Survey problems

I took the survery a couple of weeks ago. I received an email from them a few days after submitting my information. The email contained a passwork and link to the actual survey.

I have tried three times to take the survey. I thought that they would either email you the survey, or a window would open so that you could take the survey.

Nothing happens. Any suggestions?
