petscan good news bad news

I am super depressed, and honestly, I avoid posting because I am still grieving the loss of Sheila, Brenda, Chrisy and others. And now the news of Christine. Nothing feels very steady right now, including my own health. Seeing the clinical trial data from last year also just brought up all the sad emotions of friends I've lost to this stupid disease. It's all a bit much for me to handle, I'm feeling very overwhelmed and unsure of what to do next.

I need to meet with my doc to discuss the results of my latest PETscan. Good news/bad news scan results - Good news: the lung mets are gone or in radiologist speak: "non visualization of previous" and left adrenal gland SUV is 1.8 previously was at 7 and improvement noted. I'm guessing that the add in of Perjeta made the difference. BAD news is that the sclerotic disease at L1 is considerably more metabolic by 50% and SUV is 12 from 9.5. Sternum is highly active, but SUV actually decreased, so I don't understand why it was noted as more active - always leaves me wondering if the whole report is made up of connected mistakes.

Was thinking of going back to xgeva and in 12 weeks repeating the scan or doing a bone scan to see what's what. OR, I can try what's behind door #2 - Kad-psycho-cyla. I hesitate because chemo is involved and the s/e's will be worse than what I have now, don't know if TDM1 will work on all the bones stuff and don't know if I even want to continue...and what would I do if it didn't work - how could I go back on what I'm doing now - who will pay? So that's it for now - it's what has me very down and tired.

1996 cancer WTF?! 1.3 cm lumpectomy Er/Pr neg. Her2+ (20nodes NEGATIVE) did CMF + rads. NED.
2002 recurrence. Bilateral mastectomy w/TFL autologous recon. Then ACx2. Skin lymphatic rash. Taxotere w/Herceptin x4. Herceptin/Xeloda. Finally stops spreading.
2003 - Back to surgery, remove skin mets, and will have surgery one week later when pathology can confirm margins.
‘03 latisimus dorsi flap to remove skin mets. CLEAN MARGINS. Continue single agent Herceptin thru 4/04. NED.
‘04 '05 & 06 tiny recurrences - scar line. surgery to cut out. NED each time.
1/2006 Rads again, to scar line. NED.

3/07 Heartbreaking news - mets! lungs.sternum. Try Tykerb/Xeloda. Tykerb/Carbo/Gemzar. Switch Oncs.
12/07 Herceptin.Tykerb. Markers go stable.
2/8/08 gamma knife 13mm stupid brain met.
3/08 Herceptin/tykerb/avastin/zometa.
3/09 brain NED. Lungs STABLE.
4/09 attack sternum (10 daysPHOTONS.5 days ELECTRONS)
9/09 MARKERS normal!
3/10 PET/CT=manubrium intensely metabolically active but stable. NEDhead.
Wash out 5/10 for tdm1 but 6/10 CT STABLE, PET improving. Markers normal. Brain NED. Resume just Herceptin plus ZOMETA
Dec 2010 Brain NED, lungs/sternum stable. markers normal.
MAR 2011 stop Herceptin/allergy! Go back on Tykerb and switch to Xgeva.
May-Aug 2011 Tykerb Herceptin Xgeva.
Sept 2011 Tykerb, Herceptin, Zometa, Avastin.
April 2012 sketchy drug trial in NYC. 6 weeks later I’m NED!
OCT 2012 PET/CT shows a bunch of freakin’ progression. Back to LA and Herceptin.avastin.zometa.
12/20/12 add in PERJETA!
March 2013 – 5 YEARS POST continue HAPZ
APRIL 2013 - 6 yrs stage 4. "FAILED" PETscan on 4/2/13
May 2013: rePetted - improvement in lungs, left adrenal stable, right 6th rib inactive, (must be PERJETA avastin) sternum and L1 fruckin'worsen. Drop zometa. ADD Xgeva. Doc says get rads consultant for L1 and possible biopsy of L1. I say, no thanks, doc. Lets see what xgeva brings to the table first. It's summer.
June-August 2013HAPX Herceptin Avastin Perjeta xgeva.
Sept - now - on chemo hold for calming tummy we hope. Markers stable for 2 months.
Nov 2013 - Herceptin-Perjeta-Avastin-Xgeva (collageneous colitis, which explains tummy probs, added Entocort)
December '13 BRAIN MRI ned in da head.
Jan 2014: CONTINUING on HAPX…
FEB 2014 PetCT clinical “impression”: 1. newbie nodule - SUV 1.5 right apical nodule, mildly hypermetabolic “suggestive” of worsening neoplastic lesion. 2. moderate worsening of the sternum – SUV 5.6 from 3.8
3. increasing sclerosis & decreasing activity of L1 met “suggests” mild healing. (SUV 9.4 v 12.1 in May ‘13)
4. scattered lung nodules, up to 5mm in size = stable, no increased activity
5. other small scattered sclerotic lesions, one in right iliac and one in thoracic vertebral body similar in appearance to L1 without PET activity and not clearly pathologic
APRIL 2014 - 6 YRS POST GAMMA ZAP, 7 YRS MBC & 18 YEARS FROM ORIGINAL DX!
October 2014: hold avastin, continue HPX
Feb 2015 Cancer you lost. NEDHEAD 7 years post gamma zap miracle, 8 years ST4, +19 yrs original diagnosis.
Continue HPX. Adding back Avastin
Nov 2015 pet/ct is mixed result. L1 SUV is worse. Continue Herceptin/avastin/xgeva. Might revisit Perjeta for L1. Meantime going for rads consult for L1
December 2015 - brain stable. Continue Herceptin, Perjeta, Avastin and xgeva.
Jan 2016: 5 days, 20 grays, Rads to L1 and continue on HAPX. I’m trying to "save" TDM1 for next line. Hope the rads work to quiet L1. Sciatic pain extraordinaire :((
Markers drop post rads.
2/24/16 HAP plus X - markers are down
SCIATIC PAIN DEAL BREAKER.
3/23/16 Laminectomy w/coflex implant L4/5. NO MORE SCIATIC PAIN!!! Healing.

MAY 2019 - progression back on herceptin add xeloda
JUNE 2019 - "6 mos average survival" LMD & CNS new single brain met - one zap during 5 days true beam SBRT to cord met
10/30/19 - stable brain and cord. progression lungs and bones. washing out. applying for ds8201a w nivolumab. hope they take me.
12/27/19 - begin ds8401a w nivolumab. after 2nd cycle nodes melt away. after 3rd cycle chest scan shows Improvement, brain MRI shows improvement, resolved areas & nothing new. switch to plain ENHERTU. after 4th cycle, PETscan shows mostly resolved or improved results. Markers near normal. I'm stunned but grateful.
10/26/20 - June 2021 Tucatinib/xeloda/herceptin - stable ish.

I'm so sorry to hear this. I share your bewilderment about mixed scan results. And I, too, grieve for lost friends and group members.

The "chess game" of mapping out a treatment strategy is wearisome, isn't it?

I hope it does you some good to know you've been heard here on the board. You're not alone.

4/30/13 Begin Kadcyla/TDM-1
8/16/13 PET scan "mixed," with some areas of increased uptake, but also some definite improvement, so I'll stay on TDM-1/Kadcyla.
11/11/13 Finally get hormone receptor results from lung biopsy of 4/2/13. My cancer is no longer ER positive.
11/13/13 PET scan mixed results again. We're calling it "stable." Problems breathing on exertion.
2/18/14 PET scan shows a new lesion and newly active lymph node in chest, other progression. Bye bye TDM-1.
2/28/14 Begin Herceptin/Perjeta every 3 weeks.
6/8/14 PET "mixed," with no new lesions, and everything but lower lungs improving. My breathing is better.
8/18/14 PET "mixed" again. Upper lungs & one spine met stable, lower lungs less FDG avid, original tumor more avid, one lymph node in mediastinum more avid.
9/1/14 Begin taking Xeloda one week on, one week off. Will also stay on Herceptin and Perjeta every three weeks.
12/11/14 PET Scan--no new lesions, and everything looks better than it did.
3/20/15 PET Scan--no new lesions, but lower lung lesions larger and a bit more avid.
4/13/15 Increasing Xeloda dose to 10 days on, one week off.
7/1/15 Scan "mixed" again, but suggests continuing progression. Stop Xeloda. Substitute Abraxane every 3 weeks starting 7/13.
10/28/15 PET scan shows dramatic improvement everywhere. All lesions except lower lungs have resolved; lower lungs noticeably improved.
12/18/15 Last Abraxane. Continue on Herceptin and Perjeta alone beginning 1/8/16.
1/27/16 PET scan shows cancer is stable.
5/11/16 PET scan shows uptake in some areas that were resolved on the last two scans.
6/3/16 Begin Kadcyla and Tykerb combination
6/5 - 6/23 Horrible diarrhea from K&T together. Got pneumonia.
7/15/16 Begin Kadcyla only every 3 weeks.
9/6/16 Begin radiation therapy on right lung lesion that caused the pneumonia.
10/3/16 Last of 12 radiation treatments to right lung.
11/4/16 Huffing and puffing, low O2, high heart rate, on tiniest bit of exertion. Diagnosed as radiation pneumonitis. Treated with Prednisone.
11/11/16 PET scan shows significant improvement to radiated part of right lung BUT a bunch of new lung lesions, and the bone met is getting worse.
11/22/16 Begin Eribulin and Herceptin. H every 3 weeks. E two weeks on, one week off.
3/6/17 Scan shows progression in lungs. Bone met a little better.
3/23/17 Lung biopsy. Tumor sampled is ER-, PR+ (5%), HER2+++. Getting Herceptin and Perjeta as a maintenance treatment.
5/31/17 Port placement
6/1/17 Start Navelbine & Tykerb

SoCalGal, as Amy rightly points out, you are not alone with us all here. I'm sorry you are feeling as you do, but it is understandable. I hope things will be clearer once you see your doctor, and that you feel stronger.

dx Dec 2000 dcis 2.5cm clear sentinel node, ER/PR- Her-2+
lumpectomy, 6 cycles AC, 6 weeks rads
October 2007 three x 2.5cm lung mets. 8 months Taxol, started Herceptin and continue. Significant reduction in lung mets.
June 2011 3cm x 4cm liver tumour. Started Abraxane and continue with Herceptin.
November 2011. Finished with Abraxane, continue with just Herceptin. Liver tumour now reduced to 15mm x 12mm. Lung tumour now 10mm x 0.5mm
February 2012. Scans show everything stable, and brain scan clear.
July 2012. PET/CT scans show I'm in remission - no active cancer!
]Dec CT brain cllear, lungs stable, liver tumour has increased to 20mm. PET scans showed active liver met and active lung thinglet, and possible bone met.
Jan 2013 recommence Abraxane, continue with Herceptin.
June 2013 finish Cycle 6 Abraxane, continue with Herceptin. 30% reduction in liver tumour, everything stable.
December 2013. CA15-3 on rise.
February 2014. PET and CT scans show single liver tumour has increased to 35mm. No other activity.
March 2014. Planned for SBRT for liver met, but couldn't have treatment as tumour too close to bowel. Continue Herceptin.
April 2014. Surgeon advises that I am a good candidate for liver resection, so will have operation early May (after camping holiday). Tumour now 44mm x 29mm.
May 7, 2014. Two liver tumours surgically removed. Third of liver removed, and gall bladder. Am I NED?May 2014. Pathology of tumour shows it's now ER+ (95% staining).
June 2014. CA15-3 has decreased to 18 from a pre-surgery reading of 59!
June 2014. Started Femara, continue with Herceptin.
July 2014. Stop Femara due to severe Osteoporosis. Commence Tamoxifen, continue Herceptin. Waiting to hear if I can have Aclasta infusion.
August 2014. CA15-3 has decreased further to 12 - YAY!
October 2014. Aclasta infusion for Osteoporosis. November 2014, CA15-3 decreased to 11. Scans of liver all clear, something new showing up on lung, but just watching at the moment.
November 2015. Started SBRT on solitary lung met.
November 2015. Bone density scan showed very good improvement so back on Femara - yay!
December 2016. 6 treatments of SBRT radiation on lung. Seems to have had some effect.
June 2016. CA15-3 still stable and low at 9.
June 2016. Started subcutaneous Herceptin replacing infusion.
Jan 2017. LVEF dropped to 46%. Stopped Herceptin.
Feb 2017. Started ACE Inhibitor and BETA Blocker. Still off Herceptin.
Aug 2017. Two new mets - Portacaval lymph node and mediastinal lymph node.
Aug 2017. Blood tests show extremely elevated liver enzyme levels. Many tests to investigate.
Sept 2017. Portacaval lymph node blocking liver bile duct causing liver enzyme and Bilirubin problems.
Oct 2017. 8cm stent inserted into liver bile duct. Procedure caused pancreatitis, and hospitalised for 3 days. Liver enzymes improving rapidly.
Nov 2017. Commenced 4 weeks of radiation on Portacaval lymph node. 5 week break before chemo.
Jan 2018. CT scan. 11 new small liver mets, and new superclavical lymph node med.
Jan 2018. Start Kadcyla. CA15-3 426.
Apr 2018. First scans since starting Kadcyla. All tumours reducing. CA15-3 dropped to 30 from 426.
Dec 2019. Still on Kadcyla, but two small brain mets have been treated in the past month with SRS. CA15-3 stable for 12 months at 11.

Flori, You have been a tower of strength here and you are entitled to a little "wobble" now and then. It is hard with the losses of our sisters lately, I feel that, too- once you reassess with your doctors and get a plan going, I hope it brings you some peace of mind. Take good care. Ceesun

Hi Flori. I think many of us feel the same way about the losses we have had in such a short amount of time. Good days, bad days, hopeful days and hopeless days... We have all been there. I pray that God will lift up your spirits, give you courage. We will continue to be here, support each other, just like our angels did for us. Hugs to you !!!

3 weeks after 2nd surgery, a new nodular rash found near drain incision with 2 small red spots behind the chest wall biopsy on 10/1. Positive for breast cancer
Radiation 11/2014 with xeloda then weekly cisplatin
11/14 brain MRI clean
12/14 finished 33 radiations burnt and very painful. Bedridden for 1 week
12/14 t current Herceptin and perjeta only
02/15 rash on upper back right side skin mets radiation planned
02/15 staring electron radiation *35
Stopped at 30 due to severe skin burn, resumed 10 days later
05/15 red patches appeared in between previously radiated area, skin mets. Ct and brain Mri clear. Simulation planned, radiation to start after trip to Alaska.
05/24 new spot identified in scar line on previously radiated reconstructed breast- electron on both side chest wall area and scar line
07/15 multiple skin and lung recurrence begin halaven
11/15 cough much better but very tired on halaven and starting to see some new red skin blotches-suspicious
11/15 heading to China for immune therapy

Boo! Crap. So stressful. I know that you are discouraged, but why not try Kadcyla - it may suck, or it might not be as atrocious as you are expecting.

Ouch. On the other hand my girlfriend's scan always seem to get misread, mixed up with other peoples, etc.

We've had so much bad news, maybe you are a lucky one up due for some good news.

Were you ever hormone positive? I am just thinking out loud here but hormone positive cancer just loves the bones. So, in thinking out loud, if Perjeta worked well on the highly her2+ cells, then what might be left behind are those that are hormone positive. Hence, they have lots of space to grow. What about testing out an AI or Faslodex shots (that cover the ER receptor) and see if there is an improvement (while staying on everything else you're on of course).

Just an idea. Something may have changed in the bones since ER+ cancer likes them best. If they were always alittle bit hormonal, there might not have been anything to shut them down so preferentially, they grew as Perjeta/Herceptin etc does nothing for that.

PS - this is would be an experiment but not a risky one and your onc might go along for a couple of months

PET scans light up wherever more glucose is being used than normal--in bones the cause can be fracture, infection, inflammation, or cancer.


Now I really don't know if the pattern of your PET or the accompanying CT or your lack of pain or tenderness (I suppose since you didn't say) is such that they can rule out stress fracture or other causes with absolute certainty(or even if there is any such pattern) , but since it is a long-weekend and you may not reach your doctor
immediately, how about imagining it is something nicer for now.

Your ideas of how they might try to decide which it is if there is uncertainty ( trying time--with interval healing if fracture--and denosumab and/or a bone scan sound logical. It might even possible they will do a biopsy if it doesn't become clear with time, I suppose. If they do, and it is breast cancer they may find that it has changed(eg er- to er+_
and there might even now be more targets to aim for.

Aww Flori, it seems like you have just about had it. You are sick at heart with the loss of Sheila, Chrisy, and now Christine being ill. Enough is enough!
And now, with the PET scan results in question - it is the last thing you want to deal with.
I say give yourself a break this weekend if you can. Just kick up your heels and do something out side the box.
Then, when you have a bit more energy, plop yourself down and decide on an action plan. Meet with your doctor - and - follow Becky and Lani's advice. Both are intuitive and both have something that you don't have right now; the energy to think and analyze.
Please tell us what treatment you end up on. You are a gem. We want to hear what your doctor recommends.

Sharing the bad times as well as the good is what makes this forum so special. With recent losses and mixed scan results of your own, it's no wonder you are feeling down. Others have made some good suggestions in regard to those bone mets, so a long chat with your onc might be in order.

Diagnosed 2004: Lumpectomy - 2 tumours, both grade 1 infiltrating duct carcinoma, about 12mm. ER+,
C-erbB-2 status 3+.
Clear margins, no nodal involvement.
Radiotherapy, i year Tamoxifen, 4 years Arimidex.
Rediagnosed 2012: Multiple bone metastases.
3/12: began on Marianne trial - T-DM1 + Pertuzamab/Placebo.
5/12:Unexpected development of numerous bilateral liver mets. Came off trial.
Started Docetaxol/ Herceptin + Zometa.
8/12:Bones stable +major regression in liver (!)
9/12:Can't take any more Docetaxol! Start on Herceptin and Tamoxifen. Cross fingers!
Changed to Denosumab.
11/12: Scan shows stable - yay!
11/13: Still stable :-) !!!
1/16: All stable, but lowered calcium, so switched to Zometa 3 monthly.
2/19: Happily still stable on Herceptin, Letrozole and 3 monthly Zometa.

I understand the sadness - it is very hard to hear about the loss of our sisters, and the inevitable "am I next" questions these losses bring up.

I have no advice for you. I'd probably try the Kadcyla but I've been following it for a long time and have been on chemo for years so don't have any issues with it. It will be my next chemo when gemzar and SBRT fails. But you have to do what you feel is right.

I always feel a bit better when I make a decision, unqualified as I feel to make the ones I have to make. Here's hoping you will too.

5/27/18: Stable. Kadcyla put me right back in the barn. I have two teeny spots on my lungs that are metabolically inactive, and liver is clean.

I’m beating this MFer. I was 51 when this started and had two kids, 22 and 12. Now I’m 60. My oldest got married and trying to start s family. My youngesg graduates from Caltech this June. My stepdaughter gave me grandkids. Life is fantastic.

Hi flori....stay strong and positive....the pet scan always freaks me out until I speak to the dr....I am having one in 2 weeks.....don't assume anything, although I always thing the worst...the pet scan just gives dr areas to lok at more clearly with ct scans and mris..my last pet showed high activity in liver, but mri of liver was more accurate....I know exactly how you feel....eileen

Becky may be on to something. It is certainly worth a try. Brenda was only slightly hormone positive, very slightly, but she had a good response from anti-hormonal therapy.

Hang in there, Flori. You are so right when you say this has been one tough year for Her2Support. We have lost some of our best and brightest stars. It's all one big "ouch."

It's so hard when the results are confusing And we are dealing with the loss of friends. Try to stave off the fear until you see your dr. There are, as you would know, many reasons for things lighting up on PET and you had that funny issue with the last one so try to take a breath, line up your options to discuss and go from there.

I understand your frustration with the pet scan results.
Pet scans have mind of their own.
Take a day or two off and do something "fun". Then go see the doc for a discussion when you are ready for it.
Maybe a chemo adjustment is all you need. Hang in there. Remember you are in this for the long hall. Take good care of yourself.

Im sorry I didn't see this thread sooner. Did you get to talk to the doctor and see exactly what this scan means? Because that definitely is confusing. And yes, its hard to keep our positive fighting attitudes when we are losing so many members of the site this year. You said sometimes you avoid posting because of that, and sometimes I am scared to log in. It has been absolutely heartbreaking, and Im sure even moreso for those of you who have been a member much longer than I have. No matter what you have to post about tho, ups or downs, we will all be here for you, so don't ever hesitate to post and say exactly what you feel. Big hugs to you...update when you can. Sending lots of prayers and positive energy, and hoping you are feeling better soon.

Diagnosed in October 2011 Stage IV with metastasis to liver.
January 2012 after double mastectomy, started taxotere, carboplatin and herceptin.
Clear.
December 2012 was diagnosed with five brain mets, and had whole brain radiation.
Around July 2014 two mets in brain, one a residual spot and one new one growing in size. Received Cyberknife on both areas
