Computer-aided breast cancer detection pinpoints even the smallest tumors

NEW YORK (Reuters Health) - A computer-aided detection (CAD) system can help radiologists spot small breast tumors that may otherwise be missed, results of a study show. The system has a sensitivity of 92% for lesions 5 mm in size or smaller.

"The mammographic detection of small breast cancers is most challenging," Dr. Rachel F. Brem, director of breast imaging at George Washington University in Washington, DC, and colleagues note in the March issue of the American Journal of Roentgenology.

They assessed the ability of a CAD system to detect biopsy-proven breast cancers, particularly tumors measuring 1 cm or less.

The study population included 201 women with a total of 122 masses, 54 microcalcifications, and 25 mixed mass-and-microcalcification lesions.

"The CAD system correctly marked most biopsy-proven breast cancers, with a greater sensitivity for microcalcification than for mass lesions but with no significant difference in performance based on cancer size," the team reports.

The CAD system had an overall sensitivity of 89% for the detection of breast cancer. It detected 84% of the masses, 98% of the microcalcifications, and 92% of the mixed masses-and-microcalcification lesions.

The CAD system was "highly effective" in detecting even the smallest lesions, with a sensitivity of 92% for lesions 5 mm or less, 94% for lesions measuring 11 to 15 mm, and 80% for detecting cancers measuring 16 to 20 mm.

There were 1.3 false-positive marks per image, a "reasonable number that should not affect the radiologists interpretation, nor should it mean that more women would be called back for additional mammograms," Dr. Brem said in a statement.

The CAD system is a "useful tool for the improved detection of breast cancer, regardless of the size of the cancer," she and colleagues conclude in their report.
