashimoto's Dz Dx'd. Now maybe I'll feel BETTER!
5/2/14 Herceptin completed! New kitten!
8/19/14 Prophylactic mastectomy (right) and PORT OUT! I'm DONE and now I really am a SURVIVOR.
2/15 Started not feeling so swell. Memory lapses and GI issues with nausea and blurry vision.
4/30/15 U/S cystic gallbladder, cyst on right ovary and mass in my uterus. GYN consult scheduled---and cancelled. I'm not ready.
5/4/15 Brain MRI clear (big sigh of relief)
7/30/15 Back Pain
8/31/15 Radiograph: compression fracture L2
9/10/15 Bone Scan positive
9/21/15 CT scan conclusive for tumor
10/1/15 CT guided biopsy & Brain to Pelvis MRI reveal additional lesions on spine C6, T10, T11 and L2 is collapsing.
10/8/15 Abbreviated pathology: new tumor(s) poorly differentiated carcinoma consistent with known breast primary.
ER-/PR+ (40%) HER2/neu+++ Ki-67 4% Pancytokeratin AE1/3 Strong Positivity in all malignant cells.
10/13/15 Abnormal Dexa: moderate risk of fracture to both femoral head/neck R&L. Significant risk to lumbar spine.
10/14/15 Radiation consult back to the cooker.
10/20/15 MUGA 50% down from 54% after a year off Herceptin (???)
10/21/15 Kyphoplasty L2
10/22/15 Re-start Chemo: Perjeta, Herceptin & Taxotere
10/26/15 PET Scan confirms C6, T10-11, L2, new lesion noted at L4 but no visceral involvement---Happy dance!!!

11/10/15 Installed my little purple power port and not a moment too soon, took them four tries to get an IV started today.
Yes, we really are going down this road again.
12/5/15 CT for suspected pulmonary embolism demonstrates increase in T10-11 mets.
12/8/15 Bone Scan uptake at T10-11 (not seen 9/17/15) & Right 8th Rib (not evident on PET 10/26/15)
12/10/15 Consult Re: PROGRESSION. Halt THP due today. Schedule PET and order TDM1 for next week. PLAN B.
12/14/15 PET scan: NO PROGRESSION! THP is working, metabolic activity minimal. Merry Christmas to me! Sticking with PLAN A, it's working.
1/7/16 Start Taxol weekly instead of Taxotere (has been too taxing and not rebounding between txs.) Zometa instead of Xgeva.
3/28/16 CT shows new sclerotic lesions on T12, L3, L5, L6, right ilium and head of right femur. No uptake on Bone Scan (progression????)
3/31/16 Discontinue Taxol start Arimidex, still getting H&P.
6/2/16 Discontinue Arimidex and start Exemestane.
6/18/16 PET is NEAD!!
7/1/16 Discontinue Exemestane and restart Armidex (SEs)
8/29/16 CT/Bone Scan Stable (still uptake at T10-11)
10/3/16 BSO pathology negative
10/10/16 MRI: Brain clear!
10/14/16 Switched care to Harold Alfond Center for Cancer Care

12/12/16 CT/Bone scan Mostly stable significant uptake at L2 plan to PET
1/12/17 PET shows NEAD celebrate with a new puppy!
3/29/17 CT & BS = NEAD
7/31/17 Aetna denies access to H&P <gearing up for a fight>
8/4/17 CT& BS= STABLE
8/9/17 No treatment, Aetna still denying H&P
8/14/17 Aetna appeal approved H&P through February 2018!

Sorry to say welcome, but you have come to a good place, and after all these years I would have went with a bilateral as well. I was never very big on the bra thing so If I knew then what I know know, both would have gone, no mammo, ultrasounds. Either flat and even for everyday, or dress and tight shirt 2 even light weight forms.
If you do not get what you need then second opinion is always good. Be your own advocate, do have a good family/friend support, to bounce that idea off of?

10/13 Ct has shown Double left ureters with stones/cysts in them, after 3 births and lots and U/S iit takes cancer to figure out you have 2 smaller ureters going into 1 kidney!
12/13 Mri brain no new lesions, cervical and lumbar arthritis.
Tspine lesion at T3 stable, T 7-9 GROWTH lots of pain
1/29/14 HIHO HIHO its off to Neuro surgery I go

Sounds like a good plan! And YOU sound like you have exactly the right attitude to make it all work! Kudos!
Ironically...since I don't think I've ever disagreed with Darita (whose new avatar photo is stunning BTW) on anything...
As someone who did have a BMX, and recon...
If I had known then, what I know now...I probably would not have done it that way.
But...as we all know, hindsight is 20/20!
You sound like someone who get's all her ducks in a row...research, second opinions, etc ...before making decisions so I'm confident you'll do what's right for you.

Happy that you have a plan. Sounds like you are set up for "Standard of Care" treatment except for the Tamoxifen seems a little outside the box. It does seem the MO is very thorough which is a good thing.
I understand about the other breast. I'm currently not very found of mine because I have the occasional breast pain that makes me nervous. Even though I'm over 5 years out I'm trying to lose weight and then get the other breast reduced and lifted and finish my reconstruction.
I'm a big fan of 2nd opinions too. I know you are way up in beautiful Maine and cancer centers are not common. But since your MO is more attached to your other breast than you are, a different opinion might be helpful.
You can also do small things to lower risk on your own. Eat plenty of fruits & vegetables, aerobic exercise (even during treatment) and meditation have shown a decrease in recurrence rates. Women who have a support group have also had lower recurrence rates.
Statistically the chances of the cancer recurring are low. But we see from this board the ladies who have experienced progression. Luckily they (Denise, Darita, and others) are here and willing to share their experience.
The radiation and chemo are going to be challenging. But you can do it. If you have questions there seems to be someone on the board who has experienced something similar.

What about making a change based on the info Lani just posted regarding weekly versus every three weeks having a better response rate for ER+, Her+?

Hi MR!
I'm a member of the "ittibittititticommitti!!" Size A on a good day!! 
You can read my signature, as to my right breast...I am still recovering from the infection after my mastectomy. I really wanted to be DONE with both breasts, however, my BS told me that DCIS would not go to my other breast, therefore he would not remove it.
I was also tested before surgery for the BRCA 1 & 2 as I'm adopted, thank God I am negative.
In fact, I saw my BS today, we did talk about possible reconstruction again, maybe in a year...he knows I am so very, very scared to go under the knife again as I don't have much skin left to mess with! He told me, if or when I'm ready, we'll talk about it and he will advise me with a very reputable plastic surgeon...I still am a long way off...
So many questions for all of us...seems like you are making ground...keep at it...great job!!!

Hi;
I'd definitely ask about weekly treatment versus the 21day cycle. As you can see from my profile I did the weekly treatment and was able to work the whole time and did not even lose all my hair. I also live on Maine part of the year and take treatment up in Augusta.....what great weather the last two days.

May 2008 new lesion in liver 15mm Tykerb/Xeloda/Herceptin
July 2008 stable...yeah!
Sept 2008 NED again !!!
Jan 2009 fell off the wagon again spot back in the liver and fell out of menopause.
Feb 2009 RFA and 2nd liver resection to remove spot ---back on the NED wagon again continue Tykerb, Herceptin.
March 2009- oophrectomy added Femara and bi-annual Zometa
May 2009- scans clear but suspect lung nodule
June 2009- Lung VAT wedge resection to remove nodule---fungus ball not cancer!! phew
Aug 2009- NED
Nov 2009-scans again clear YAHOO!
March 2010- scans clear continue Tykerb, Herceptin, Femara, Zometa Q6mo
Nov 2009-Nov 2019 scans clear done every 6 months
Feb 2020 - Fell out of the NED wagon hard! single liver lesions and large infect cyst. 3 weeks in ICU.
March 2021- 5 cycles perjeta, herceptin, navelbine. lesion stable.
June 2021 - 3rd liver resection to remove single liver lesion. Continued perjeta, herceptin.
Nov. 2021- PET scan show 5 hot nodes near liver. 9 cycles Kadcyla
June 2021- PET scan shows progression. nodes size unchanged but even more SUV uptake.
July 2021- start ENHERTU

I wish I had done weekly treatment. Maine, try to have the test to see if you can metabolize Tamoxifen.
