LVEF drop - off herceptin - CoQ10 and?????

Hi Amazing Group,

Well, my lvef dropped again, this time 15 points, so no more herceptin for a month at least. I have been taking CoQ10 since years before this all happened and still take 200 a day. Anything else known out there to help with this heart mending?

The first time my lvef dropped 13 points, but bounced back up on the next test. This time it dropped from 67 to 52. :-(

I have had 8 months of treatment - I remember reading somewhere about a Finnish (?) study that showed 9 treatments were effective - anyone know about this or where it is at in study now?

Hi, Donna. I am also off Herceptin for 4 weeks. My LVEF dropped from 65 to 51%. I'm still getting Taxol (3 doses left), so I can't take CoQ 10 yet. I have an appointment with a cardiologist on Friday. My feet and ankles are very swollen. I'll be having a repeat MUGA scan in 4 weeks to see if I can resume the Herceptin.

3 cm tumor, 8 cancerous lymph nodes
Grade 3, ER/PR -, HER-2/neu +++ (by FISH, average 20 copies per cell)
A/C x 4, Taxol x 12, Herceptin x 47 weeks
35 radiation treatments
Completed treatment on April 3, 2008
Chest port removed October 2009
11 years since diagnosis NED

I had to quit herceptin due to low LVEF too, after 8 months, but I was told European studies showed 6months of herceptin is just as effective as 1 year. So I feel confident that I'm doing good. I feel great and my last muga showed much improvement!

oh, my the way, I tried CoQ10, and I had terrible heartburn from it, and had to discontinue it..........oh well, I tried!

My LVEF was 47 when I was required to take a break. Prior to that it was in the 60's. It went up to 54 and I was allow to complete my last 6 treatments.

I have another MUGA in 2 weeks to see if those treatments lowered my LVEF again.

To ALL:

Important

Try to schedule your MUGA or ECHO as close as possible PRIOR to your scheduled day of receiving Herceptin.

Try to maintain this schedule for all future tests.

When infused, Herceptin will cause an ejection factor drop in all patients, if you are on a triple dose, your drop will be even more dramatic.

By changing your test schedule:

1. Your heart will be at its strongest
2. There will be less fluctuation if done at the same time in the infusion cycle.

Hi All,

I took my last MUGA test on Thursday and had herceptin scheduled for the following Tuesday. I am surprised my oncologist actually called me on Monday to cancel the herceptin, that was quick! I get infusions every three weeks and will be careful with future MUGA's to keep them out from that time.

Thanks for bringing this up.

Donna, as my MUGA was slowly dropping I thought i needed to do soemthing so i could stay on Herceptin. So i joined a gym and stated taking CoQ10 and fish oil. It has improved slightly but more importantly, it hasn't dropped.

Exclamation heart protection INFO

There is a doctor at ucla - he's an MD in their Integrative Oncology Dept. His area of study and expertise is keeping the heart healthy during chemo. He pioneered the work with CO Q 10 and other supplements.

Just as a side note, I have never been able to tolorate vitamins in the past - always gastro probs. I am taking about 20 pills a day - NO PROBS. The brand he recommmends seems very pure - in fact, the co q 10 pills are in chewable form for optimum effect. From Douglas Laboratories.

His name is Doctor Kenneth Conklin. Maybe you can call about a consult or try and find his research on the subject.

2002 recurrence. Bilateral mastectomy w/TFL autologous recon. Then ACx2. Skin lymphatic rash. Taxotere w/Herceptin x4. Herceptin/Xeloda. Finally stops spreading.
2003 - Back to surgery, remove skin mets, and will have surgery one week later when pathology can confirm margins.
‘03 latisimus dorsi flap to remove skin mets. CLEAN MARGINS. Continue single agent Herceptin thru 4/04. NED.
‘04 '05 & 06 tiny recurrences - scar line. surgery to cut out. NED each time.

3/07 Heartbreaking news - mets! lungs.sternum. Try Tykerb/Xeloda. Tykerb/Carbo/Gemzar. Switch Oncs.

3/08 Herceptin/tykerb/avastin/zometa.
3/09 brain NED. Lungs STABLE.
4/09 attack sternum (10 daysPHOTONS.5 days ELECTRONS)
9/09 MARKERS normal!
3/10 PET/CT=manubrium intensely metabolically active but stable. NEDhead.
Wash out 5/10 for tdm1 but 6/10 CT STABLE, PET improving. Markers normal. Brain NED. Resume just Herceptin plus ZOMETA
Dec 2010 Brain NED, lungs/sternum stable. markers normal.
MAR 2011 stop Herceptin/allergy! Go back on Tykerb and switch to Xgeva.
May-Aug 2011 Tykerb Herceptin Xgeva.

April 2012 sketchy drug trial in NYC. 6 weeks later I’m NED!
OCT 2012 PET/CT shows a bunch of freakin’ progression. Back to LA and Herceptin.avastin.zometa.

March 2013 – 5 YEARS POST continue HAPZ
APRIL 2013 - 6 yrs stage 4. "FAILED" PETscan on 4/2/13
May 2013: rePetted - improvement in lungs, left adrenal stable, right 6th rib inactive, (must be PERJETA avastin) sternum and L1 fruckin'worsen. Drop zometa. ADD Xgeva. Doc says get rads consultant for L1 and possible biopsy of L1. I say, no thanks, doc. Lets see what xgeva brings to the table first. It's summer.
June-August 2013HAPX Herceptin Avastin Perjeta xgeva.
Sept - now - on chemo hold for calming tummy we hope. Markers stable for 2 months.
Nov 2013 - Herceptin-Perjeta-Avastin-Xgeva (collageneous colitis, which explains tummy probs, added Entocort)
December '13 BRAIN MRI ned in da head.
Jan 2014: CONTINUING on HAPX…
FEB 2014 PetCT clinical “impression”: 1. newbie nodule - SUV 1.5 right apical nodule, mildly hypermetabolic “suggestive” of worsening neoplastic lesion. 2. moderate worsening of the sternum – SUV 5.6 from 3.8
3. increasing sclerosis & decreasing activity of L1 met “suggests” mild healing. (SUV 9.4 v 12.1 in May ‘13)
4. scattered lung nodules, up to 5mm in size = stable, no increased activity
5. other small scattered sclerotic lesions, one in right iliac and one in thoracic vertebral body similar in appearance to L1 without PET activity and not clearly pathologic
APRIL 2014 - 6 YRS POST GAMMA ZAP, 7 YRS MBC & 18 YEARS FROM ORIGINAL DX!
October 2014: hold avastin, continue HPX
Feb 2015 Cancer you lost. NEDHEAD 7 years post gamma zap miracle, 8 years ST4, +19 yrs original diagnosis.
Continue HPX. Adding back Avastin
Nov 2015 pet/ct is mixed result. L1 SUV is worse. Continue Herceptin/avastin/xgeva. Might revisit Perjeta for L1. Meantime going for rads consult for L1
December 2015 - brain stable. Continue Herceptin, Perjeta, Avastin and xgeva.
Jan 2016: 5 days, 20 grays, Rads to L1 and continue on HAPX. I’m trying to "save" TDM1 for next line. Hope the rads work to quiet L1. Sciatic pain extraordinaire :((
Markers drop post rads.
2/24/16 HAP plus X - markers are down
SCIATIC PAIN DEAL BREAKER.
3/23/16 Laminectomy w/coflex implant L4/5. NO MORE SCIATIC PAIN!!! Healing.

JAN 2019 - ZW25 seems to be keeping me stable
APRIL 2019 - ONE DOZEN YEARS LIVING METASTATIC
MAY 2019 - progression back on herceptin add xeloda
JUNE 2019 - "6 mos average survival" LMD & CNS new single brain met - one zap during 5 days true beam SBRT to cord met
10/30/19 - stable brain and cord. progression lungs and bones. washing out. applying for ds8201a w nivolumab. hope they take me.
12/27/19 - begin ds8401a w nivolumab. after 2nd cycle nodes melt away. after 3rd cycle chest scan shows Improvement, brain MRI shows improvement, resolved areas & nothing new. switch to plain ENHERTU. after 4th cycle, PETscan shows mostly resolved or improved results. Markers near normal. I'm stunned but grateful.
10/26/20 - June 2021 Tucatinib/xeloda/herceptin - stable ish.

One time I actually had the Herceptin scheduled 2 hours after the MUGA. When they got the results (lvef 49), they came in and disconnected the IV and threw it away (*sigh*). But that's about as 'wait until the last minute' as it can get (*laugh*).

May 2008: Heart NORMAL! Yippee.

Hi Flori,

I did check out Dr. Conklin and also the Douglas Labs CoQ10 - and I ordered it! Sounds like the best one out there. Can I ask, does Dr. Conklin recommend all the supplements you take from Douglas Labs or just this one? I will still check out getting a phone consult, but I was just curious - y'know - TODAY. :-) Not the patient type.

Quick update. I was on Herceptin for 7 months when my LVEF dropped below 50% (just to 49%) in May. 3 weeks later, it was 48%. Now I just got my results from a MUGA in July and it has dropped to 44% (with the right side being affected too for the first time). So I'm permanently off Herceptin and will have a MUGA in 3 more months hoping that it's finally going UP instead of down.

The good thing: I've called my surgeon's office to schedule getting my port removed. Poke a fork in me, I'm DONE! (*grin*)
