I found this provoking from the original study Lani posted:

Now..does the more recent abstract mean the scale has been tipped towards even using CSFs prophylactically in some cases?
"Meta analysis from randomized trials shows that granulocyte colony-stimulating factor prophylaxis is associated with patients receiving more intensive chemotherapy, having better survival, but also having a higher risk of secondary acute myeloid leukemia."

It doesn't say whether they analyzed adjuvant, metastatic or both settings.
I'd love to see the full text...when it comes out.
