Smile Making switch "again"

Hi everyone - have not been posting much this year but always have my eye on how everyone is doing. I have been feeling terrible since the holidays and had a difficult time shaking sinus issues and feeling very very down and tired. Am finally getting past that I hope. My taste buds died about 2 weeks into this combo (which I thought was very strange. This has never happened to me before so early on) I have lost about 10 pounds due to such bad taste. The pounds were welcome but not the lost of taste.

I started new combo in Dec. (Herceptin/Avastin/Taxotere) and was just scanned last Friday. It seems nothing much is happening and my CEA has elevated , tumors about same size but more uptake so we decided to stop and start on IXEMPRA/XELODA beginning this Friday. I think my cancer has found its way around taxanes. They do not seem to work for any longer. My onc. office is opening the Herceptin/DM1 trial but due to my having a little too much Adriamycyn back in 1999 mind you, I am excluded. I was so excited since we found out that Genetech was signing an amendment to the exclusion for more then 3 chemos., but now this.

Anyway, my next option is the Ixempra so we will give that a try.

If anyone is on this combo - can you please let me know how you are doing and the side effects you are having. I hear it is a pretty good drug but not much known since it is so new.

Hello Sweetheart

As you PM stated, "I Love You" and I have missed you terribly. I am sorry your in this combo is not your magic potion, but I am praying the next course of treatment is. Cancer is a smart disease and can grow resistant to our ammo but thankfully we have many more options today. Also keep in mind even with that possibility, these can still be vital combinations in our futures. Ed's doctor and Dana Farber were considering him going back on past failures in the future as new combinations are born. WHY I asked, they stopped working in the beginning. Dana Farber said this was not a concern, that much has happened in research for it to be of concern. Still a bit hesitant since there are many things we have not tried and do not feel we are desperate enough to use them again just yet. Not to mention I am praying this is HIS magic potion.

There are a several ladies in chemo that are on or going to be on these drugs in the same combo. Of course you know I will be following them closely for all of us, but especially for you Irene. There are a few ladies here on Ixempra but since it is the end of my day, I cannot remember in what combination. Kareng, Lopsided...

I shall continue to have you close to heart as you enter this part of your journey. Oh, what is...."A little too much Adriamycin??" Ed is going to get 12 total doses, but we are using them in a different than normal manner for BC. You can get more than the 4-6 doses, I have a friend who has been on it in smaller doses for years for another cancer. MY CONCERN: Excluding him from future trials.

Oh Irene, it was so very good to hear from you, but not the news! I was so hoping that the current concoction was IT!!! I guess the right one is waiting to be found this time. I sure hope your sinuses behave and your new treatment is kind to you. Wishing you only the quickest positive response to your new meds and the very best of feeling like a million. irene.. which herceptin -dm1 trial were you excluded from..
the phase 2 (every 3 week trial) or the newer (once a week trial)

the once a week trial is expanding to a full 100 patients from what i here with fewer restrictions. than the 3 week trial..

what is the name of your treatment facility in florida.

The Herceptin/DM1 trial is the Phase II (I believe this is the every 3 weeks)

I was previously excluded due to the fact that I had had so many different chemos (over 3) I found out now that that was amended but one of the exclusions is the Adria. Apparently I must have had a little over what is allowed in this trial. All this back in 1996 too. this was my first chemo.

I just found out today that I was excluded for this. This of course is very disappointing.

My oncs office is: BAY AREA ONCOLOGY and I know the trial cordinator was really trying hard to get me accepted. It do know that the Phase I is less strict but they are all out of Florida which would make it difficult to travel on a weekly basis. I believe the closes I found was in Nashville, TN and when I called they were not accepting any more patience's.

I will keep checking for more trials and also on this Her/DM1 to see if anything changes.

Meantime I need to begin something so hopefully this will keep me at the least stable for awhile.

This board is such a big help I just can't tell you. When I need new information , I just can't wait to get on line with my sisters/brothers because I know they will be there for me.

I, too, don't post regularly and I'm always so scared to find news about us.

I'm so sorry about the progress of this. Don't get tired, keep fighting for all the treatments you can. Your "voice" in the mail sounds like you are very fatigued, but just don't get tired. Keep looking for answers.

mastectomy right breast chemo before surgery herceptin/carboplatin/taxotere ,clear and surgery have radiation 20, `& then herceptin and tamoxifen
NED until Aug/07 body only then 'n June 04-06-07 .1 lesion of 1.6 cm on cerebellum ...novalis ,open sugery
5m.m brain met again novalis, 4mm.In the liver. Waiting 2 months now 3 tumors enroll on T-MCC trial start first infusion Nov 5/07 at Dec 17 scan show one tumor despair the 2nd and 3th diminish Doc said great results until March/08 ct scan show progression

Irene, I can relate to the tast buds "dying", and for me it was during Taxotere also. We switched to Taxol and I got my appetite back, so I'm sure it was Taxotere. Now just have to get my skin and nodes under control, or I'll probably be switching also. I'll be watching for your posts on Ixempra, as that will probably be next for me too...

Herceptin/Navelbine weekly till NED August 2001, then maintenance Herceptin. Right Mast. April 2002. Local/Regional recurrence April '04, Herceptin plus/minus chemo until May '07. Gemzar added from Feb.'07-April '07; Tykerb/Abraxane until August '07, back on Herceptin plus Taxotere and Xeloda Sept. '07. Stopped T/X Nov. '07, stopped Herceptin Dec. '07, started Avastin/Taxol/Carboplatin Dec. '07. Progression in chest skin, stopped TAC March '03, started radiation.

Herceptin has served as the "Backbone" of my treatment strategy for over 6 years, giving me great quality of life. In 2005, I was privileged to participate in the University of Washington/Seattle HER2 Vaccine Trial.

I hope this new combination is so tolerable that you bounce right back. I'm curious, Irene, how much adria did you have? I'm still hoping that a trial of herceptin-dm 1 will take you and it will be convenient. What about fractionated cyberknife like Esther is doing?

I am so sorry that the latest combo did not do its job, but I know you will find the right one...you are truly an inspiration to me, you are such a trooper with a great attitude....you are usually one step ahead of me on treatments...I have learned much....just wish this nasty disease would cut you some slack, and give you a break.....you are in my heart and prayers dear friend....I know you will overcome this setback.

Well poo on that.
Interesting combo you are trying - and I think you and your onc are right to be monitoring this so closely. I think of you every day, and hope that this one will change the direction for you - it may well do so, being a completely new agent that those complacent cancer cells haven't seen yet. Remember how the new combo of tykerb/xeloda shocked them - I hope that will happen with this new pair.

(Inspired by Lily's bilingual post and hoping to make you laugh. I hope you understand Yiddish. But if not - I will try to translate. I think what I said was "Oh-oh my friend. Woe is me and yuck yuck yuck. Tell that cancer to go kiss my tush").

Also - I know that a chronic sinusitis can affect your taste buds. I also know that a virus can make you loose your sense of taste but that it can also return - even after a whole year.

2002 recurrence. Bilateral mastectomy w/TFL autologous recon. Then ACx2. Skin lymphatic rash. Taxotere w/Herceptin x4. Herceptin/Xeloda. Finally stops spreading.
2003 - Back to surgery, remove skin mets, and will have surgery one week later when pathology can confirm margins.
‘03 latisimus dorsi flap to remove skin mets. CLEAN MARGINS. Continue single agent Herceptin thru 4/04. NED.
‘04 '05 & 06 tiny recurrences - scar line. surgery to cut out. NED each time.
1/2006 Rads again, to scar line. NED.

3/07 Heartbreaking news - mets! lungs.sternum. Try Tykerb/Xeloda. Tykerb/Carbo/Gemzar. Switch Oncs.
12/07 Herceptin.Tykerb. Markers go stable.
2/8/08 gamma knife 13mm stupid brain met.
3/08 Herceptin/tykerb/avastin/zometa.
3/09 brain NED. Lungs STABLE.
4/09 attack sternum (10 daysPHOTONS.5 days ELECTRONS)
9/09 MARKERS normal!
3/10 PET/CT=manubrium intensely metabolically active but stable. NEDhead.
Wash out 5/10 for tdm1 but 6/10 CT STABLE, PET improving. Markers normal. Brain NED. Resume just Herceptin plus ZOMETA
Dec 2010 Brain NED, lungs/sternum stable. markers normal.
MAR 2011 stop Herceptin/allergy! Go back on Tykerb and switch to Xgeva.
May-Aug 2011 Tykerb Herceptin Xgeva.
Sept 2011 Tykerb, Herceptin, Zometa, Avastin.
April 2012 sketchy drug trial in NYC. 6 weeks later I’m NED!
OCT 2012 PET/CT shows a bunch of freakin’ progression. Back to LA and Herceptin.avastin.zometa.
12/20/12 add in PERJETA!
March 2013 – 5 YEARS POST continue HAPZ
APRIL 2013 - 6 yrs stage 4. "FAILED" PETscan on 4/2/13
May 2013: rePetted - improvement in lungs, left adrenal stable, right 6th rib inactive, (must be PERJETA avastin) sternum and L1 fruckin'worsen. Drop zometa. ADD Xgeva. Doc says get rads consultant for L1 and possible biopsy of L1. I say, no thanks, doc. Lets see what xgeva brings to the table first. It's summer.
June-August 2013HAPX Herceptin Avastin Perjeta xgeva.
Sept - now - on chemo hold for calming tummy we hope. Markers stable for 2 months.
Nov 2013 - Herceptin-Perjeta-Avastin-Xgeva (collageneous colitis, which explains tummy probs, added Entocort)
December '13 BRAIN MRI ned in da head.
Jan 2014: CONTINUING on HAPX…
FEB 2014 PetCT clinical “impression”: 1. newbie nodule - SUV 1.5 right apical nodule, mildly hypermetabolic “suggestive” of worsening neoplastic lesion. 2. moderate worsening of the sternum – SUV 5.6 from 3.8
3. increasing sclerosis & decreasing activity of L1 met “suggests” mild healing. (SUV 9.4 v 12.1 in May ‘13)
4. scattered lung nodules, up to 5mm in size = stable, no increased activity
5. other small scattered sclerotic lesions, one in right iliac and one in thoracic vertebral body similar in appearance to L1 without PET activity and not clearly pathologic
APRIL 2014 - 6 YRS POST GAMMA ZAP, 7 YRS MBC & 18 YEARS FROM ORIGINAL DX!
October 2014: hold avastin, continue HPX
Feb 2015 Cancer you lost. NEDHEAD 7 years post gamma zap miracle, 8 years ST4, +19 yrs original diagnosis.
Continue HPX. Adding back Avastin
Nov 2015 pet/ct is mixed result. L1 SUV is worse. Continue Herceptin/avastin/xgeva. Might revisit Perjeta for L1. Meantime going for rads consult for L1
December 2015 - brain stable. Continue Herceptin, Perjeta, Avastin and xgeva.
Jan 2016: 5 days, 20 grays, Rads to L1 and continue on HAPX. I’m trying to "save" TDM1 for next line. Hope the rads work to quiet L1. Sciatic pain extraordinaire :((
Markers drop post rads.
2/24/16 HAP plus X - markers are down
SCIATIC PAIN DEAL BREAKER.
3/23/16 Laminectomy w/coflex implant L4/5. NO MORE SCIATIC PAIN!!! Healing.
APRIL 2016 - 9 YRS MBC
July 2016 - continue HAP plus Xgeva.
DEC 2016 - PETCT: mets to sternum, lungs, L1 still about the same in size and PET activity. Markers not bad. Not making changes if I don't need to. Herceptin/Perjeta/Avastin/Xgeva

December 2017 - Progression - gonna switch it up
FEB 2018 - Kadcyla 3 cycles ---->progression :(
MAY30th - bronchoscopy, w/foundation1 - her2 enriched
Aug 27, 2018 - start clinical trial ZW25
JAN 2019 - ZW25 seems to be keeping me stable
APRIL 2019 - ONE DOZEN YEARS LIVING METASTATIC
MAY 2019 - progression back on herceptin add xeloda
JUNE 2019 - "6 mos average survival" LMD & CNS new single brain met - one zap during 5 days true beam SBRT to cord met
10/30/19 - stable brain and cord. progression lungs and bones. washing out. applying for ds8201a w nivolumab. hope they take me.
12/27/19 - begin ds8401a w nivolumab. after 2nd cycle nodes melt away. after 3rd cycle chest scan shows Improvement, brain MRI shows improvement, resolved areas & nothing new. switch to plain ENHERTU. after 4th cycle, PETscan shows mostly resolved or improved results. Markers near normal. I'm stunned but grateful.
10/26/20 - June 2021 Tucatinib/xeloda/herceptin - stable ish.
