Kadcyla success

I'm hesitant to post here because I just saw the news about the passing of some sisters (I've been on hiatus), and I didn't want to overshine their courageous battles.

I recently became the first patient at the University of Iowa to receive Kadcyla (TDM1)! And after 6 weeks, we just saw a reduction in my tumor markers and improvement in my pain. Minor victory despite many losses here. Thanks to all the brave women who paved the way for me, and helped organize this site. Peace.

Aug 2010: diagnosed stage 3b, 4 mo. after birth of son. 29 yrs old and breastfeeding, ER/PR-, Her-2+ started Neoadjuvant therapy: 4x FEC, 10x abraxane & Herceptin
Feb 2011: L mx with recon. Path. showed only DCIS but 4/10+ nodes.
March 2011: 6 wks rads.
Mother passed, lower back pain.
Late May 2011: Bone mets but organs clear; Tykerb, Xeloda, Xgeva. Stopped Herceptin. Implant infected: removed implant.
October 2011: Bone progression; Gemzar and Carboplatin & restarted Herceptin.
Jan 2012: Progression, re-classified as ER+; Tykerb, Herceptin, Zoladex & Femara. Anti-E is working!
May 2012: ovaries out, markers stable but elevated. Cont. Herceptin, Tykerb, Xgeva & Femara.
Dec 2012: aromasin
Jan 2013: faslodex, herceptin, tykerb
Jun: Kadcyla
Aug: Rads to hip, then Perjeta, Herceptin & Taxotere
Nov 2013: Perjeta, Herceptin, Halaven
Early 2014: Affinitor, Aromasin, Perjeta, Herceptin.
June 2014: Estradiol, Perjeta, Herceptin
Aug 14: Tamoxofin, H & P

I've been sad about the difficulties some are having and the loss of some very important people to our group. So hearing your improvement from Kadcyla is singing to my ears. I am so pleased for you.

MY STORY SO FAR ~~~~
Found suspicious lump 9/2000
Lumpectomy, then node dissection and port placement
Stage IIB, 8 pos nodes of 18, Grade 3, ER & PR -
Adriamycin 12 weekly, taxotere 4 rounds
36 rads - very little burning
3 mos after rads liver full of tumors, Stage IV Jan 2002, one spot on sternum
Weekly Taxol, Navelbine, Herceptin for 27 rounds to NED!
2003 & 2004 no active disease - 3 weekly Herceptin + Zometa
Jan 2005 two mets to brain - Gamma Knife on Jan 18
All clear until treated cerebellum spot showing activity on Jan 2006 brain MRI & brain PET
Brain surgery on Feb 9, 2006 - no cancer, 100% radiation necrosis - tumor was still dying
Continue as NED while on Herceptin & quarterly Zometa
Fall-2006 - off Zometa - watching one small brain spot (scar?)
2007 - spot/scar in brain stable - finished anticoagulation therapy for clot along my port-a-catheter - 3 angioplasties to unblock vena cava
2008 - Brain and body still NED! Port removed and scans in Dec.
Dec 2008 - stop Herceptin - Vaccine Trial at U of W begun in Oct. of 2011
STILL NED everywhere in Feb 2014 - on wing & prayer
7/14 - Started twice yearly Zometa for my bones
Jan. 2015 checkup still shows NED
2015 Neuropathy in feet - otherwise all OK - still NED.
Same news for 2016 and all of 2017.
Nov of 2017 - had small skin cancer removed from my face. Will have Zometa end of Jan. 2018.

Fantastic Kristin! Great news!

I have had three cycles now, so I have scans in two weeks to see if it is working. Fingers crossed. Unfortunately, I don't have tumor markers to give me a hint!

World Trade Center Survivor (56th Floor/North Tower): 14 years and still just like yesterday.
Graves Disease, became Euthyroid via Radioactive Iodine, June 2001.
Thyroid Eye Disease. 2003. Decompression surgery in 2009; eyelid lowering surgery in 2010.
Diagnosed: June 2010, liver mets. ER-/PR+10%; HER2+++.
July 2010: Begin Taxol/Herceptin. Eliminate sugar from diet. No surgery or radiation.
January 2011: NED
April 2011: Progression in liver only. Other previous affected areas eradicated. Stop Taxol/Herceptin after 32 infusions.
May 2011: Brain MRI: clear.
May 2011: Begin Tykerb daily, Xeloda twice per day for one week on, one week off, and Herceptin.
November 2011: Progression in liver. All other tumors remain eradicated.
December 2011: BEGIN TRIAL #09-093 Taxol, MCC-DM1 (T-DM1), Perjeta.
Trial requires scans every six weeks, bloodwork and infusions weekly.
Brain MRI: clear.
January 2012: NED. Liver mets, good riddance!
March 2012: NED. Developed SMA (rare blood clot) in intestinal artery and loss of sight in right eye due to optical nerve neuropathy. Resolved when Taxol removed this month.
Continue Protocol of T-DM1 weekly and Perjeta every 3 weeks.
May 2012: NED.
June 2012: Brain MRI: clear.
June-December 2012: NED.
December 2012: TRIAL CONCLUDED; ENTER TRIAL EXTENSION #09-037. CT, Brain MRI, bone scan: clear. NED.
January-March 2013: NED.
June 2013: Brain MRI: clear. CEA upticking; CT shows new met on liver.
July 3, 2013: DISASTER STRIKES during liver ablation: sloppy surgeon cuts intercostal artery and I bleed out, lose 3.5 liters of blood, have major hemothorax, and collapsed lung requiring emergency resuscitative thoracotomy, lung surgery, rib rearrangement and cutting deep connective tissue, transfusion. Ablation incomplete. This life-saving procedure would end up causing me unforgiving pain with every movement I make, permanently, otherwise known as forever.
July 26, 2013: Try Navelbine/Herceptin. Body too weak after surgery and transfusion. Fever. CEA: Normal.
August 16, 2016: second dose Navelbine/Herceptin; CEA: Normal. Will skip doses. Watching and waiting.
September 2013: NED, Herceptin only. CEA: Normal. Started Arimidex.
October-November 2013: NED. Herceptin and Arimidex. CEA, CA125, 15-3: Normal.
December 2013: Something brewing. PET lights up on little spot on liver; CEA upward trend, just outside normal. PET and triphasic liver scan confirm Little Met. Restart Perjeta with Herceptin, stay on Arimidex. Genomic sequencing completed for future treatments, if necessary.
January 2014: Ablate Little Met on the 6th. Happy New Year.
March 2014: Brain MRI: clear. PET/CT reveal liver mets return; new lung mets. This is not funny.
March 2014: BEGIN TRIAL #10-005 A(11)-Temsirolimus plus Neratinib.
April 2014: Genomic testing indicated they could work, they did not. Very strange drug combo for me, felt weird.
April 2014: Started Navelbine and Herceptin. Needed something tried and true, but had significant progression.
June 2014: Doxil and Herceptin.
July 2014: Progression. Got nothing out of it. Brain: NED.
July 2014: Add integrative medical hematologist-oncologist to my team. Begin supplements. These are tumor-busting, immune system boosters. Add glutathione, lysine and taurine IV infusions every three weeks.
July 2014: Begin Gemzar, Herceptin & Perjeta. Happy.
August 2014: ECHO perfect.
January 2015: Begin weekly Vitamin D Analog infusions. 25 mcg. via port.
February 2015: CT: stable.
April 2015: Gem working, but not 100%. Looking into immunotherapy. Finally, treatments for the 21st century!
April 2015: Penn Medicine. Dendritic cell immunotherapy.

dx Dec 2000 dcis 2.5cm clear sentinel node, ER/PR- Her-2+
lumpectomy, 6 cycles AC, 6 weeks rads
October 2007 three x 2.5cm lung mets. 8 months Taxol, started Herceptin and continue. Significant reduction in lung mets.
June 2011 3cm x 4cm liver tumour. Started Abraxane and continue with Herceptin.
November 2011. Finished with Abraxane, continue with just Herceptin. Liver tumour now reduced to 15mm x 12mm. Lung tumour now 10mm x 0.5mm
February 2012. Scans show everything stable, and brain scan clear.
July 2012. PET/CT scans show I'm in remission - no active cancer!
]Dec CT brain cllear, lungs stable, liver tumour has increased to 20mm. PET scans showed active liver met and active lung thinglet, and possible bone met.
Jan 2013 recommence Abraxane, continue with Herceptin.
June 2013 finish Cycle 6 Abraxane, continue with Herceptin. 30% reduction in liver tumour, everything stable.
December 2013. CA15-3 on rise.
February 2014. PET and CT scans show single liver tumour has increased to 35mm. No other activity.
March 2014. Planned for SBRT for liver met, but couldn't have treatment as tumour too close to bowel. Continue Herceptin.
April 2014. Surgeon advises that I am a good candidate for liver resection, so will have operation early May (after camping holiday). Tumour now 44mm x 29mm.
May 7, 2014. Two liver tumours surgically removed. Third of liver removed, and gall bladder. Am I NED?May 2014. Pathology of tumour shows it's now ER+ (95% staining).
June 2014. CA15-3 has decreased to 18 from a pre-surgery reading of 59!
June 2014. Started Femara, continue with Herceptin.
July 2014. Stop Femara due to severe Osteoporosis. Commence Tamoxifen, continue Herceptin. Waiting to hear if I can have Aclasta infusion.
August 2014. CA15-3 has decreased further to 12 - YAY!
October 2014. Aclasta infusion for Osteoporosis. November 2014, CA15-3 decreased to 11. Scans of liver all clear, something new showing up on lung, but just watching at the moment.
November 2015. Started SBRT on solitary lung met.
November 2015. Bone density scan showed very good improvement so back on Femara - yay!
December 2016. 6 treatments of SBRT radiation on lung. Seems to have had some effect.
June 2016. CA15-3 still stable and low at 9.
June 2016. Started subcutaneous Herceptin replacing infusion.
Jan 2017. LVEF dropped to 46%. Stopped Herceptin.
Feb 2017. Started ACE Inhibitor and BETA Blocker. Still off Herceptin.
Aug 2017. Two new mets - Portacaval lymph node and mediastinal lymph node.
Aug 2017. Blood tests show extremely elevated liver enzyme levels. Many tests to investigate.
Sept 2017. Portacaval lymph node blocking liver bile duct causing liver enzyme and Bilirubin problems.
Oct 2017. 8cm stent inserted into liver bile duct. Procedure caused pancreatitis, and hospitalised for 3 days. Liver enzymes improving rapidly.
Nov 2017. Commenced 4 weeks of radiation on Portacaval lymph node. 5 week break before chemo.
Jan 2018. CT scan. 11 new small liver mets, and new superclavical lymph node med.
Jan 2018. Start Kadcyla. CA15-3 426.
Apr 2018. First scans since starting Kadcyla. All tumours reducing. CA15-3 dropped to 30 from 426.

So here I go again, Kadcyla lasted about 4 months and then we saw tumor markers increase and pain in my foot take off. It turns out that the cancer was pressing on the SI joint and causing foot problems (pain, burning, throbbing, stabbing, pins and needles), so 10 cycles of radiation later- we're going to be looking at Perjeta with taxotere (bye bye hair). So thankful I still have this Ace up my sleeve, but sooooo worried about what if this doesn't work and what has the cancer been doing in the past 17 days I've been off chemo. Such short lived celebration with Kadcyla, hoping that Perjeta is the one.

Sorry to hear TDM-1 hasn't given long lasting improvement, and hope the rads have taken care of the pain. Taxotere is one powerful cocktail and, combined with Perjeta, let's hope it gives you a longer period of stability.

Diagnosed 2004: Lumpectomy - 2 tumours, both grade 1 infiltrating duct carcinoma, about 12mm. ER+,
C-erbB-2 status 3+.
Clear margins, no nodal involvement.
Radiotherapy, i year Tamoxifen, 4 years Arimidex.
Rediagnosed 2012: Multiple bone metastases.
3/12: began on Marianne trial - T-DM1 + Pertuzamab/Placebo.
5/12:Unexpected development of numerous bilateral liver mets. Came off trial.
Started Docetaxol/ Herceptin + Zometa.
8/12:Bones stable +major regression in liver (!)
9/12:Can't take any more Docetaxol! Start on Herceptin and Tamoxifen. Cross fingers!
Changed to Denosumab.
11/12: Scan shows stable - yay!
11/13: Still stable :-) !!!
1/16: All stable, but lowered calcium, so switched to Zometa 3 monthly.

Hi Kristin I wish Kadcyla had worked for you longer than a few months ! Good news is all your major organs are clear !! I think we just have to look on the positive side to get through these treatments. I remember Barbara kept on progressing until she got on Perjeta, which has kept cancer at bay from what Barbara's husband's update on her. I too have the fear seeing the weapons in my arson is getting less as i am going from A to B and to C. Its like you have 10 keys but only 1 will get you in the house. It may take time to find that key, but once you find it, you will be fine, for a long time ! Hope you will find that key very SOON !

07/15 multiple skin and lung recurrence begin halaven
11/15 cough much better but very tired on halaven and starting to see some new red skin blotches-suspicious
11/15 heading to China for immune therapy

That is Awesome.

I have been on it since Jan 2013. First 3-4 infusions I didnt see improvement and was still losing weight but since April, Ive gained back 15-20 pounds!!! I feel so much better, no longer gray, gaunt and ill. Fingers crossed its working. Scans in Sept.
We took a Scan holiday. Initially bc my research hospital did not have it in the pharmacy so if I had enough progression or something wonky on scan the trial may of thrown me off, however trial (TERESA Crossover) no longer required any scans but echo after I went into crossover.
Left Trial in JUne to go back to my community hospital for treatment.Since I feel well and can eat and gain weight no one is in a hurry to scan and look for problems. I am finally getting strength back. I am almost out of tx options and well Perjeta/Taxotere left me ill 21/21 day cycle and I lost 10 pounds every cycle and was in severe pain no Quality of life.
Feel as close to normal on this drug .

Kadcyla Extends Survival in Heavily Pretreated Advanced HER2-Positive Breast Cancer
Kadcyla™ (ado-trastuzumab emtansine, formerly known as T-DM1) extends progression-free survival in women with advanced HER2-positive breast cancer that progressed on two or more previous HER2-directed therapies, according to the results of a study presented at the ESMO 2013 Congress of the European Society for Medical Oncology in Amsterdam.
HER2 is a protein involved in normal cell growth. Approximately 20-25% of breast cancers overexpress (make too much of) the HER2 protein, and this over-expression contributes to cancer cell growth and survival. HER2-targeted therapies such as Herceptin have dramatically improved outcomes for women with HER2-positive breast cancer, but researchers continue to explore new approaches to treatment.
Kadcyla is part of a new class of drugs called antibody-drug conjugates, which consist of an antibody attached to a toxic chemotherapy. It combines Herceptin and a chemotherapy drug (emtansine or DM1) that interferes with cancer cell growth. Kadcyla delivers Herceptin and DM1 directly to HER2-positive cells, and limits exposure of the rest of the body to the chemotherapy.
After more than two HER2-directed treatment regimens for metastatic breast cancer, there is no clear standard of care for patients with progressive disease. TH3RESA is a phase 3 study that included 602 patients with advanced breast cancer previously treated with at least two HER2-directed therapies. Patients were randomly assigned in a 2:1 ratio to Kadcyla or physician’s choice of chemotherapy (83% received Herceptin-based regimens and 17% received chemotherapy). Treatment was continued until disease progression, at which point patients were allowed to crossover to Kadcyla.
Patients in the Kadcyla group had a median progression-free survival of 6.2 months—nearly double that of the patients in the control arm (3.3 months). This was a highly significant difference in favor of Kadcyla. A pre-specified subgroup analysis showed a consistent, near-doubling of progression-free survival with Kadcyla regardless of subgroup (age, geographic area, race, performance status, tumor characteristics, and visceral disease).
Median overall survival has not yet been reached in the Kadcyla group; median overall survival was 40.9 months in the control group. This suggests that there may be a survival advantage for Kadcyla, but long-term confirmation is needed. Final survival results are expected in 2015.
Adverse events of grade 3 or higher were more frequent in the control group (43.5%) than in the Kadcyla group (32.3). Adverse events leading to discontinuation of therapy occurred in 10.9 percent of controls compared with 6.7 percent of the Kadcyla group.
The researchers concluded that Kadcyla resulted in a statistically significant improvement in progression-free survival, with fewer grade 3 adverse events than other treatments in patients previously treated with two or more HER2-directed regimens for HER2-positive metastatic breast cancer.

Hi Kristin, So glad to hear you are doing well.


I'm a senior on this site and continue with treatments at the moment, also have had some issues with bone involvement but at the present time everything is stable for me. Can't complaint about anything and I won't!! It's almost 20 years since my initial diagnosis, so I'm pleased and grateful to continue receiving treatment from my local hospital and also willing to once again participate in another clinical trial when needed.
